Nonogaki","doi":"10.5180/JSGOE.36.2_106","DOIUrl":"https://doi.org/10.5180/JSGOE.36.2_106","url":null,"abstract":"Objective: Perioperative outcomes in the first year of introducing robot-assisted hysterectomy (RAH) using the daVinci Xi surgical system were evaluated based on comparisons with conventional laparoscopic hysterectomy (CLH) for simple hysterectomies performed during the same period. Methods: This was a retrospective study involving comparisons between 38 cases of RAH from June 2019 to April 2020 and 28 cases of CLH from August 2018 to April 2020. All cases involved simple hysterectomies performed by 4 doctors (1 endoscopic technique-certified medical doctor and 3 doctors with less than 10 years of experience). Results: The characteristics of patients who underwent RAH and CLH were as follows, respectively: age, 47 vs 49 years (mean, p=0.62); body mass index, 23.4vs 22.9 kg/m (mean, p=0.57); uterine weight, 152 and 175 g (median, p=0.39); manipulator use, 28 (73%) and 21 (75%) cases (frequency, p=0.90); and 0/4 cases with endometriotic adhesions with Douglas fossa closure, 0 and 4 cases (frequency, p=0.028). The total operation time (from skin incision to skin closure) was significantly shorter in RAH cases (mean difference, 29 minutes; p=0.048) than in CLH cases. There was no significant difference between the two groups in terms of blood loss and perioperative complications. Conclusion: We safely introduced RAH by selecting appropriate cases in advance. Fibroids are commonly associated with infertility and/or menorrhagia, especially if they are submucosal. Endometriosis is diagnosed in more than 35% of patients undergoing a laparoscopy for infertility. Both diseases are oestrogen-dependant and suppression of the secretion of gonadal steroids by administration of a superactive agonist of LH-RH has been proposed as the non-surgical approach to fibroids and endometriosis. Because most leiomyomata and endometriotic cysts return to near pretreatment size within 4 months after cessation of GnRH-a therapy, these agents cannot be used as difinitive medical therapy but must be considered as an ad juvant for preoperative-reduction of tumor size. Because the GnRH-a therapy induces a menopausal status, it cannot be used during a prolonged period. She had no respiratory syndrome. Blood tests revealed an elevated CA125 level (1,224 U/mL), but other tumor markers were within their normal limits. Chest radiography and abdominal computed tomography (CT) detected no signs of malignancy or tuberculosis, but positron emission tomography/CT disclosed abnormal FDG uptake in the omentum, mediastinal lymph node, and diaphragm. Microbiological testing and cytological examination of ascites yielded negative results. We conducted diagnostic laparoscopy while wearing N95 masks and found numerous tiny nodular lesions on the peritoneal surfaces. Pathological examination showed epithelioid granuloma and Langhans giant cells with caseous necrosis, which are characteristic to tuberculosis. Although the Ziehl-Neelsen staining result was negative, interferon-gamma release assays were positive. Tuberculous peritonitis (TBP) was diagnosed, and a combination drug regimen of isoniazid, rifampicin, and pyrazinamide was administered. She has been free from recurrence since the completion of chemotherapy. Throughout the clinical course, Mycobacterium tuberculosis was undetected. TBP is a form of abdominal and pelvic tuberculosis that accounts for about 0.04% of all cases of tuberculosis. The risk is increased in patients with cirrhosis, acquired immune deficiency syndrome, diabetes mellitus, steroid use, or underlying malignancy, and those undergoing continuous ambulatory peritoneal dialysis. TBP, which might be confused with widespread ovarian cancer, should be included in the differential diagnosis of ascites. Because preoperative diagnosis of TBP is difficult, laparoscopic surgery is helpful to distinguish TBP from ovarian cancer. However, TANKO is often more technically difficult than multi-port laparoscopic surgery (MLPS) because of the restricted mobility; furthermore, several prospective randomized controlled trials have shown that TANKO has only cosmetic advantages. Moreover, if TANKO is performed on a patient with a shallow umbilicus and sparse subcutaneous adipose tissue, the umbilical scar may be highly visible because of the requirement of a larger periumbilical skin incision for TANKO, compared to MLPS. To resolve this problem, we describe a method of umbilicoplasty using TANKO for removal of ovarian tumors in three patients.
