Previous studies have stated that PCOS is associated with an increased risk of endometrial hyperplasia and cancer. This study aims to investigate the feasibility of using the HOPE system in KHUH for exploring the association between PCOS and Endometrial hyperplasia and cancer. Methods: A retrospective cross-sectional study to find the incidence of association between PCOS and Endometrial cancer cases following up in King Hamad University Hospital (KHUH) and Bahrain Oncology Center (BOC) in 2018. Results: A total of 241 women included in our study. A total of 165 women were diagnosed with PCOS in 2018, and a total of 55 women that underwent endometrial sampling, and 21 women diagnosed with endometrial cancer. No significant relationship between PCOS and EH and EC. High BMI was found significant in all sample groups. Conclusion: Our study showed that there is a link between PCOS and EC but the percentages are very low. Factors, such as obesity, smoking, and the consumption of high fat diet have been reported to play a role in the aetiology and progression of cancers in general. However, the exact relationship between obesity and prostate cancer in Africans is still poorly described. The present study aimed to evaluate the association between obesity as measured by the body mass index (BMI) and high-grade prostate cancer in Cameroonians. Methods: This was a retrospective analyses of the clinical records of 300 consecutive men who underwent transrectal ultrasound-guided core needle prostate biopsy between January 2010 and January 2020. Patients were grouped as obese (BMI ≥30kg/m²) and non-obese (BMI <30kg/m²) and binary logistic regression modelling was used to compare the association between obesity and patient characteristics, such as age, digital rectal examination (DRE) findings, prostate specific antigen (PSA) level, prostate volume, PSA density, and Gleason score (GS). Results: The mean age at diagnosis of PCa was 65.46 ± 8.54 years (Range: 34-89 years), the mean BMI was 28.93 ± 4.57 kg/m² (Range: 18.35-42.37 kg/m²), and the median total PSA level was 44.2 ng/ml. In total, 142 men (47.3%) were obese and biopsies detected high-grade prostate cancer in 160 men (63.3%). Obese men were significantly younger and had a higher risk (OR= 2.74, p <0.001) of having high-grade PCa compared to nonobese men. Furthermore, 94 obese men (58.75%) had high-grade PCa. Obese men had higher age-adjusted PSA densities (P<0.001), while there was no statistical difference in DRE findings (OR= 1.06, p= 0.225). Conclusion: This study demonstrated that obesity is associated with a higher risk of high-grade prostate cancer. Prostate biopsy remains the gold standard to confirm its diagnosis. Material and Method: This is a prospective study during 6 months, starting from 1st of February to 31st of July 2022. It included all patients who underwent a prostate biopsy that was carried out either following a high tPSA or due to an abnormal digital rectal examination. Results: We performed digitally guided biopsies in 32 patients from whom, average age was 72.43 years and elevation of total PSA was the main reason (71.88%) behind biopsies. The mean total PSA level was 45.69 ng/ml. The tissues removed were essentially prostate tissues, malignant in 28.12% of cases with adenocarcinoma as the only histological type. Conclusion: Prostate biopsy remains the gold standard for the diagnosis of prostate cancer. Method: A seven-year retrospective study was conducted from January 2012 to December 2019. The study concerned all men diagnosed with PCa graded using the novel Gleason grade group. Data were analyzed descriptively and analytically using Statistical Package for Social Sciences (SPSS) version 20.0. Bivariate analysis was done to identify independent associations. Results: A total of 253 files were reviewed; fulfilling the inclusion criteria. The mean age of our study population was 66.62±8.288 years with the most affected age being those aged between 59-69 years. The commonest finding on DRE was a hard-craggy prostate (46.0%). The mean initial total PSA was 98.78 ng/dL ± 300.17 ng/mL, while the mean PSA density was 1.73 ± 5.94ng/mL². Gleason grade group 2 was the commonest grade (30.6%). There was a positive correlation between initial total PSA and Gleason grade group (r = 0.314 (p-value 0.000)), and between PSA density and Gleason grade group (r = 0.919 (p-value 0.000)). Initial total PSA and the Gleason grade