The case report describes the clinical approach to a patient with optic neuropathy in an optometric practice by simple yet valid methods which can be used auxiliary to standard procedures. Material and Methods. Funduscopy was performed via Volk 90D lens and slitlamp. Functional tests, such as visual acuity, pupil reactions, visual field assessment, colour vision, red saturation test, Pulfrich phenomenon and pupil cycle time test, were employed for their differential diagnosis value. Results. The visual acuity and central colour vision were nor- mal; the other tests revealed abnormal state of the patient‘s right eye or visual pathway. Ophthalmoscopy revealed a disc oedema in one half of the optic nerve head. After urgent referral with tentative diagnosis of optic neuropathy of un- known aetiology, a diagnosis of non-arteritic anterior ischemic optic neuropathy (NA-AION) was made at the eye hospital. Conclusion. In patients with sudden onset of monocular visual disturbances where no obvious lesions in visual axis are found various types of optic neuropathies are a frequent finding. Some of these conditions include systemic diseases, such as multiple sclerosis, and some can be associated to life threatening diseases, such as giant cell arteritis. The case report describes the optical rehabilitation of one eye by scleral lens fitting after a sulfuric acid related occupational accident. Material and Methods. Based on the corneal topography determined by Pentacam and other slit-lamp examinations relevant for contact lens fitting, a custom-made scleral lens was fitted on the basis of diagnostic contact lenses. The fit assessment of the diagnostic contact lens was performed by interpreting the fluorescein image as well as the centration behavior of the lens. Results. With the help of the fitted scleral lens a sufficient visual acuity of 20/25 in the right eye with a stereopsis in connection with a good subjective and objective acceptance could be achieved. This was associated with a professional reintegration. Conclusion. In many cases of complex ocular injuries, con- tact lens fitting with scleral lenses is the means of choice for optical rehabilitation of the affected eye. The fitting of scleral lenses requires an experienced practitioner both in the evaluation of the issues relevant for the contact lens design and in the evaluation of the anterior segment of the eye and here in particular the cornea. The aim of the study was to test a newly developed prototype for corneal sensitivity measurement (liquid jet (LJ) with saline as stimulus) for repeatability and correlation with the Cochet-Bonnet esthesiometer (CB). This is an excerpt of a master thesis, which was part of a larger study Material and Methods. Corneal sensitivity thresholds (CST) were determined for 30 subjects, in a clinical trial with 30 subjects, with each device on two separate dates (with inter- vals of at least one day and a maximum of fourteen days). In addition, the pain sensitivity questionnaire (PSQ) was applied to determine any possible correlation with corneal sensitivity. Results. Similar standard deviations of the CSTs for LJ and CB were obtained (LJ (M ± SD): 24.3 ± 2.0 dB; CB (M ± SD): 20.0 ± 2.0 dB). No statistically significant difference in CST was found between the two visits for LJ (mean differen- ce: -0.063 dB, p = 0.78), however there was a statistically significant difference for CB (mean difference: -0.641 dB, p = 0.003). LJ CSTs correlated moderately positively with CB CSTs and PSQ (LJ-CB: r = 0.476, p < 0.001; LJ-PSQ: r = 0.437, p < 0.001). Conclusion. LJ offers a significantly larger stimulus bandwidth than CB. Better reproducibility was observed for LJ, while correlation between the results for the two instruments was good. Significant moderate positive correlation was found between LJ CSTs and general pain sensitivity. The aim of this study was to examine the benefit of +3-glasses as a supplement to the measurement of ob- jective ametropia using the PlusOptix A12R vision screener in children. Material and Methods. Experience from practice and the evaluation of the measurement results with and without the use of the +3-glasses are presented. In addition, a way of interpreting the results in practice is presented. Results. The measurement with both methods was evaluated in 1534 out of 2760 cases. The measurements showed no significant difference (z = -0.250, p = 0.802, r = -0.005), how- ever, showed greater scatter results in the measurement with +3-glasses. If you select the more positive measurement, the mean is shifted by 0.26 dpt (SD = 0.68) towards hyperopia. In addition, a lower scatter result and a significant difference