This meta-analysis aimed to assess MPs' role in modulating aging biomarkers and longevity, analyzing 33 studies (n=1400 observations) from 2010-2025, selected via PRISMA from PubMed, Scopus, Web of Science, and eLibrary. MPs significantly increased reactive oxygen species (ROS, SMD=0,56; 95% CI 0,45-0,67; p<0,001), inflammation (IL-6, TNF-α; SMD=0,52; 95% CI 0,39-0,65; p<0,001), and cellular senescence (p16, p21; SMD=0,44; 95% CI 0,30-0,58; p<0,001). In humans (n=783), MPs accumulated in the brain (up to 4806 μg/g), correlating with dementia (OR 3,2; 95% CI 2,5-4; p<0,05). In models (n=617), lifespan decreased by 12-20% (SMD=-0,51; p<0,001). Heterogeneity (I²=56-65%) stemmed from MP types and exposure; no publication bias was detected (Egger's test, p=0,32). At the same time, legislation on the elderly, even the relatively new Law on Social Services, which entered into force on 01.01.20215, sees them as needing care, weak, and lacking the necessary information skills. The social group of the elderly makes up a significant part of the population of St. Petersburg (more than 20% according to 2023/2024). Understanding the importance of this social group for the city of the region is reflected in infrastructure development plans and social policy aimed at improving conditions for the elderly in St. Petersburg, including access to services in digital form. The most in-demand digital service today is the Unified Map of St. Petersburg, which was used to conduct a survey of older users in the summer of 2024. The number of elderly people using a variety of digital information resources is growing rapidly, and at the same time, the number of elderly people who are excluded from the digital world or have only minimal digital skills. Research design: longitudinal, non-interventional; type: case study; duration 6,5 years. The general sample included patients of all ages (n=1261) with impaired carbohydrate metabolism registered in the specialized local registry of the developed software Automated Workplace of an Endocrinologist for outpatient admission of patients with DM (AWPE DM). A group of elderly patients with T2DM with comorbid pathology (n=462) was selected from the sample in order to develop modules for a medical decision support system based on an analysis of clinical and metabolic parameters. In the course of the study, the following elements of the personalization system were developed: a formalized protocol for outpatient consultation of a patient with diabetes; AWPE DM is a computer program that allows the collection and storage of primary patient medical information in digital format in an outpatient setting.; a database (local registry) that provides the systematization and accumulation of medical information in the form of structured electronic medical documents; a comprehensive medical decision support system that allows you to personalize the therapeutic tactics of managing comorbid elderly patients with T2DM based on embedded algorithms. Causal events leading from endothelial dysfunction to the development of atherosclerosis are associated with age-related changes and aging. In this regard, much attention is paid to the study of biological aging in humans, which will stimulate the development of new therapeutic strategies. The prevalence of cardiovascular diseases (CVD) increases significantly with age. Risk factors and predictors of cardiovascular events, such as hypertension, atherosclerosis, diabetes, are observed with greater frequency in older people. The main factor in vascular aging is endothelial dysfunction, characterized by impaired endothelium-dependent signaling processes. Vascular endothelium plays a fundamental role in the regulation of vascular tone and structure, maintaining vascular homeostasis through complex interactions with cells in the wall and lumen of the vessel, balancing the production of vasodilators, the main one of which is nitric oxide (NO). Endogenously generated NO has, in addition to paracrine, an endocrine function, causes a remote physiological reaction in various organs and stimulates cellular signaling. Endothelial dysfunction is defined as combined changes in the endothelial phenotype that contribute to the development of atherosclerosis and acute coronary syndromes. The findings demonstrate that AI models achieve high diagnostic accuracy in the automated classification of radiographic images, comparable to dual-energy X-ray absorptiometry. AI reduces subjectivity in image interpretation, although further standardization, dataset expansion, and development of explainable models are necessary. A total of 150 women aged 63-74 years who had experienced two or more falls within 12 months (postural instability) were examined. The study group included women who were found to have a fear of falling. The comparison group included women without the fear. To assess the components of postural balance, the Sensory Organization Test (SOT) and Motor Control Test (MCT) on the Smart Equitest Balance Manager computer complex of dynamic posturography were performed. Analysis of the SOT and MCT results revealed that elderly women with a fear of falling experienced decreased general postural stability and automatic postural control, reduced vestibular information in balance control, and increased time of coordinated motor reactions. In addition, this group of women showed an increase in the speed and area of oscillatory movements of the center of gravity while maintaining static-dynamic balance. The obtained results of the study indicate that the development of fear of falls in elderly women with postural instability is a consequence of the deterioration of not only their sense of postural stabilization, but also orientation, leading to reduced spatial relationships with the external environment, which significantly increases the risk of subsequent falls. The prospective, controlled, randomized study included 78 patients diagnosed with primary and secondary gonarthrosis stage III-IV, elderly patients (75-90 years). Surgery - knee arthroplasty, anesthesia during surgery - combined spinal-epidural anesthesia. Randomization was performed into 2 groups - propofol was used as a sedative during the intraoperative period and dexmedetomidine was used as a sedative during the intraoperative period. The degree of sedation depth was assessed using the Ramsey sedation scale, to monitor the depth of drug-induced sleep, we used a bispectral index device - BIS monitoring. Cognitive status was assessed using the Hamilton Depression Scale at admission and on day 2 after surgery. Intraoperative hypotension was more common in the group of patients with propofol sedation, and intraoperative bradycardia was more common in the group of patients with dexmedetomidine sedation. The cross-sectional study included 150 participants aged 60 and older, selected through stratified random sampling in 2016. Participants were required to have sufficient cognitive abilities and willingness to complete the questionnaires. Data analysis was performed using independent t-tests, ANOVA, and structural equation modeling (SEM) in R software. The findings showed that 54% of participants were male, with an average age of 63,74 years, and most had a primary or middle school education. A significant relationship was found between education level and life satisfaction (p<0,001). Structural equation modeling revealed that happiness had a significant positive effect on life satisfaction (β=0,35; p<0,05). Additionally, living standards had a positive direct effect on both life satisfaction (β=0,46; p<0,001) and happiness (β=0,81; p<0,001). The study concludes that improving access to education, even later in life, can enhance life satisfaction among the elderly. Promoting social and recreational programs to boost happiness and strengthening economic support and living conditions are essential for improving well-being. Integrating these policies can provide a more supportive environment for aging populations. Application of an extended complex of diagnostic techniques in ophthalmologic practice in order to expand the possibilities of early diagnosis and prognosis of the disease is urgent. Screening examination of patients with AMD was carried out. The technology of registration of global rhythmic activity of the brain with narrow-band filtering and long summation time was applied with the allocation of mainly long-lasting rhythmic oscillations in the frequency spectrum of 0,1-30 Hz. The peculiarities of the state of autonomic receptors of the eye in AMD have been determined. The prognostically significant signs of changes in the spectrum of the brain activating system at early and late stages of AMD were identified. The information characteristic of the weight of the sign of AMD development was obtained; that allowed to distinguish the ophthalmologic status of patients with AMD.
