Al-Wesabi, Abdullwahab Aldialami, Jameel Helmi","doi":"10.20428/yjms.11.1.s1","DOIUrl":"https://doi.org/10.20428/yjms.11.1.s1","url":null,"abstract":"Objective: To compare between the conventional and modified preauricular incision approaches to the management of temporomandibular joint (TMJ) ankylosis among Yemeni patients in Sana’a city, Yemen. \u0000Methods: This comparative clinical study recruited ten patients (7 males and 3 females; age range: 6–35 years) with true fully or partially bony or fibrous TMJ ankylosis. These patients were admitted to and followed up in the Department of Oral and Maxillofacial Surgery, College of Dentistry, University of Science and Technology Hospital in Sana’a in the period from May 2013 to February 2016. Five patients with condylar capsule ankylosis were subjected to the conventional preauricular approach, while the other five cases with ankylosis that extends beyond the condylar capsule and reaches the zygomatic arch were subjected to the modified preauricular approach. Both approaches were evaluated and compared for the time required, intra- and post-operative complications and aesthetic results. \u0000Results: The mean times required to carry out the operations were 4.45 and 3.20 hours for conventional and modified preauricular approaches, respectively.Facial nerve injury was present in two patients undergoing the conventional preauricular approach, but this reversed to normal after four to five months. Patient of both groups experienced no facial paralysis or post-operative auriculotemporal syndrome. Intra-operative bleeding was mild to moderate among the patients of both approaches, and none of the patients required blood transfusion intra- or post-operatively. Major blood vessels were injured in three cases undergoing the conventional preauricular approach. All patients subjected to both approaches showed satisfactory aesthetic end results. \u0000Conclusions: Conventional and modified preauricular approaches provide excellent accessibility and visibility to the surgical field during the management of TMJ ankylosis among Yemeni patients, with the latter being slightly superior. El-Zine","doi":"10.20428/YJMS.11.1.CS1","DOIUrl":"https://doi.org/10.20428/YJMS.11.1.CS1","url":null,"abstract":"Objective: To analyze the outcomes of neurosurgical treatment of dorsal spinal tuberculosis (TB) among Yemeni patients. \u0000Methods: Forty-three patients with histopathology-confirmed dorsal spinal TB, who had been admitted to Kuwait University Hospital of Sana’a University and the University of Science and Technology Hospital, Sana’a, were followed up for the outcomes of neurosurgical treatment in the period from Ott. 2009 to Oct. 2005. Data about the clinical presentation, radiologic findings and the outcomes of surgical treatment were recorded and analyzed. \u0000Results: Of patients undergoing neurosurgical treatment of dorsal spinal TB, 28 were males and 15 were females, with a mean age of 41.3 years (range: 18–70). Nearly a half of patients (50.6%) were farmers and 80% of patients were admitted to hospitals three months after the disease onset. All patients presented with back pain, while paraplegia/paraparesis and sensory impairment were reported in 81.4% and 87.4% of patients at presentation, respectively. Thirty models were divided into three equal groups: ten models not supported with glass fibers (control group), ten models with one layer of glass fibers (experimental group I) and ten models with two layers of glass fiber (experimental group II). All models were exposed to a three-point bending test, and fracture loads were analyzed using a one-way analysis of variance (ANOVA) followed by Bonferroni post-hoc test. \u0000Results: IOD models reinforced with two layers of glass fibers (experimental group II) showed a mean ultimate load at fracture of 48.69 ± 3.71 Newton (N) compared to mean loads of 32.78 ± 2.41 N and 24.42 ± 2.73 N for the models reinforced with one layer (experimental group I) and non-reinforced with glass fibers (control group), respectively. Data on sociodemographic and clinical characteristics as well as possible risk factors of T. vaginalis infection were collected from 287 pregnant women using a pre-designed questionnaire by trained researchers. Vaginal discharges were then collected and examined microscopically for motile T. vaginalis trophozoites. \u0000Results: The overall prevalence of T. vaginalis infection was 11.1%, with a higher infection rate among pregnant women aged 26-40 years old, housewives and rural residents. Age, literacy status and the age at first sexual intercourse were significantly associated with T. vaginalis infection among pregnant women. Presence of vaginal discharge (OR = 8.33; 95% CI: 2.47–28.03, P <0.001), itching (OR = 2.57; 95% CI: 1.02–6.48, P = 0.027) and presence of unpleasant odor (OR = 4.37; 95% CI: 1.63–11.70, P = 0.001) were the clinical manifestations significantly associated with T. vaginalis among pregnant women. \u0000Conclusions: T. vaginalis is prevalent among pregnant women seeking health care in Sana’a, particularly housewives, illiterate women and those coming from rural areas. Presence of vaginal discharge with unpleasant