OBJECTIVES: Evaluate feasibility and concordance of two musculoskeletal screening tools, the 12-item questionnaire for early arthritis detection of (DPA-12) and the musculoskeletal examination pGALS (pediatric, gait, arms, legs and spine), in the same patient, during the routine practice. METHODS: Participants between 5 and 18 years of age were examined by a trained resident, performing pGALS maneuvers and applying DPA-12 for parents. The diagnoses, descriptive analysis and scores were analyzed, categorizing the pGALS maneuvers as normal or abnormal and DPA-12 scores greater or lower than six. The internal consistency was calculated by Cronbach alpha coefficient and the correlation between categorical measures by kappa statistics. RESULTS: Sixtyseven participants were included, 40 (59.7%) were boys. Forty (59.7%) had at least one abnormal pGALS manoeuver. The diagnoses were classified as musculoskeletal (juvenile idiopathic arthritis and connective tissue diseases, flat feet, hypermobility syndrome, mechanical limb pain) and nonmusculoskeletal (cellulitis and constipation). There were 65 valid questionnaires, 21 (32.3%) scored greater or ecqual to 6, indicating higher probability of musculoskeletal diagnoses. The internal consistency scoreswere 0.79 and 0.87, respectively. The comparison of normal and abnormal variables results, resulted in low agreement by the Cohen kappa coefficient (0.31). CONCLUSION: The application of both tools in the same patients performed well, showing simplicity and feasibility, for identifying functional or structural abnormalities, regardless of the diagnosis. Birth weight may be a determinant of overweight/obesity risk and is considered a health indicator, which may influence the childs growth and development, also affecting health in adulthood. OBJECTIVE: This study aims to check the association between birth weight and overweight/obesity in children aged 5 to 9 years. METHODS: Observational and descriptive cross-sectional study with primary data on a sample of 500 schoolchildren aged 5 to 9 years, enrolled in public institutions in Fortaleza, CE. Primary data was collected with a structured form, family history and measures of weight and height at the time of the interview. Continuous variables were compared using non-parametric tests and categories of independent variables using chi-square and Fishers exact test, with a p-value<0.05. RESULTS: Both low weight (<2.500g) and weight over 3.500g were positively correlated with the presence of overweight/obesity, with 40.54% of those born with low weight and 52.9% of those born with weight over 3.500g were overweight/obese at school age (p=0.001). A statistically significant association was found with the lowest social class (OR=3.51, 95%CI: 1.60-7.69), with the highest per capita income (OR=1.59, 95%CI: 1.08-2.33) and with birth weight over 3.500g (OR=2.55, 95%CI: 1.67-3.90). Methods: Chest CT scans performed from January 2016 to January 2020 in a pediatric care service were selected. The reports were consulted, verifying those with compatible criteria currently accepted for the diagnosis of bronchiectasis. The medical records of those selected underwent a review process in search of the variables analyzed. Patients diagnosed with cystic fibrosis and with outdated medical records were excluded. Results: Of the 2319 reports studied, 42 patients were identified. Among these, 59.5% were female, 66.7% were followed up at a pneumopediatrics outpatient clinic, and the mean age at the time of imaging diagnosis was 9.3 years (±4.9). At follow-up, 57.1% of patients underwent spirometry and 35.7% underwent sputum culture. The most observed bacterial growth was that of Pseudomonas aeruginosa (33.4%) and the most frequent spirometric pattern was restrictive (29.2%). Of the presumptive etiologies, the most prevalent was recurrent pneumonia (21.4%), followed by bronchiolitis obliterans (14.3%) and collagen diseases (14.3%). Conclusions: The results found in this study allowed a better characterization of this subgroup of patients. It is more common in the northern region, especially from June to August, due to the season of river beaches and greater contact with rivers, dams and freshwater lakes: natural habitat of those species. It has an intense and also late inflammatory character, being a poison composed of polypeptides with thermolabile properties. There is no antivenom for those cases in Brazil. The treatment is based on symptom control and prevention of secondary infection, with status for the late effects of the poison. OBJECTIVES: To review emergency management and medical follow-up of an acantotoxic accident case in a pediatric patient and report the singularities related to this variety of accidents. METHODS: A case report of a 9-year-old boy admitted to a pediatric emergency department of a Federal District School Hospital, with a history of freshwater stingray injury, a clinical presentation of cellulitis and a dragged course. RESULTS: Regarded evolution compared to usual cellulitis patterns, requiring the antibiotic change and late signs of a form of necrotic formation. CONCLUSION: The approach to cellulitis was redirected due