{"title":"Patient Safety Culture Dimensions as Perceived by Nurses","authors":"Zeid Alrowely, O. Shared culture of patient safety should be of primary significance and there is resolute need for theory lead research approaches for safety culture in health care organizations. The main objective of this research was to investigate staff nurses’ understanding of patient safety culture in governmental hospitals in the northern region of Saudi Arabia. The descriptive cross-sectional design was used in this research. 503 nurses from six hospitals were included. This study introduced credible findings owing to the whole high response rate (73%) and outcomes with specified results. The staff nurses rated patient safety as very good (Mean = 2.30; SD = 0.839) supposing that efforts of the organizations and nurses were executed to offer the safest patient care possible. The whole patient safety culture was rated neutral supposing that (63%) of the staff nurses who participated in the current study have merged ideas about the status of patient safety culture in their respective organizations. Another essential result that required further investigation was the extent of frequency of events reported wherein between 25% and 32% or the whole 21% never or rarely reported events that occurred. This study aimed at secondary analyzing of reported symptoms experienced by Saudi children and adolescents with cancer over multiple cycles of chemotherapy treatment, identifying the experiencing of similar symptom trajectories. Methods: Subjects were 130 Saudi children and adolescents recruited from four tertiary hospitals who completed the sociodemographic and clinical data sheet, Memorial Symptom Assessment Scale (MSAS 10-18) and Karnofsky Performance Status (KPS) Scale. Findings: Four discrete classes using latent class profile analysis were recognized: minimum distress (Class I), physical prominent distress (Class II), psychological prominent distress (Class III), and maximum distress (Class VI). Vomiting (M=0.88) was reported as the most distressing symptom in Class I, whereas lack of energy was the most prevalent distressing symptom in Class II (M=3.1). Subjects reported worrying (M=2.68) as the most distressing symptom in Class III of clustering, while lack of energy (M= 4.85) was reported to be the most prevalent in Class VI. Methods: We took consecutive patients between the ages of 35 and 70 years to be able to see possible consequences of excess weight on health and to avoid debility induced weight loss in elder individuals. Results: The study included 270 cases (145 females) with normal weight, 270 cases with overweight, and 270 cases with obesity. Female ratio was 53.7% in the three groups. Mean ages were 47.1, 46.3, and 48.9 years in the three groups, respectively (p>0.05 for both). As a pleasure point in life, smoking did not show higher prevalences in the overweight or obesity groups, and its prevalences were similar in the three groups (35.9%, 32.9%, and 33.7%, respectively, p>0.05 for both). On the other hand, prevalences of hypertension (HT) (8.1%, 13.7%, and 21.8%), diabetes mellitus (DM) (9.6%, 20.0%, and 28.5%), and dyslipidemia (19.2%, 32.5%, and 40.3%) showed highly significant increases from the normal weight towards the overweight and obesity groups, respectively (p<0.001 nearly for all). Conclusion: Parallel to its severity, excess weight is associated with greater prevalences of HT, DM, and dyslipidemia. As a pleasure point in life, smoking may also show the weakness of volition to control eating in cases with excess weight. It has been found out that the 3 top rank factors contributing to medication errors were interruptions during medication pass (M= 2.32), shortage of nursing staff (M= 2.13), and caring for high acuity patients (M= 2.11). Whereas, hostile work environment (M= 1.56) was the least among all factors. Moreover, all four types of medication technologies: barcode medication administration (M= 1.79), computerized physician order entry (M= 1.9), automated medication dispensing (M= 2.28), and smart infusion pumps (M= 2.34) were perceived as very helpful in reducing medication errors. Lastly, the several demographic characteristics, years of clinical experience (p= 0.01), and the years of attending pharmacology courses (p= 0.04) were found significantly related with medication errors.
