SARS-CoV-2 virus (further – COVID-19) damages the respiratory tract and lungs, leads to the development of not only acute heart, kidney, multiple organ failure, but also accompanied by symptoms of nervous system damage. The most common and severe among the neurological complications of COVID-19 are cerebrovascular diseases, acute necrotic encephalopathy, encephalitis, encephalomyelitis, hypoxic encephalopathy, Hyena-Barre syndrome. Studies have shown that patients with COVID-19 have an average risk of stroke of 5-8%. All subtypes of stroke can occur as a result of infection. Recommendations for the management of stroke patients with COVID-19 are presented. Lesions of the peripheral nervous system are manifested in the form of hyposmia, anosmia, Hyena-Barre syndrome. An association between the severity of the viral infection and the frequency and severity of neurological disorders has been established. The results of own researches of 42 patients in the postcovid period are presented. It was shown that 95.2% of patients had neurocognitive disorders of varying severity, asthenic syndrome (increased fatigue on the MF1-20 scale 13.0 points), sleep disorders, dizziness, vestibular disorders, cephalic syndrome, hyposmia in 19% of patients. The subjects also had anxiety and depressive disorders according to the HADS scale. Ischemic stroke and transient ischemic attacks (TIA) have been reported in some patients. The trend of increasing mental disorders, especially due to borderline disorders, which has been observed in recent years, requires the search for new approaches to maintaining mental health, prevention and, in particular, early diagnosis of non-psychotic mental disorders (NPD). The aim of the study. Investigate the frequency and structure of new cases of non-psychotic mental disorders in students of higher education institutions. Materials and methods. During 2015-2017, we conducted a continuous comprehensive survey of 1,235 students in compliance with the principles of bioethics and deontology. Applied methods: clinical, clinical-psychopathological, clinical-epidemiological, clinicalanamnestic, experimental-psychological and statistical. The sample did not have signifi cant diff erences in gender and age, place of residence, form of education. The survey was conducted in the intersessional period. Results. Evaluation of primary cases of NPD, which were detected during re-examination, was performed among practically healthy students based on the results of the fi rst examination and compared the data with the structure of NPD, which we diagnosed in the entire cohort of subjects studied in the fi rst cross section. The majority of students in whom the initial examination revealed no mental pathology remained virtually healthy after a year (858 people, 93.46%). Analyzing the structure of NPD, which arose during this period in this contingent, was narrower in comparison with the identifi ed patients at the initial examination. Mainly, NPD were represented by aff ective disorders (F30.0, F32.0, F34.0) and neurotic, stress-related and somatoform disorders (F40.1, F41.2, F42.0, F43.20, F43 .21, F43.22). An interesting fact was that in the structure of the NPD detected during the re-examination there were no other, than previously diagnosed during the initial examination, the category of mental pathology according to ICD-10. Conclusions. Thus, mainly NPD of aff ective (1.2%) represents the structure of new cases of NPR diagnosed during the year of study and neurotic registers (5.34%) (In the ratio ≈ 1/5, respectively), so other forms of mental pathology occur more quickly in period before the beginning of training or at long training. {"title":"Individual-psychological and pathocarterological features of patients with depressive disorders","authors":"","doi":"10.26565/2312-5675-2021-17-06","DOIUrl":"https://doi.org/10.26565/2312-5675-2021-17-06","url":null,"abstract":"The problem of depressive disorders is one of the most important problems of modern psychiatry due to the high prevalence and signifi cant medical and social consequences of depression, which determines the relevance of the study of individual psychological and pathocharacterological characteristics of patients with depressive disorders for prevention of depression. The aim of the study: to determine the individual psychological and pathocharacterological characteristics of patients with depressive disorders, taking into account age and sex of patients. Objectives of the study: to determine the structure and quantify the severity of individual psychological and pathocharacterological characteristics, as well as to establish their relationship with the severity of aff ective symptoms in patients with depressive disorders in diff erent age groups and taking into account sex of patients. Methods: using Mini-Mult test clinically and psychologically examined 107 men and 138 women with depressive disorders. Results. It was found that patients with depressive disorders have stable characteristics of dysthymia (84.1±4.6 T-points in men, 81.3±1.9 T-points in women, p<0.01), anxiety (respectively 78,8±9.5 T-points and 83.7±8.3 T-points, p<0.01) in combination with hypochondriac fi xations (71.4±8.5 T-points and 75.6±5.7 T-points, p<0.01), and moderate manifestations of rigidity, dissociative traits and psychopathy. Sex diff erences are more pronounced in men with persistent dysthymic and rigid traits, and in women – hypochondriac, anxious traits and manifestations of aff ective lability. The most signifi cant pathocharacterological features are manifested in the initial stages of the disease, which is manifested by the greatest diff erences in their manifestations between younger and middle age groups, especially in relation to hypochondriac, dissociative, rigid, schizoid characteristics and manifestations of psychopathy. The COVID-19 crisis is a crisis of both physical and mental health. Proper mental health is crucial to the