Oliveira, Jocemara Gurmini","doi":"10.25060/residpediatr-2021.v11n3-174","DOIUrl":"https://doi.org/10.25060/residpediatr-2021.v11n3-174","url":null,"abstract":"In aim to facilitate the use of replacement measures of body segments for monitoring the child’s growth, the calculation of medication doses and the organic function rates, this study selected previously healthy children from 5 to incomplete 18 years of age, at a pediatric hospital, and measured their height, ulna’s length and the forearm length. An amount of 120 patients were selected, 61 female and 59 male. From these, 77 declared their color as white, 40 as brown and 3 as black. The respective averages were 9.5 years of age, 135cm in height, 36.2cm in forearm length and 20.9cm in ulna’s length, with a statistically significant difference between the two genders (p<0.05). The ulna’s and the forearm length were reproducible, with low coefficient of variation. There were no statistical significance when evaluated if the replacement measure could underestimate or overestimate the height. At the Wilcoxon test, there were more errors on predicting the height through ulna’s length on females, and more errors through forearm length on males (p<0.05). At Student t-test statistical evaluation, for the whole sample of the study as for the stratified evaluation of each gender and ethnicities, there were no significant statistically difference between the estimated height through the forearm length and the measured height (p=0.28). Tough, the estimated height through the ulna’s length compared with the measured height had a statistical significance difference (p<0.05). The disease is characterized by multiple congenital anomalies, which include distinctive facial features, prenatal and postnatal growth deficiency, neuropsychomotor delay, gastroesophageal reflux disease and upper limb malformations. We describe the case of a male patient who presented phenotypic alterations compatible with the classic form of CdLS. The diagnosis can be clinically determined even in the absence of molecular confirmation. Orogastric tube and admission to a neonatal Intensive Care Unit were necessary due to difficulties with suction. Subsequently, the patient underwent gastrostomy. They can be identified from the neonatal period or in the first two years. The pathogenesis is still uncertain opening possibility to several theories correlated to its formation during the embryogenesis. The clinical manifestations are very varied depending on the location and size of the lesion. Diagnosis is initially performed by abdominal ultrasonography and confirmed by histopathological examination. In these patients, the treatment is always surgical. It occurs by complication of pharyngotonsillitis caused by beta hemolytic streptococcus of the A group. This case report aims to describe a case of rheumatic fever diagnosed from a clinical picture of Sydenhams chorea. A descriptive case study of a female child was carried out, she was twelve years old, she sought the emergency care unit with involuntary movements in lower and upper rights limbs and face for five days, she had a previous history of infection an upper respiratory infection with self-limited evolution. The physical examination found, was in good general condition, with extension and flexion limbs movements, opening and closing hands movements, finger and face movements, complementary exams, they did not demonstrate any alterations, except ASLO of 217IU/ml. It was started a treatment with haloperidol, benzathine penicillin and and school progress evolved asymptomatic with improved choreatotic movements. METHODS: Cross-sectional epidemiological study of children and adolescents aged six and 16 years; evaluated at the general pediatrics outpatient clinic of the Hospital Universitário do Oeste do Paraná, in Cascavel City, State of Paraná. The variables analyzed were: sex, age, BMI, and their relationship to the pediatric symptoms checklist (PSC). For the analysis of the data the chi-square hypothesis tests and Fishers exact hypotheses were used for the proportions. The measure of effect of interest was the 95% confidence interval ratio, obtained with the Poisson regression, considering significant results with value of p<0.05 Wald test. RESULTS: 157 subjects aged six to 16 years participated, 123(78%) of which were under 12 years of age. Of these, 66 (42%) were female and 91 (58%) male, 88 (56%) were obese and 69 (44%) were overweight. The pediatric symptoms checklist was positive in 39 (24%) of the participants. Most participants with positive PSC were less than 12 years old and obese. There was no statistical significance between age, sex and BMI and positive score on the pediatric symptoms checklist. An integrative review was carried out using the descriptors “magnesium sulfate”, “severe asthma”, “children”, and “adolescents” in English, Portuguese, and Spanish. The inclusion criteria were full articles that portrayed the topic of intravenos use of magnesium sulphate in the treatment of severe acute asthma in children and adolescents, published between 2010 and 2018. CASE REPORT: Infant, female, born at term, vaginal delivery, 3.570g, natural and coming from Uberaba. After birth, she was hospitalized for a congenital heart disease and Down syndrome, cardiac surgery was performed. In this period, the respiratory physiotherapist perceived the difficulty of introducing the nasal aspirate, and was sent to the otorhinol, without statistical findings, a cranial tomography was performed at 4 months, which diagnosed an infant with nasoethmoidal and nasofrontal encephalocele. The encephalocele is a rare anomaly, with high morbidity, mortality and risk of complications. It is related to other pathologies diagnosed in patients and to the antecedent factors at birth, genetic and environmental causes. Swallowing disorders in children may compromise growth and development with significant compromise to the child’s general health, sometimes leading to multiple hospital admissions and diagnostic exams that expose the child to unnecessary risks, when ordered outside the context of a multidisciplinary approach. Most times dysphagia is multifactorial. Several pathologies a part from neurologic impairments may be involved as the main cause or a co-factor for swallowing disorders. Treatment may be medical, surgical or both, and some interventions may be needed even during follow-up due to consequences of long-term dysphagia. Specialists central to all multidisciplinary programs are: the otolaryngologist, the pneumologist, the gastroenterologist and the speech and language pathologist to ensure that evaluation of structural and functional aspects of the respiratory and digestive systems and its repercussions on the child’s general health have been encountered and will be continuously accounted for.
