However, some of them obtained conflicting data confirming both the advantage of carotid stenting (SAPPHIRE) and carotid endarterectomy (EVA-3S), as well as the equivalence of the two treatments (CAVATAS, SPACE, CREST). Also, unsatisfactory results of both single-stage and staged surgical approaches were shown in the treatment of patients with combined atherosclerotic lesions of the coronary and carotid arteries. The lack of clear international guidelines for the management of patients with lesions of several vascular beds makes it necessary to look for new methods of surgical treatment based on minimally invasive endovascular technologies. We describe an intriguing case of cardiac syncope resulting from an free-floating thrombus in the aorta. Case presentation: A 27-year old male smoker presented with recurrent episodes of syncope. Diagnostic investigations (echocardiography, computed tomography, magnetic resonance imaging) unveiled the presence of thrombi within the ascending aorta and the left ventricle. The patient underwent successful surgical removal of free-floating thrombus in aorta and left ventricle. The proper anticoagulation was initiated before and continued after surgery along with antiaggregant therapy to prevent embolization and recurrence. Conclusion: Given the lack of extensive data, the management of aortic thrombosis remains variable, encompassing strategies ranging from anticoagulant pharmacotherapy to surgical intervention, or a combination of both. The absence of established guidelines further underscores the intricacies in the therapeutic approach to this condition. Physiological changes in the cardiovascular system occur during pregnancy: heart rate increases by 10-25% and cardiac output by 45-50%. Also, pregnancy is associated with increased sympathetic and decreased parasympathetic activity. This, together with hormonal changes, can lead to a change in the structure and function of the myocardium and have a proarrhythmic effect (1-4). In addition, maternal age, prevalence of cardiovascular risk factors, e.g. obesity, and comorbidity, including congenital heart diseases, are increasing (6). The presence of arrhythmias during pregnancy varies across different populations, and more prevalent in the setting of structural heart disease (SHD). While most arrhythmias observed in pregnant patients are generally benign, such as sinus arrhythmia (60%), supraventricular tachycardia (SVT, 14%), and premature beats (19%), life-threatening arrhythmias like significant SVT or ventricular tachycardia (VT) are far less common (6). Vaidya et al. found a significant increase in hospitalizations due to atrial fibrillation (AF) by 111% and VT by 127% over 12 years (2). Of particular concern, AF is emerging as the most commonly diagnosed sustained arrhythmia during pregnancy (2). The presence of arrhythmias during pregnancy has significant implications for both the mother and the developing fetus. Maternal outcomes can involve palpitations, chest pain, syncope, heart failure, and an increased risk of thromboembolic events, preterm labor, and delivery. Poorly controlled arrhythmias may result in reduced blood flow to the placenta, potentially leading to fetal distress, growth restriction, or even fetal demise. Additionally, certain antiarrhythmic medications may pose risks to the developing fetus, necessitating careful consideration of maternal treatment choices to ensure fetal safety. Diagnosing arrhythmias during pregnancy presents challenges due to overlapping symptoms with normal pregnancy-related physiological changes. Noninvasive techniques like electrocardiography, echocardiography, and ambulatory monitoring play a vital role in diagnosing arrhythmias and evaluating their severity. The treatment options must carefully weigh the potential risks and benefits for both the mother and the fetus, depending on the presence of underlying structural heart disease, type and severity of the arrhythmia, and the stage of the pregnancy. Interventions may include adenosine, beta-blockers, and specific arrhythmic drugs with safety data during pregnancy, rarely, catheter ablation with minimal/ zero fluoroscopy can be used (3, 4). Methods: Research design - randomized experimental study, the population of sixteen Wistar rats is divided into a main and control group. The main group was subjected exclusively to a protein diet, and the control group was on standard feed for a month. On the 15th and 30th days, the animals were phased out of the experiment and a histological analysis of the lungs was carried out to detect changes in the bronchio-alveolar-vascular microaxis. Age and body mass of animals were measured and compared at the beginning of the experiment and before autopsy. The independent and paired samples t-test has been applied for statistical analysis. Results : Animals in both groups were comparable in age and body weight. Over time, the body mass increased in each group (217.5 (18.5) vs 246.2(18.2) gr., (p=0.0001) for the main group and 211.8 (10.6) vs 240.6 (11.7) gr, (p=0.0001) for the control group), but the weights of the main group did not differ from the control group (246.2 (18.2) gr vs 240.6 (11.7) gr, p=0.47). The isolated protein diet has led to an increase in collagen and elastin in the stroma surrounding the ventilation and perfusion pathway, leading to