groups were independently associated with the radiological tumour stage (p-value 0.000 and p-value 0.03 respectively). Conclusion: There is a positive correlation between initial total PSA, PSA density, and novel Gleason grade group. We, therefore, recommend that more attention should be placed on PSA density in pre-biopsy decisionmaking. Moonshot was a presidential project to set a classic record of success attributable to the right approach and war on cancer was another presidential project to set a classic record of failure attributable to the wrong approach. Cancer moonshot was brought up by President Biden to urge health profession to rely on right approaches to save cancer patients. Perpetual replication of cancer cells (CCs) is the most outstanding feature of cancer. Naturally, killing of replicating CCs becomes the top choice of cancer establishments to combat cancer. Killing of CCs is actually a wrong approach of cancer therapy, because cancer is caused by wound unhealing due to the collapse of chemo-surveillance, which is a natural mechanism to ensure perfection of wound healing. Killing of CCs by cytotoxic agents promotes cancer stem cells (CSCs) to proliferate in order to repair the damages created by cytotoxic agents, eventually pushing CSCs to reach a high proportion to become unresponsive to further treatments. Killing of CCs is a wrong approach that causes the failure of cancer solution. To save cancer patients, we have to drastically change past practices. Apparently, wound healing process is the right approach of cancer therapy. Induction of terminal differentiation of PSCs is the most critical mechanism of wound healing, which is achieved by differentiation inducers (DIs) and differentiation helper inducers (DHIs). CDA is a collective term of the preparations consisting of different proportions of DIs and DHIs. Therefore, CDA formulations are the best solution of cancer to fulfill cancer moonshot and to win the war on cancer. CDA formulations display the features as pro-wound healing that can eliminate both CSCs and CCs by inducing these cells to undergo terminal differentiation, and to restore the functionality of chemo-surveillance, which is the creation of the nature to prevent the buildup of cells with abnormal MEs. A perfect cancer drug must have the ability to eliminate both CSCs and CCs, and to restore the functionality of chemo-surveillance. However, data on local control have not been confirmed at 5.6 years median follow-up in favor of the TNT arm because patients from the experimental (EXP) group received more often 3D-CRT (p=0.029). The primary end-point of this multicentric observational prospectic study is to assess the real life safety and efficacy of RAPIDO trial protocol using VMAT and IMRT treatment in all enrolled patients. Moreover FDG-PET and MRI images for predictive features compared with cancer phenotype (status of K-ras and EGFR) and study on tumor microenviroment response to short course RT will be also provided. Methods: Locally advanced rectal cancer patients undergoing TNT as defined in RAPIDO trial will be enrolled and prospectively included in this study. MRI and FDG-PET will be performed to staging and restaging the disease. The tumor microenviroment changes assessment with immunophenotype and soluble PDL1 and on EVCs will be done. For sample size calculation, using OR 0.5, 100 patients will be enrolled into two years. Kaplan -model and proportional Cox method will be adopoted for OS, DFS and predictive factors for local control. Conclusions: The results of this study will be useful to assess prospectively the safety and effectiveness of VMAT and IMRT in short course TNT assuming predictive factors for response through radiomics, metabolomics and immunological features. Hijazi","doi":"10.33425/2833-0390.1015","DOIUrl":"https://doi.org/10.33425/2833-0390.1015","url":null,"abstract":"Objectives: To analyze the setup accuracy among patients treated for breast cancer with hypofractionation radiation therapy (HFRT) regimen (five fractions instead of 15-16 fractions in standard regimen) and predict the necessity of performing the setup imaging in the 4th and 5th fractions as a function of setup accuracy in the first three fractions. Methods: This retrospective study reviewed setup displacements in each direction (lateral, longitudinal, and vertical) for 51 women with breast cancer treated with HFRT at the Radiation Therapy Unit between