However, few studies have focu-sed on the drug treatment of pediatric patients affected by COVID-19. OBJECTIVE: Thus, this study conducted a review of clinical trials registered in the Clinical Trials database on drug treatment in pe-diatric patients with COVID-19. METHODS: A systematic search was conducted in the Clinical Trials database (https://clinicaltrials.gov/) using the web tracking technique to collect registered clinical tri-als that conducted drug treatment in pediatric patients aged 0 to 17 years. After the drugs used were identified, their dosage and other pertinent information were obtained from the DrugBank databases (https://go.drugbank.com) and the DrugBase (https://www.drugbase.de). RESULTS: Eight clinical trials were found in the research, classified as completed and 34 in progress, aimed at pediatric patients. The United States was the country that most research involving the drug use in patients with pedia-tric COVID-19. Hydroxychloroquine was the most observed drug in the treatment of pediatric pa-tients, followed by remdesivir. An increase in phase 3 and randomized trials was also observed among ongoing research when compared to completed research. CONCLUSION: Compared to adults, pedia-tric COVID-19 has distinct treatment needs. The aim of this study was to describe clinical cases of newborns admitted to the Neonatal Intensive Care Unit of a Brazilian university hospital who were monitored with NIRS, and to evaluate the applicability of NIRS demonstrating its use as a continuous monitoring method for newborns at high risk for brain injury. This study was part of the Conclusion Work of Medical Residency in Neonatology in Neonatology. Case 1: Persistent ductus arteriosus-NIRS guided indication for drug treatment. Case 2: Septic shock-NIRS was helpful identifying the need of inotropes. Case 3: Anemia of prematurity-NIRS showed an increase of cerebral oxygenation after red blood cells transfusions. Case 4: Hypoxic-ischemic encephalopathy-NIRS identified sign of poor neurological prognostic. NIRS is a continuous cerebral and renal perfusion monitoring method that has easy and good applicability and no complications. The cases reflect the importance and potential of NIRS in clinical practice and in helping diagnose for newborns, as well as enabling prognostic assessment. NIRS proved to be a method of good applicability, without short-term complications. The results showed that the mothers’ confidence in pediatric guidelines is an indispensable factor for excellence in monitoring the mother’s and the child’s health. However, one of the biggest barriers to a better quality of care in this profession is insufficient time during the consultation to extend the care to the empowered mother regarding her desire for the doctor’s support for shared decision-making. The results made explicit the similarities and differences in this dynamic and the need to converge the two sides of this relationship. Its prompt recognition and early and precise etiological investigation are fundamental to its proper management. A patient with ambiguous genitalia, born cesarean due to severe pre-eclampsia and oligohydramnios at 34 weeks and 2 days, 1505g, considered small for gestational age (SGA). Examination showed an 1.9cm falus, penoscrotal urethral meatus and bilaterally palpable gonads. In the investigation, he presented normal testosterone (T), androstenedione (A) and dihydrotestosterone (DHT); T/DHT ratio of 9.7 (<10) and T/A of 7.4 (>0.8) and karyotype 46,XY. It was decided for male sex assignment. Testosterone stimulus test was performed, showing penis enlargement of 1.5cm. Intrauterine growth restriction is a considerable risk factor for genital ambiguity in individuals 46,XY. This seems to be the etiology in this case, given its normal hormonal and cytogenetic evaluation and the response to the testosterone stimulus. Its action in multiple tissues promotes maturity of fetal structures, with significant impact in the reduction of neonatal morbimortality. Objective: Evaluate the effects of AC in very low birth weight preterm infants (VLBWPI). Methods: Data from the Rede Gaúcha de Neonatologia of those VLBWPI born between 01/01/2008 and 12/31/2014 at the Fêmina Hospital in Porto Alegre was used. The exposure to AC (one or two doses of intramuscular 12mg betamethasone) and its relation to mortality and following comorbidities were analyzed: respiratory distress syndrome (RDS), intraventricular hemorrhage (IVH), periventricular leukomalacia (PVL), retinopathy of prematurity (ROP), necrotizing enterocolitis (NEC), patent ductus arteriosus (PDA), and bronchopulmonary dysplasia (BPD). Results: A total of 496 patients were obtained, of whom 68% received at least one dose of CA and 32% received none. A significant reduction in the incidence of RDS was obtained in the group that exposed to AC (OR 0.468, p = 0.001). A 60% reduction in IVH (OR 0.401, p < 0.001) was noted. A reduction in the incidence of BPD was also observed with the use of AC (OR 0.296-0.903, p = 0.018). No statistical difference was observed in the incidence of PVL (p = 0.3), PDA (p = 0.68), NEC (p = 0.44) or ROP (p = 0.58). Neonatal death was significantly reduced by 78% in those treated with AC (OR 0.22, p < 0.001). Conclusion: exposure to AC, regardless of the number of doses, confers lower morbidity and lower mortality to VLBWPI. METHODS: Cross-sectional and observational study that analyzed the medical records of patients seen in the form of inter-consultations in a PR outpatient clinic of a tertiary hospital for 14 months. Epidemiological, clinical history and laboratory tests data were collected and descriptive statistics were performed. RESULTS: 37 patients were included, 27 (73%) female and 10 (27%) male, with a median age of 12.4 years (min.: 1.3/max.: 15.9). Having performed several complementary exams, patients were referred by 13 different specialties, with a predominance of pediatric dermatology. In the first consultation with a pediatric rheumatologist, the main complaints were arthralgia (51.3%), skin blemishes (32.4%) and skin rash (29.7%). The period between the onset of symptoms and the consultation ranged between 16 and 4554 days, with a median of 730 days. Twenty (54.1%) patients had the exclusion of a rheumatological disease, three (8.1%) lost follow-up and 14 (37.8%) were diagnosed with rheumatological diseases, the most common being linear scleroderma (28.6%), juvenile systemic lupus erythematosus (28.6%), juvenile idiopathic arthritis (14.3%), and juvenile dermatomyositis (14.3%). CONCLUSION: The main complaints of patients referred to RP were arthralgia and skin lesions, with a long period of symptom evolution, in addition to a wide age range and the predominant role of female patients. The objective of this study is to carry out a bibliographic review of the medications used in pediatric SRI, and to propose the institution of an updated protocol that includes a safe analgesic medication. The estimated incidence varies between 1-4 cases for 100.000 inhabitants in the endemic regions of Brazil. It is known that PCM can coexist with other systemic mycoses such as Histoplasmosis. We report a case of a 10-year-old adolescent from Barra Mansa, RJ, Brazil. He was hospitalized after recurrent visits to the emergency department for approximately one month, presenting with non-bloody diarrhea, weight-loss of 6kg in six weeks, dry cough and nocturnal fever (average temperature 38ºC). Counterimmunoelectrophoresis and lymph node biopsy were positive for Paracoccidioides brasiliensis. The treatment consisted of Amphotericin B IV 0,6 mg/kg of body weight per day for 10 days and Itraconazole PO 200mg per day for 12 months. After 5 years, the patient remains asymptomatic. Reports of similar cases are scant in the literature and, the few encountered, are of immunocompromised children.