to the etiological factor correlated to the acantotoxic accident, implying to broaden the spectrum for gram-negative and anaerobic bacteria. It has a benign course. The only treatment that is proven to be effective is supportive. Objectives: To evaluate the frequency of pharmacological and non-pharmacological treatments used in patients who received bronchiolitis diagnosis in the Emergency room (ER) in the last five years, bronchiolitis evolution in these patients and develop a treatment protocol for bronchiolitis patients. Methods: Longitudinal retrospective study including children diagnosed with bronchiolitis in the ER from January 1, 2014 to December 31, 2018. Patients were compared for age, month of year in which they sought the ER, clinical characteristics before and after initial approach, need for ventilatory assistance, and outcome. Results: We analyzed 614 children; 58.3% were male; mean age was 5.21 months; there was a concentration of cases between April and July; Complication was described in 18.24% of patients. First therapeutic choice was inhaled bronchodilator (60.6%). All treatments were related to a statistically significant reduction in wheezing. Treatments that showed the highest improvement rates in infant respiratory pattern were nasal washing with SF0.9% and oxygen therapy. Conclusion: Evolution in bronchiolitis patients mainly depends on how the organism reacts to infection. No treatment demonstrated efficacy in changing bronchiolitis outcome, although transitory clinical improvement. Drug usage seems to be associated with greater physician concern due to greater severity in these children. França","doi":"10.25060/residpediatr-2023.v13n2-694","DOIUrl":"https://doi.org/10.25060/residpediatr-2023.v13n2-694","url":null,"abstract":"OBJECTIVE: To evaluate the prevalence of autoimmune markers associated with beta-pancreatic cells, autoimmune thyroid diseases and celiac disease through research of anti-glutamic acid decarboxylase antibodies (GADA), antiislet cell antibodies (ICA), anti-insulin antibodies (IAA), anti-tyrosine phosphatase (IA-2A), anti-transglutaminase antibodies (anti-tTG), anti-endomysium anti-transglutaminase antibodies (EMA ), immunoglobulin A (IgA), thyroperoxidase antibodies (anti-TPO) and thyroglobulin antibodies (anti-TG), in addition to related hormonal measurements in children and adolescents with type 1 diabetes mellitus. METHODS: Observational, longitudinal study, with retrospective data collection by reviewing the medical records of 92 children and adolescents, diagnosed with DM1 between January 2014 and December 2016, seen at a Pediatric Endocrinology service of a tertiary hospital in Curitiba- PR. RESULTS: At the diagnosis of DM1, the prevalence of anti-GAD antibody positivity was 69.6% (n = 79), ICA 42.9% (n = 70), IAA 32.8% (n = 64), IA2 of 60.6% (n = 33). Positivity for markers of other associated autoimmune diseases was 27.4%, EMA 6.0% (n = 50), anti t-TG IgA 12.5% (n = 8), anti-TPO 13, 1% (n = 84) and 22.9% anti-TG (n = 83). Autoimmune thyroid diseases were the most frequently found comorbidity at diagnosis (22.1%). Among patients with positivity for antithyroid antibodies, 19% developed clinical hypothyroidism. CONCLUSIONS: Patients with DM1 have a high prevalence of autoimmunity. METHOD: The Mobile Phone Problematic Use Scale short version (MPPUS-10) was applied in adolescents of two public schools, which classifies the subject as a problematic user of a cellular device or not. For this study scores above 51 was considered problematic use; between 29 and 50 points, users at risk of becoming problematic; from 18 to 28 points were regular users and below 18, occasional users. The following variables were evaluated: age, school grade, gender, cell phone ownership, having access to other cellular devices, and their relation to MPPUS-10 score. RESULTS: Age ranged from 12 to 19 years (average: 15.5; median: 16; standard-deviation: 1.8) 420 questionnaires were given, of which only those that were correctly filled were selected, totaling 168, of which 100 (59.5%) were female. In the evaluated group, 68 (40.5%) presented a risk of becoming problematic users and 65 (38.69%) were classified as problematic users of the device. There was a positive relationship between female gender and MPPUS-10 scores. Pacientes com alterações abdominais parciais, criptorquidia ausente ou unilateral e femininos são classificados como portadores da PSPB. A SPB constituiu uma patologia rara, com incidência de 1/40.000 nascidos vivos, sendo ainda mais incomum encontrarmos pacientes com PSPB, visto que acomete 3-5% dos casos incialmente diagnosticados como SPB. O relato de caso a seguir apresenta uma paciente de 1 anos e 9 meses, supostamente portadora de SPB desde o nascimento, admitida por causa infecciosa resolvida, e que durante a internação e avaliação complementar, teve seu diagnóstico modificado para PSPB. Devido ao subdiagnóstico e diagnósticos equivocados da PSPB, o relato visa promover maior conhecimento acerca da doença, reforçar o diagnóstico clínico a partir dos achados mais prevalentes e ratificar a importância da terapêutica precoce como determinante da qualidade de vida.