September 2020 and May 2022. Besides the five fractions (#1– # 5), the mean setup error was computed for the first three fractions (AVG-III) for each direction. Setup accuracy rates were computed for each direction and fraction as the percentage of fractions with setup error ≤ 0.5 cm. The correlations of #1, #2, #3, and AVG-III setup errors and their value in indicating #4 and #5 setup error and accuracy were analyzed using Pearson's coefficient and Receiver Operating Characteristics (ROC) curve, respectively. Furthermore, the effect of body mass index (BMI) on setup reproducibility was analyzed using logistic regression Result: The mean (SD) age of the participants was 54.41 (11.46) years. There was a high percentage of overweight (25.5%) and obese (53.0%). The mean setup error was 0.05) and #5 (r = -0.128–0.254; p>0.05) within any of the first three fractions or AVG-III, in any of the directions. In the ROC curve, only #5 accuracy was indicated by #3 in the longitudinal direction (AUC=0.89, p=0.025). BMI was only associated with inaccurate setup for fraction #3 in the lateral direction, in a positive relationship (OR=1.15, 95% CI = 1.01–1.30; p=0.031). Conclusion: Setup accuracy in the first fractions of HFRT does not predict accuracy in the two last fractions nor is predicted by the patient’s BMI. In addition to sorafenib and lenvatinib that represent the standard-of-care options in the first-line treatment, the multikinase inhibitors regorafenib and cabozantinib or ramucirumab, the anti-vascular-endothelial growth factor-2 (VEGF-R2) in the second-line setting but their poor tolerability have brought out the need for new therapeutic strategies, the immune checkpoint inhibitors (ICIs) might represent the most important novelty and the future perspective also in the field of HCC. Therefore, the need to identify predictive biomarkers to select those patients who might actually benefit from ICIs-based treatment is urgently a challenge. Our central hypothesis is that the global assessment of predictive biomarkers could help patient-specific choices for ICIs treatment by developing simplified therapeutic algorithms and novel prognostic index for efficient HCC management. Objectives: To determine the prevalence, aetiologic risk and other contributing factors, complications as well as outcome of Obstetric Uterine rupture at Federal Teaching Hospital, Katsina. Study Design: A Five year retrospective study that reviewed uterine rupture in Obstetrics and Gynaecology department of Federal Teaching Hospital, Katsina from 1st January 2010 to December 31st 2014. Results: There were 42 uterine ruptures out of 9995 deliveries, with an incidence of 4.2 per 1000 deliveries. The mean age of the patients was 31yrs± 0.9. The highest incidence was among unbooked patients (85%). The diagnosis was made pre-operatively in 88.1% of the cases. Major risk factors were prolonged obstructed labour (26.2%), previous caesarean section scar (19%) and Oxytocin infusion in unscarred (14.3%) and scarred Uteri (11.9%) as well as use of Misoprostol (11.9%). Majority of the ruptures occurred anteriorly (52.4%) while scar dehiscence accounted for 23.8% of cases. Repair with bilateral tubal ligation was the most frequent mode of treatment (47.6%), while hysterectomy was done for patients with extensive ruptures (34.3%). The commonest complication was postoperative Anaemia (38.1%). There were two maternal deaths, with a case fatality rate of 4.8%, while the perinatal mortality was 78.6%. Conclusion: Uterine rupture is still a major cause of maternal and perinatal morbidities and mortality in our setting. The patient learned about this event through a nurse who attended the party. The issue of medical confidentiality is very important in medical oncology practice. I decided for this reason to address this topic. The objective of this commentary is to analyze four ethical dilemmas concerning medical secrecy: Does a doctor have the duty to remain silent while off duty? How to resolve an ethical conflict? Is the extension of secrecy lawful? In addition, can confidentiality be violated in any circumstance? A broad ethical overview allows us to conclude that the duty of silence accompanies the doctor in all times of life. The nurse mismanaged this conflict. The secret can be shared, and it is relative because sometimes it is lawful to violate confidentiality. Finally, this article presents a fable about the virtue of prudence.