functioning of society at all times in every country and should be a priority and at the center of response and recovery after the COVID-19 pandemic. Mental health and well-being - the well-being of society as a whole - have been severely compromised during the pandemic and are a priority for urgent consideration. In the qarantine period, the social media were important sourses of information. The aim of this study was to fi nd out the relation between social media use and stress together with anxiety during this pandemic of COVID-19. We are looking for trends of social media use and infl uence that on behavioral and mental disorders. This study was conducted with the help of an online based survey of individuals from a fi rst wave and during a second wave of pandemic in Ukraine. It was conducted to understand the relationship between social media and stress during the Covid-19 pandemic and to see dynamics of change. A total of 199 participants of the fi rst test period and 152 participants of a second test period responded to the survey which was conducted by snowballing sampling techniques in the convenient atmosphere. We applied two validated, self-reporting questionnaires namely Perceived Stress Scale and test of Generalized Anxiety Disorder-7. When period of use of social media had increased during the pandemic, it led to an increase in the perception and dissemination of false information (fakes) that negative aff ected psychological well-being. Negative aspects of the use of social networks can aff ect the level of anxiety and stress of the population of our country. Among the respondents, 120 (34.2%) think that their mental health and wellbeing was being deteriorated during this period of pandemic. {"title":"Psychoneuroimmunological aspects of post covid19 syndrome","authors":"","doi":"10.26565/2312-5675-2021-16-07","DOIUrl":"https://doi.org/10.26565/2312-5675-2021-16-07","url":null,"abstract":"A sample screening study was conducted by anonymous questionnaire of 223 adult outpatients with signs of post COVID19 syndrome (PCS) from 1 to 3 months after the disease. Among the respondents 77.6% are men, 22.4% are women of different ages. Young and middle-aged people predominated. 77.3% of them were treated on an outpatient basis. 89.6% of respondents had mild to moderate disease. Viral pneumonia was diagnosed in 28.2%. Respiratory distress syndrome survived 5,8%. Re-infection of SARS-CoV-2 had 3,5%. Prolonged low-grade fever (more than 4 weeks) after the disease was maintained at 33.6%; in 15,6% revealed immunosuppression of the cell and phagocytosis. 75% were diagnosed with long-term fever of non-infectious origin. Complete recovery from 2 to 4 weeks was noted by 82.9%. Three months or more required 19,2% for convalescence. They were dominated by signs of CFS and psycho-emotional imbalance. Regarding cognitive disorders, the greatest difficulties arose with the perception of information in 12.7%, with its analysis in 9.8%, with decision-making in 8%. 25.9% of respondents note mnemonic disorders. Attention disorders occurred in 24.9% of respondents. Among the most pronounced mental states that manifested themselves in the post COVID19 period are: anxiety was: high level - 21%, medium level – 26.6%, low level – 40.7% of respondents, as well as irritability: severe – 37.5 %, expressed – 27.2%, weakly expressed – 35.3%; confusion: severe – 36.6%, severe – 19.1%, mild – 44.1%; depressive disorders: severe – 36.5%, severe – 19.1%, mild – 44.1%; fear, despair, apathy: strongly expressed – 33.9%, expressed – 26.3%, weakly expressed – 39.8% of respondents; panic, panic attacks was: high level – 41%, medium level – 14,1%, low level – 44.9%, hysteria: strongly expressed – 42.7%, expressed –13.5%, weakly expressed – 47.6%; aggression, anger, irritability: strongly expressed – 41.9%, expressed – 18.9%, weakly expressed – 39.3% of respondents. A change in the value-semantic component of personality has been recorded. It was found that patients with depressive disorders are characterized by a low level of socio-psychological adaptation, while men have indicators of socio-psychological adaptation are significantly lower than women in terms of adaptability, self-acceptance, acceptance of others, emotional comfort, adaptation, self-acceptance, acceptance of others, as well as on integrated indicators of emotional comfort and internality. Indicators of maladaptation are higher in men than in women: maladaptation, self-rejection, rejection of others, emotional discomfort and external control. There is a tendency to decline the state of socio-psychological adaptation with age, which is manifested by a decrease in older age groups of adaptability, self-acceptance, emotional comfort, adaptation, self-acceptance, and integrated indicators of emotional comfort and internality, and increasing indicators of maladaptation, self-rejection, rejection, external control, payroll and escapism. This is necessary to improve anesthetic safety during surgery and to alleviate post-anesthetic intoxication after anesthesia.\u0000\u0000Therefore, the aim of our study was to compare the neuroprotective activity of original derivatives of sulfur and nitrogen-containing heterocycles (heterosides) and reference drugs, which are already known in pharmacology (cerebroprotective agent with neurotrophic action ¾ cerebrocurin and nootropic drug with neuroprotective action ¾ noopept).