odor and itching are associated with a higher risk of infection among symptomatic women. Albashari, Hisham Ebrahem","doi":"10.20428/yjms.11.1.a3","DOIUrl":"https://doi.org/10.20428/yjms.11.1.a3","url":null,"abstract":"Objective: To assess the drug prescription knowledge among dental students and interns in public and private Yemeni universities. \u0000Methods: A descriptive, observational study of drug prescription knowledge and practices was conducted through the distribution of self-administrated questionnaires to a total of 464 dental students and interns enrolled in four universities in Yemen in 2015. The study investigated the factors to be considered before drug prescribing, indications for prescribing antibiotics in dental situations, common antibiotics and analgesics prescribed, difficulties in writing a prescription and knowledge of the World Health Organization’s Guide to Good Prescribing. Findings were presented as descriptive statistics of frequencies and percentages, and differences and associations were tested at P-values <0.05. \u0000Results: The majority of respondents were females (65.9%; 304/464), being enrolled in public universities (54.1%; 251/464) and from the fifth level of study (54.9%; 251/464). The mean respondents’ age was 23.0 ±1.3 years (range: 22–32). Presence of systemic diseases (75.0%), pregnancy status (72.2%) and presence of allergies (68.8%) were the factors most frequently considered by the respondents prior to drug prescribing. On the other hand, presence of abscesses was the most frequent reason for prescribing antibiotics (36.6%). Amoxicillin was the most frequently prescribed antibiotic (74.1%), while paracetamol (58.2%) and ibuprofen (47.0%) were the most frequently prescribed analgesics. Not knowing the brand names of prescribed drugs and wrongly filled prescriptions were the most frequent errors in writing prescriptions, being reported by 79.3% and 55.3% of respondents, respectively. Regarding the factors associated with common prescribing errors, female students had significantly more errors than their male counterparts regarding the posology and knowledge of brand names. In addition, fifth-level students reported significantly fewer errors in writing a drug prescription than fourth-level students, and the interns showed fewer errors than their undergraduate colleagues. Moreover, students from public universities reported significantly more errors in knowing brand names and duration of treatment than private university students. \u0000Conclusions: Dental students enrolled at the fourth and fifth levels as well as in the internship year of dentistry have insufficient knowledge and skills in prescribing drugs used in the dental field because of the perceived gaps in clinical pharmacology courses during their study. Biodata and clinical data were collected using a pre-designeddata collection sheet. Hemoglobin (Hb) concentration was measured, and anemia was defined as Hb concentration less than13.0 g/dLin males and less than 12.0 g/dL in females. The factors associated with anemia were analyzed, and independent predictors of post-transplant anemia (PTA) among renal transplant recipients were identified using a multivariable logistic regression model. \u0000Results: PTA was detected in 23.0% of renal transplant recipients with functioning grafts. Bivariate analysis showed a significant association of PTA with age of 50 years or older (Odds ratio (OR) = 2.7; 95% CI: 1.10–6.72; P = 0.03), history of acute rejection (OR = 3.6; 95% CI: 1.17–11.28; P = 0.019) and delayed graft function (OR = 6.2; 95% CI:1.60–24.16; P = 0.004). Multivariable analysis using a logistic regression model identified history of acute rejection (adjusted OR = 3.9; 95% CI: 1.11–12.94; P = 0.034) and delayed graft function (adjusted OR = 4.6; 95% CI: 1.07–19.81; P = 0.04) as independent risk factors for PTA among recipients. However, no association was found between PTA and recipient’s gender, graft source, immunosuppressive protocols, erythropoietin treatment or use of antihypertensive drugs. \u0000Conclusions: The prevalence of PTA among Yemeni renal transplant recipients is high, with history of acute rejection and delayed graft function being the independent risk factors. Mahmood (alaashm91@gmail.com) \u0000Abstract: \u0000Objective: To assess the effect of methotrexate and omega-3 combination on cytogenetic changes of bone marrow and activities of some enzymatic antioxidants. \u0000Methods: Fifty-six mature male Wistar rats were divided into two experimental groups and a control group. The first experimental group was sub-divided into three sub-groups depending on the concentration of methotrexate (MTX): X1 (0.05 mg/kg MTX), X2 (0.125 mg/kg MTX) and X3 (0.250 mg/kg MTX), which were given intraperitoneally on a weekly basis for eight weeks. The second experimental group (MTX and omega-3 group) was also sub-divided into three sub-groups (Y1, Y2 and Y3), which were injected intraperitoneally with 0.05, 0.125 and 0.25 mg/kg MTX, respectively, weekly for eight weeks accompanied by the oral administration of 300 mg/kg omega-3. The rats of the control group were given distilled water. The enzymatic activity of catalase (CAT), superoxide dismutase (SOD) and glutathione reductase (GR) were measured in the sera of rats. In addition, the mitotic index (MI) and chromosomal aberrations of bone marrow were also studied. \u0000Results: MTX resulted in a significant decrease in the activities of CAT, SOD and GR compared to the controls. It also increased the MI and chromosomal aberrations of rat bone marrows. On the other hand, omega-3 significantly increased the activities of the investigated enzymatic antioxidants and reduced the MI and chromosomal aberrations in treated mice when given in combination with MTX. \u0000Conclusions: MTX has a genotoxic effect on the bone marrow by increasing the MI and all types of chromosomal aberrations and decreasing the enzymatic activity of CAT, SOD and GR. It was distributed among all clinicians in UST Hospital.Results: Of 40 residents and consultants from the UST Hospital invited to participate in the present study, 52.5%agreed to join the study. Of them, 81.0% reported the notice of diagnostic radiological errors during their daily practice,wherethe majority of daily noticed errors (64.7%) were in the range of1–10 diagnostic errors. More than half of radiological errors were reported in the interpretation of ultrasound, and abdomen was described as being the most frequent body part with diagnostic radiological errors (65.0%). The respondent clinicians thought that poor communication between radiologists and clinicians was the most frequent cause of diagnostic errors, being reported by29.0% of clinicians.However, 24.0% and 19.0% of clinicians attributed errors to the lack of experience and knowledge of radiologists, respectively.Conclusions: Errors in radiological diagnosis are still common in Yemen. Establishing a good healthcare system with a proper communication between the radiologists and clinicians is critical to identify and minimize these errors. Female rats were investigated for the cytological changes in the phases of the estrous cycle. All rats were observed for behavioral changes throughout the experiment. Histopathological examination of the thyroid, testes and ovaries and the assessment of thyroid and gonadal hormones in the sera of control and experimental rats were performed.Results: Cytological examination revealed stopped estrous cycle in the diestrous phase in all female rats. Thyroid hormones revealed a decrease in the levels of triiodothyronine and thyroxin; however, non-significant changes were detected in the thyroid-stimulating hormone level in experimental rats compared to the controls. Gonadal hormones revealed a great discrepancy in their levels among both sexes.Conclusions: The results of the present study show that sexual excitation is one of the stressful factors affecting sexual behavior, hypothalamic-pituitary-thyroid and hypothalamic-pituitary-gonadal axes as well as sex organs with secretory functions. Alemad, Maha Abdul Alaziz","doi":"10.20428/yjms.10.1.a6","DOIUrl":"https://doi.org/10.20428/yjms.10.1.a6","url":null,"abstract":"Objective: To investigate the knowledge, attitudes and practices (KAPs) of Yemeni women attending primary healthcare centers (PHCCs) in Sana’a city towards family planning (FP).Methods: A descriptive, cross-sectional study was conducted among women attending three PHCCs in Sana'a city; namely, in Hadah, Al-Soneinah and Madhbah zones, between 21 November and 1 December 2011. The study included a sample of 281 married women, where data about socio-demographic characteristics and the KAPs towards FP were collected by interviewing women using a pre-designed, structured questionnaire and then analyzed using appropriate statistical tests.Results: Of the married women attending the PHCCs in Sana'a, the majority of respondents were from urban areas (96.4%; 271/281), aged between 25–29 years old (23.1% 65/281), employed (75.8%; 213/281) and with primary or secondary levels of education (60.9%; 171/281). In addition, the majority of women had a marriage length of 6–11 years (65.5%; 182/281) and 3–4 children (44.8%; 126/281). The majority of respondents (89.7%) knew about FP, and 60.2% considered it as birth spacing. Moreover, most respondents (87.5%) were aware of at least four methods of FP, and 53.6% heard of modern FP contraceptive methods. Of them, 85.9% and 74.0% heard of contraceptive pills and intrauterine contraceptive devices (ICDU), respectively; however, the least known contraceptive method was the use of male condoms (28.1%). Healthcare providers were the source of information on FP for the majority of respondents (60.5%). The majority of respondents believed that the optimum spacing between births should be two or three years, being 31.7% and 38.8%, respectively. In addition, most respondents (80.8%) believed that both couples must share the decision-making on FP. Socio-cultural beliefs and values were thought to be the most common (57.3%) barriers to the practice of FP.Conclusions: Although the majority of Yemeni women seeking healthcare after marriage have a good level of knowledge of several traditional and modern methods of FP and their preference of long birth spacing, the use of such methods is still limited to contraceptive pills and ICDU.
