However, this technique in not used to its full capacity, especially in local hospitals, such as those of the Nagano prefecture. Laparoscopic surgery was introduced in our hospital in 2008. Predominantly, laparoscopic operations for benign ovarian tumors were performed until 2015. Since January 2016, a lead surgeon from another hospital has been regularly educating gynecologists at our hospital about laparoscopic surgery. We have also begun performing total laparoscopic hysterectomies (TLHs) and laparoscopic myomectomies (LMs), and the frequency of laparoscopic surgeries at our hospital has increased remarkably, from about 8 to 52 cases per year. We aspire to learn how to change the motivation of gynecologists and nurses through changing incentives before 2015 and after implementation of lead surgeon education in 2016. Design: We prepared questionnaires about changes in laparoscopic surgery incentives for gynecologists and nurses before 2015 and after 2016. Setting: This report analyzed the results of the questionnaire surveys from 5 gynecologists and 75 nurses. We compared the desire for laparoscopic surgery, preparation for the operation, and impression on the attitude of the doctors before 2015 and after 2016. Primary Outcomes: Although we suspected that nurses may be frustrated by the long operation times, our analysis showed that nurses had a relatively good impression of the gynecologists. No serious problems were identified. Normal myometrium and uterine fibroids secrete PRL, but the function of myometial PRL is unknown. We report a case of complete remission of hyperprolactinemia after laparoscopic myomectomy. The patient was 37 years-of-age, gravid 0, para 0. She was referred to our hospital elsewhere because of a diagnosis with hyperprolactinemia and uterine myoma. At the age of 35, she had been diagnosed with secondary amenorrhea and hyperprolactinemia. Brain MRI and CT scans failed to detect any morphological abnormality. She had been taking cabergoline followed by terguride for nearly two years, but her hyperprolactinemia failed to respond to the deopamine agonists despite a gradual increase of their dosages. However, the hyperprolactinemia unexpectedly regressed completely after the patient underwent laparoscopic myomectomy. The patient was a 41-year-old woman, gravida 6 para 1, who underwent a total laparoscopic radical hysterectomy for the treatment of clinical stage IB1 cervical adenocarcinoma. The right external iliac artery was lacerated during the monopolar dissection of a broad ligament, which induced severe bleeding. The bleeding was stopped by grasping of the lacerated site with an atraumatic grasper. An endovascular clip was placed both proximal and distal to the lacerated site, which was repaired with an uninterrupted intracorporeal suture. The endovascular clips were then removed and hemostasis was confirmed. The right external iliac artery was occluded for a total of 28 minutes. The laparoscopic surgery did not require a laparotomy. Here, we report a rare case of unusual bilateral ovarian fibroma torsion in a young woman. A 38-year-old nulliparous woman presenting with an acute abdomen had a history of laparoscopic right partial ovarian resection for bilateral ovarian fibroma and cystectomy for a left ovarian chocolate cyst at 24 years of age. Magnetic resonance imaging showed bilateral solid adnexal masses with microcysts similar to the tumor seen 14 years prior, with the largest measuring 10 cm. We diagnosed torsion of re-enlarged ovarian tumors and performed emergency laparoscopic bilateral salpingo-oophorectomy rather than conservative surgery because it was difficult to macroscopically identify normal ovaries. For a young woman with bilateral fibroma, partial tumor excision may be selected to preserve ovarian function and fertility. Methods: Thirty-two patients with MCT who were surgically treated at our hospital between January 2013 and March 2015 were included in this study. We investigated the correlation between tumor diameter and tumor marker expression (squamous cell carcinoma antigen [SCC], carcinoembryonic antigen [CEA], carbohydrate antigen [CA]125, and CA19-9). Results: The median tumor diameter was 7.8 cm (range 5.0-16.0 cm). The median values of SCC, CEA, CA125, and CA19-9 were 1.25 ng/mL (range 0.6-3.5 ng/mL), 1.4 ng/mL (range 0.5-10.9 ng/mL), 16 U/mL (range 5.0-86.1 U/mL), and 34.6 U/mL (range 2.0-949.4 U/mL), respectively. The levels of SCC and CEA showed significant positive correlations with tumor diameter (respective correlation coefficients = 0.499 and 0.460), while the levels of CA19-9 and CA125 showed no significant correlation with the tumor diameter. We also encountered an example of malignant transformation of MCT during the study period. The tumor diameter in this case was 18.8 cm, and the preoperative level of SCC was 25 ng/mL, which is remarkably high, even considering the tumor diameter. Conclusion: In this study, the levels of SCC and CEA showed significant positive correlations with tumor diameter. High preoperative SCC and CEA levels may suggest that the tumor diameter should be considered when planning laparoscopic surgery. A recent increase has been observed in the number of women who develop multiple and giant uterine myomas, indicating the need to react according to each case. In this report we present a method of laparoscopically assisted hysterectomy that we introduced as a countermeasure against multiple and giant uterine myomas. We review and compare the 312 cases of laparoscopic hysterectomy that we have performed and discuss on the selection of operation method based on the review.