\u0000\u0000In the course of the research, it was found that after ketamine anesthesia, the excitability of the central nervous system increases, the anxious behavior of animals increases, while the indicators of the research work of animals in the experiment sharply deteriorate. Mishchenko","doi":"10.26565/2312-5675-2020-15-05","DOIUrl":"https://doi.org/10.26565/2312-5675-2020-15-05","url":null,"abstract":"Metabolic syndrome is a complex of metabolic, hormonal, and clinical disorders, which are based on insulin resistance and compensatory hyperinsulinemia, abdominal obesity, disorders of lipid, purine metabolism, and arterial hypertension, is a combination of factors, each of which, both individually and in combination with others, has a significant effect on the risk of occurrence and the characteristics of the course of cardiovascular diseases, in particular ischemic strokes. The clinical feature of metabolic syndrome is the frequent development of paroxysms of atrial fibrillation, as a manifestation of the late stages of \"hypertensive heart\", which is illustrated in a number of large-scale population studies on the relationship between metabolic syndrome and the risk of atrial fibrillation. Our work raises the question of the features of the relations between metabolic syndrome and ischemic stroke caused by atrial fibrillation. Information concerning the qualitative and quantitative characteristics of the components of the metabolic syndrome with a breakdown of the clinical features of ischemic cardioembolic stroke in persons with the pathology under discussion, their natural course, the prognosis of the outcome, and sensitivity to treatment is very low.\u0000\u0000The article presents the results of the analysis of factors that make it possible to predict the course and outcome of acute ischemic stroke caused by atrial fibrillation against the background of metabolic syndrome. In order to determine the prognostic criteria for the outcome of cardioembolic stroke, 86 patients were examined, 41 patients were diagnosed with a cardioembolic stroke on the background of metabolic syndrome, 45 patients with cardioembolic stroke without metabolic syndrome. The analysis was based on the construction of correlation pleiades. A similar factor structure of indicators in both studied groups of patients with cardioembolic stroke indicates the same nature of changes in the state against the background of acute vascular catastrophe. A more severe course of the disease, based on the data of correlation analysis and data obtained by comparing indicators, is noted in the presence of the metabolic syndrome. {"title":"Epidemiological characteristics of multiple sclerosis in Volyn region: a prospective study","authors":"O. Shulga, T. About 20,000 of adult Ukrainians suffer from MS. Prospective studies, taking into account the prevalence and incidence, combined with environmental, territorial and urbanization factors can optimize medical care for patients with MS. A prospective study of MS within Volyn Polissia and Opillya zones has not been conducted before.\u0000\u0000Objective: To evaluate prospectively the prevalence and morbidity rates of MS among the adult population of Volyn region within the physical-geographical zones.\u0000\u0000Materials and methods: Epidemiological and medical-statistical methods of morbidity study were used to study features of MS prevalence in the Volyn region. The standard descriptive statistics and factor analysis (principal component method) were used to estimate the variation of a set of indicators. Factor analysis included incidence and prevalence rates of MS in Volyn region since 1999.\u0000\u0000Results: A statistically significant difference in MS prevalence at the significance level p <0.05 was found in the MS prevalence in 2014 and at the p = 0.005 level in the 2015–2017 section. Also, a statistically significant difference in the incidence of MS at a significance level of p <0.1 was detected in the incidence of MS in 2015 and at the level of p = 0.001 in 2013. Thus, comparing the prevalence and incidence rates, we can conclude that the prevalence of MS in the Volyn Opillya region is significantly higher than in the Volyn Polissia area. Factor analysis (principal component method) was conducted to identify latent factors. As a result, it was found that the 3-factor model allows to describe more than 88% of the variation of the set of indicators: the 1st (main) factor describes 69%, the 2nd - 12,6%, the 3rd - 6,6% (together 88,2%).\u0000\u0000Conclusions: The prevalence and incidence rates of MS in the Volyn Oblast were found to be uneven. The prevalence of MS in the Volyn Polissia area is lower than in the Volyn Opillia region. The developed 3-factor model allows to describe more than 88% of the variation of the population prevalence of MS. Psychoeducation is one of the most important components of psychosocial interventions in a multifaceted system of psychosocial rehabilitation. The primary task of psychoeducation is to provide patients and their family caregivers with realistic knowledge about mental disorder, on the basis of which the patient and his or her family members can get more control over the symptoms of the disease. The aim of our work was from the standpoint of a systematic approach, based on the study of clinical and psychological manifestations and risk factors for the development of pathological functioning in a family, where a patient with endogenous mental disorder lives, to develop, substantiate scientifically and introduce a psychoeducational module as an element of psychosocial interventions in complex system of medical -psychological support. To achieve this goal, according to the principles of bioethics and medical deontology, a comprehensive examination of 243 patients with endogenous mental disorders (168 patients with paranoid schizophrenia, 75 patients with affective disorders) and 243 family caregivers was performed. The work was done in three stages: during the the first stage we examined patients and their family caregivers. During the second stage, an in-depth study of psycho-emotional, individual-psychological, interpersonal-communicative and psychosocial predictors of reducing the adaptive capacity of the family was performed. The third stage included scientific substantiation, development and implementation of an appropriate comprehensive system of medical and psychological support for the families, based on the analysis of data obtained during the previous stages.