to the measurement without glasses are achieved. This is in pairs between 0 D and 4.75 D, with an average of 0.27 (SD = 0.46), with 261 cases (17 %) differences of 0.75 D or more. Conclusion. If only the measurement with +3-glasses is con- sidered, there is no benefit at first. However, the professional selection between the measured values provides a more re- liable statement about the refractive status, especially about the influence of accommodative compensation of hyperopia. The cost-effective production of the glasses, a high acceptan- ce by the children, the low time required and the additional safety of the results speak for the use of the +3-glasses in early childhood vision screenings. Pathological Myopie (PM) causes structural chan- ges at the posterior segment of the eye that may lead to pathological changes including severe vision loss. The aim of this paper is to describe the pathologies most commonly associated with PM. Material and Methods. The literature review includes the current state of knowledge regarding the structural changes of the posterior segment due to PM, based on theoretical and methodological literature. The selected literature includes nine studies published in 2020 – 2021 as well as reports and assessments of public institutions. Publications by the World Health Organization (WHO), the World Council of Optometry (WCO), the International Myopia Institute (IMI) and the Brien Holden Vision Institute (BHVI) as well as cohort studies with children and adults from China, Singapore, Korea, Taiwan, Japan and Germany were taken into account. Results. Four eye diseases and their relationship to PM are explained and their pathological consequences described. Current therapies to improve the visual acuity in pathologies in the macula are presented. Conclusion. PM often causes structural changes in the macu- lar area. This leads to a significant loss of vision in those affec- ted. Without treatment, PM can lead to blindness. In view of the increase in myopia and the number of myopes worldwide, it is important to sensitize the population to this topic. The prevalence of myopia varies highly between different ethnic groups. Especially in East and Southeast Asia the prevalence of myopia is very high. In addition to ethnic factors, the prevalence of myopia is determined by genetic factors. The aim of the literature review is to determine the prevalence of myopia among children and adults of Caucasian origin in Europe and North America. Material and Methods. A systematic literature review was performed for the prevalence of myopia in Europe, Middle East, North America, and Australia. For the evaluation of the results, the methodological requirements of an epidemiologi- cal study of myopia prevalence were formulated. A threshold of -0.50 D and retinoscopy should be standard. Results. The prevalence of myopia is not increased among Caucasians with 30 % from a statistical point of view. In children, the prevalence of myopia can be assumed to be 15 %. The risk of myopia is increased threefold in children with myopic parents compared to children with nonmyopic pa- rents. Refraction determination with autorefractometers and omission of cycloplegia leads to a significant overestimation of the prevalence of myopia. Conclusion. An increase in myopia prevalence can not ne- cessarily be inferred from the observed prevalence of 30% among Caucasians. A reliable determination of myopia pre- valence requires standardized examination methods and thresholds for myopia. The aim of the study was to determine the thera­ peutic effect of low­dose atropine for inhibiting myopia pro­ gression in clinical practice outside of controlled clinical trials. Material and Methods. We performed a retrospective analysis on data collected on a regular basis (refraction, axial length) pertaining to 79 children treated with atropine and 100 un­ treated children over an observation period of 12 months. Results. Our findings show that myopia progressed in both groups with highly variable progression rates. A two­way ANOVA for age and atropine effects on axial length growth revealed that axial length growth rates decline with age (p < 0.0001) and that atropine has a significant inhibitory effect on axial length growth (p < 0.0015) independent of age. However, this effect showed only a 0.08 mm inhibition rate per year (28 %) as compared to the average growth rate in the untreated children, meaning that its magnitude is quite small. Furthermore, the effect of atropine on the refractive power was not significant. Conclusion. We found that, in this clinical setting, the effects of atropine were not distinctive and of only minor