The future research is currently working towards development of objective measures such as biomarkers which can identify and quantify periodontal disease risk and its progression. Biomarkers are used to objectively evaluate normal biologic processes, pathogenic processes and pharmacologic response to therapy protocols. Biomarkers are tell-tale molecules that could be used to monitor health status, disease onset, treatment response and outcome. Rough surface implants provide predictable osseointegration but results in the development of peri-implantitis when exposed to the oral environment. The peri-implant mucosa provides protection to the underlying bone via its immune response and protection from apical biofilm migration. An adequate band of attached keratinized mucosa also improves comfort with performing oral hygiene, limit early marginal bone loss and improved aesthetic outcomes around implant prostheses. Aim: The purpose of this article is to provide a literature review on the importance of attached keratinized mucosa around dental implants. It also shares simple strategies to improve the peri-implant submarginal approach reduces the risks the procedure worsening soft tissue recession around the implant; (C) A case with implants in the lower right quadrant replacing teeth 35,36 and 37. The site has no attached keratinized mucosa and if tender to oral hygiene and bleeds on proving; (D) Submarginal AAPTF; (E) FGG sutured to the periosteum bed; (F) 3-month review of the site showing an excellent band of attached keratinized mucosa. Material and Methods: Forty seven patients with one singular missing tooth and 6 patients with two singular missing teeth in non-adjacent positions were included in this study. 29 tooth gaps were closed with FDPs and thirty with ISCs. Material costs, treatment hours and number of visits were recorded and compared for the two treatment groups. Results: Due to surgery, the total number of visits was higher for the ISC than for the FDP group (7.5±2.8 vs. 6.2±3.6; p=0.03), but the actual chair side time was the same (5.3±1.8 hours vs. 5.0±2.5 hours; p=0.22). The patient presents with class I dental occlusion and lower anterior crowding. Upon comprehensive orthodontic examination, it was suspected the patient had sleep disordered breathing and was sent for a polysomnography (PSG). The sleep physician diagnosed severe sleep apnoea with an apnoea hypopnoea index (AHI) of 54.1. Cone beam computerized tomography (CBCT) scan, which was taken as part of the orthodontic records, revealed airway issues so the patient was referred to an ear nose throat (ENT) doctor with a sub specialty in airway and sleep. ENT surgery was performed resulting in the AHI coming down to 20.4. Orthopaedic expansion was done using removable appliances followed by fixed appliances (braces) using the controlled arch technique and protraction mechanics. The post orthodontic treatment AHI was 14.4 and a Farrar splint was made as the retainer which resulted in an AHI of 6.4. Vitamin D is carried by vitamin D-Binding Protein (DBP) in the circulation and binds to host cells through the Vitamin D Receptor (VDR).
