{"title":"Development of Proactive Lane-change Assistant Strategy based on Infrastructure Cooperation for Managing Mixed Traffic in Connected Environments","authors":"Young JO, Cheol OH","doi":"10.7470/jkst.2023.41.5.607","DOIUrl":"https://doi.org/10.7470/jkst.2023.41.5.607","url":null,"abstract":"차량과 인프라 간의 V2X 무선통신이 가능한 커넥티드(connected) 환경에서 위험 상황을 사전에 회피하기 위한 선제적 교통관리전략 수립이 요구된다. 자율차와 비자율차의 안전한 도로주행을 위한 거동제어 및 의사결정을 지원하는 교통정보 제공은 인프라 협력 환경에서 현실화 될 수 있다. 따라서 본 연구의 목적은 커넥티드 환경에서 자율 및 비자율 차량이 혼재된 교통류 관리를 위한 인프라 협력형 선제적 차로변경지원 정보 제공 전략(Proactive LanechangE Assistant Strategy for Automated iNnovative Transportation, PLEASANT)을 개발하고 평가하는 것이다. The aim of this study was to determine whether the use of these drugs affects the likelihood of surgery in patients with chronic subdural hematoma (CSH), with consideration of other factors. Methods This study analyzed clinical and radiological data of 348 patients who were diagnosed with CSH between January 2011 and December 2021. In total, 254 patients were included. We investigated clinical and radiological findings to identify factors that were associated with the likelihood of surgery in CSH. Results Initial Glasgow coma scale (GCS) and radiological factors, such as hematoma thickness, density type, midline shifting, and laterality, were significantly related to the likelihood of surgery (P<0.05). The use of anti-thrombotic drugs in patients with trauma (P=0.043) or chronic alcoholism (P=0.018) was associated with the likelihood of surgery. The use of anti-thrombotic drugs was associated with the density type (P=0.037) of computed tomography scans, but head trauma was not significantly related to the likelihood of surgery. The use of anti-thrombotic drugs was significantly related to the recurrence of CSH (P=0.021). Conclusion This study reported that the initial GCS and several radiological factors were significantly associated with the likelihood of surgery in CSH. Especially in patients taking anti-thrombotic drugs, head trauma or chronic alcoholism was significantly associated with the likelihood of surgery. Close monitoring of these patients is suggested. Surgical treatment for symptomatic CSDH has shown favorable clinical outcomes (>80%). Since some recent reports have described unfavorable outcomes of surgical treatment of CSDH in older patients, we evaluated the functional outcomes of CSDH in older patients. Methods We retrospectively analyzed the functional outcomes of CSDH patients aged >60 years who underwent burr-hole surgery between January 2017 and December 2021. Using the modified Rankin scale (mRS), we analyzed the functional outcomes of burr surgery and investigated the factors affecting the surgical outcomes. Results In total, 172 patients (115 men and 57 women) were analyzed for the functional outcomes of surgery. Functional outcomes improved in 120 patients (69.8%) and worsened in 52 patients (30.2%). Age (P=0.046), preoperative mRS (P=0.002), and the presence of underlying disease (P=0.025) were factors affecting the postoperative functional status. Conclusion The decision regarding surgical treatment of CSDH in older patients should be made carefully, considering their preoperative functional status and underlying disease. The primary objective was to examine the relationship between the postoperative subdural drainage catheter's position and the recurrence rate. Methods In a retrospective study conducted at Ewha Womans University Mokdong Hospital between September 2020 and August 2023, patients diagnosed with chronic subdural hematoma underwent surgical intervention. We defined the recurrence of chronic subdural hematoma as an increase in the volume of subdural fluid on the treated side compared to the volume measured just before the computed tomography scan was performed. Results The recurrence rates were not significantly different between these 2 groups (20.3% vs. 20.5%, P=0.594). Additionally, there was no significant difference in mean age (74.9 vs. 75.8 years, P=0.733), male-to-female ratio (3.29 vs. 3.25, P=0.478), or maximum depth (13.5 mm vs. 12.7 mm, P=0.323) between the 2 groups. Conclusion There was no significant difference in the recurrence rates between the 2 groups. This suggests that the location of the burr-hole trephination and the subdural catheter tip location might have had no significant impact on patientsâ prognosis. In order to reduce the recurrence rate of chronic subdural hematoma, making efforts to minimize other factors like subdural air collection is important.