clinical relevance. The results of this study show that the beneficial effects of a treatment with 0.01% atropine eye drops may not be apparent in every individual case, information that should be communicated to the parents of myopic children and general ophthalmologists. The case report describes the conversion of a cor- neal lens wearing keratoconus patient to scleral lenses. Material and Methods. Based on an analysis of the corneosc- leral profile using the CSP module of the Oculus Pentacam, the patient was fitted with individual scleral lenses with a peripheral toricity. The exact determination of the corneal topography, the corneoscleral profile, the interpretation of the fluorescein images of diagnostic and prescription lenses as well as all other fitting relevant measurements and exami- nations are described. Results. In the present case a successful conversion of the contact lens fitting from a corneal lens to a scleral lens could be performed. Wearing comfort as well as subjective and objective tolerance of the contact lenses could be optimized in connection with a simultaneous increase of visual acuity. Conclusion. Visual rehabilitation of keratoconus patients with scleral lenses can be a good alternative to other contact lens treatments depending on the individual case. Quantification of the corneoscleral profile is of great benefit in determining scleral lens parameters. Current prevalence rates of myopia in children and adolescents vary all over the world, with especially high prevalence rates in East Asian countries. The objective of this study was to describe the refractive status in children and adolescents growing up in Germany. Material and Methods. Non-cycloplegic refractive status of children of the LIFE Child study in Leipzig, Germany, was measured by wavefront-based autorefraction in 1934 sub- jects (925 girls / 1009 boys), aged 3 to 16 years (mean = 9.05 ± 3.91). Myopia was defined as spherical equivalent refractive error (SE) ≤ -0.75 diopters (D), emmetropia as -0.75 D > SE < +0.75 D, hyperopia as SE ≥ +0.75 D and astigmatism as cylinder ≤ -0.75 D. Anisometropia was defined as a difference of ≥ 1.0 D in the SE between the two eyes. Results. Analysis revealed that refractive error became more myopic with older age (b = -0.08, p < 0.001), with an observed prevalence of myopia of 27 % in 16-year-old children (4 % in 3-year-olds). The true prevalence of myopia might be lower as non-cycloplegic measurements might overestimate ref- ractive error in myopes and underestimate refractive error in hyperopes which in turn may overestimate the prevalence of myopia. The prevalence of anisometropia also increased with growing age (OR = 1.14, observed prevalence in 3- versus 16-year-olds = 2.3 % and 8.1 %, respectively). The prevalence of astigmatism was 11.8 %. Conclusion. Myopia prevalence in German children aged 3 to 16 years is around or even less than 10 %, taking into consideration that measurements were carried out without cycloplegia. Our results are comparable to other European paediatric studies. In comparison to East-Asian countries myopia prevalence, and thus the risk for eye diseases related to high myopia, is much lower in Germany. Seventy-four left eyes of healthy sub- jects were evaluated. Data was collected as part of previous research (bachelor-thesis) using the CSP-Report module of the Pentacam Basic. Sagittal height, limbal angle and scleral angle were analyzed over three diameters in eight semi- meridians. Results. At diameter 10.00 mm the shape of the anterior segment of the eye was nearly rotationally symmetrical, the average sagittal height of the eight semi-meridians was between 1734 µm and 1806 µm. At diameter 12.80 mm the shape was more asymmetrical, average sagittal height ranged from 2797 µm to 3000 µm. At diameter 15.00 mm the asym- metry was most pronounced, the average sagittal height was between 3595 µm and 4033 µm. The average limbal angles at 12.80 mm were in the range of 35.68° and 42.02°. For the scleral angle at 15.00 mm mean values between 34.71° and 43.11° were determined for the eight semi-meridians. The nasal segments always showed smaller sagittal heights, accompanied by smaller limbal and scleral angles compared to the temporal semi-meridians. The same pattern was obser- ved for the superior semi-meridians in relation to the inferior segments. For the nasal semi-meridians, concave transitions of the corneo-scleral profile were observed, whereas tempo- rally a convex course of the CSP was observed. Conclusion. The CSP-Report provides a method to describe the shape of the anterior segment of the eye. These additional data are useful for fitting scleral lenses.