dystrophic changes not only in the organ’s stroma, but also in the bronchial walls and blood vessels of the lungs during a 30-day experiment. The above morphological changes showed significant remodeling of the microaxis of the broncho-alveolo-vascular exposome. At 15-day, only adaptive compensatory changes were observed with small differences from the control group. Conclusion: The 30-day isolated protein diet disrupts the interaction and interdependence of the functioning and structural organization of the components of the bronchial-alveolar-vascular microaxis of the lungs. Relatively 15-day results showed adaptive and compensatory changes. {"title":"Giant myxoma of the right sections of the heart obstructing the inferior vena cava: A case report","authors":"Khamidulla Abdumajidov, Kh D Buranov","doi":"10.24969/hvt.2023.405","DOIUrl":"https://doi.org/10.24969/hvt.2023.405","url":null,"abstract":"Objective: We present a rare case of surgical management of giant myxoma of the right heart with obstruction of the orifice of the inferior vena cava (IVC), obstruction of the right atrioventricular orifice, with the development of high pulmonary hypertension, and severe heart failure.\u0000\u0000Case presentation: A 39 years old male patients applied to the consultative polyclinic with typical complaints of severe shortness of breath, palpitations, severe weakness, almost bed rest, severe swelling of the lower extremities, with a transition to the anterior abdominal wall (edema). On physical examination signs of severe heart failure NYHA class IV were revealed. Imaging demonstrated giant right atrial myxoma obstructing IVC with base originated in fossa ovalis, obstructing tricuspid valve and protruding in the right ventricle occupying 50% of its cavity.  The surgical removal of the giant RA myxoma protruding the right ventricle and obstructing IVC and tricuspid valve was performed under cardiopulmonary bypass and hemodynamic support. The patient was discharged on the 10th day after surgery and only residual minimal tricuspid regurgitation was revealed on follow-up examination. \u0000\u0000Conclusion: The described case from our practice proves the possibility of a severe course of giant myxomas of the right heart, with the development of deep hemodynamic complications of systemic and pulmonary circulation. The chosen correct tactics of surgical treatment provided a favorable result, despite the severity of the initial condition and the degree of hemodynamic disturbance. Intraoperative tactics and technique for performing the stages of urgent surgery were chosen based on the situation, the features of the anatomical structure of the tumor, and the severe complications that arose. Our case proves the possibility of adequate implementation of such interventions, when choosing the right tactics of surgical treatment.\u0000\u0000Take home message: If clinical signs of circulatory failure, heart failure are detected, it is necessary to perform echocardiography. The objective of the present study is to report the treatment plan, roadblocks involved in management, pre-operative, and post-operative patient outcomes. Case presentation: In this case report we have outlined in detail about the unusual presentation of a 58-year-old male patient with a ruptured mycotic abdominal aorta aneurysm. The pre-operative workup that led to the diagnosis of this uncommon condition and the use of a femoral vein as a bypass conduit to repair the infected diseased aorta was elucidated. Conclusion: From our findings, we can state that early diagnosis using combination of clinical, laboratory, and radiological findings was proven to be effective. Pashimov","doi":"10.24969/hvt.2023.403","DOIUrl":"https://doi.org/10.24969/hvt.2023.403","url":null,"abstract":"Hypertrophic cardiomyopathy is a common genetic heart disease characterized predominantly by non-dilated hypertrophy of the left ventricle, in the absence of other cardiac, systemic, or metabolic diseases that can cause the observed degree of hypertrophy. The main features of left ventricular outflow tract obstruction include anatomical - basal septal hypertrophy in combination with a relatively small left ventricular cavity, leading to a change in the geometry of the outflow tract from the left ventricle, pushing the mitral valve apparatus forward, and functional - systolic anterior motion of elongated mitral valve leaflets with subsequent mitral-septal contact. Primary anatomical changes of the mitral valve make the valve more susceptible to abnormal flow vectors generated in the left ventricle cavity, creating the conditions for systolic anterior motion and mitral regurgitation. Septal myectomy, performed by experienced surgeons in specialized centers, eliminates obstruction at all levels of the left ventricular outflow tract, with a clinical success rate of 90-95% and a mortality rate of less than 1%. Some surgeons complement septal myectomy with intervention on the mitral valve, taking into account anatomical features when isolated septal myectomy may be ineffective. The choice of the most optimal concurrent intervention on the mitral valve during septal myectomy is undefined. Techniques for valve-sparing mitral valve surgery have been developed, including resection of secondary chords, edge-to-edge repair, anterior leaflet plication, and others.
