Tulsan","doi":"10.3329/jdmc.v30i1.56908","DOIUrl":"https://doi.org/10.3329/jdmc.v30i1.56908","url":null,"abstract":"Aims: Aim of the study was to know the clinical presentations of biliary ascariasis.\u0000Materials and Methods: It was a cross-sectional observational study had been carried out on 50 patients over a period from January 2017 to December 2018 at the Department of Medicine, Sadar Hospital, Cox’s bazar. The clinical presentation, ultrasonographic findings, management and follow up of patients diagnosed to have biliary ascariasis were recorded in a pre-designed case record form and finally all the relevant data were processed and analyzed.\u0000Results: This study revealed that maximum patients of biliary ascariasis belonged to 31-40 yrs age group. Number of female patients was more (86%) in comparison to male patients (14%). Among female patients, 20.93% patients were pregnant. Most of the patients 92%came from rural areas. All patients in this study presented with upper abdominal pain. Other complaints were nausea, vomiting, fever and jaundice. Most of the patients (38%) had history of passage of worm with vomitus. Ultrasonographic profile of this study revealed that ascaris was most commonly entrapped in the common bile duct (80%). The percentage of complications was 38%. Acute acalculous cholecystitis was the commonest. The percentage of Cholangitis and acute pancreatitis was also significant.\u0000Conclusion: Bangladesh is an endemic zone for ascaris lumbricoides infestations. Ultrasonography is the investigation of choice for diagnosis and follow up of the patient of biliary ascariasis. Majority of the patient can be managed with conservative treatment and mechanical removal of worms by endoscopy. Akhter","doi":"10.3329/jdmc.v30i1.56897","DOIUrl":"https://doi.org/10.3329/jdmc.v30i1.56897","url":null,"abstract":"Background: Cerebral Palsy (CP), a complex neurodevelopmental disorder in children, is often accompanied by comorbidities like intellectual impairments besides core motor dysfunction.\u0000Aims: To assess the relationship between gross motor and intellectual functions among the children with CP.\u0000Methods: A cross- sectional study was carried out in the Institute of PaediatricNeurodisorder and Autism (IPNA), BSMMU, Dhaka, during Oct. 2017 to May 2018 including 82 children up to 12 years of age having CP. The gross motor function of all the children was evaluated using GMFCS. Psychological assessment was done by professional psychologist using age specific psychometric tools.\u0000Results: One third of the children (39.0%) belonged to age 2-4 years. Male to female ratio was 1.7:1. More than three fourths of the children (85.4%) were of term gestational age. Perinatal asphyxia (PNA) was associated in 92.7% children.More than half of the children (57.3%) had spastic quadriplegia. Almost half of the children (48.8%) GMFCS level was II and 28.0% children were of GMFCS level III. Nearly a half (48.8%) of the children was with mild intellectual impairment and 22.0% were with moderate intellectual impairment. Proper feeding practices are essential for proper nutrition, growth, development and survival of infant and young children.Maternal knowledge and feeding attitude has been shown to be a stronger predictor of breast feeding initiation and complementary feeding practice.\u0000Objectives: To evaluate the knowledge, attitude and practices of mothers regarding infant and young child feeding.\u0000Material and Methods: This cross sectional study was done in Dhaka Medical College Hospital, Dhaka from June 2017 to November 2017.A total number of 218 mothers having children between 0-23 months were included.Modified Iowa Infant Feeding Attitude Scale (IIFAS) and 5 point Likert scale was applied to assess maternal attitude towards infant and young children feeding. The relationship between infant feeding knowledge, attitude and practices and socio-demographic variables was tested by using Chi-square test. Relationship among mothers knowledge, attitude and practices regarding IYCF was showed by Spearman’s correlation.\u0000Results: The study result showed that, 83.5% mothers had good knowledge about IYCF but only half of them had good practice. Almost everyone ( 99.1% ) agreed that breast milk is best for new born baby. Most of the mother (94%) knew that exclusive breast feeding should be given for six months but only half of them (51.1%) practiced EBF.Though 72.5 % mothers had positive attitude towards breast feeding but about two third (63.6%) of them gave formula milk within 6 completed months. About two third of mothers (60.2%) used bottle for feeding their child. Most of the mother (94%) had knowledge about complementary feeding but only 55.7% mother provide complementary feeding accurately with atleast four different types of food. Arslan","doi":"10.3329/jdmc.v30i1.56905","DOIUrl":"https://doi.org/10.3329/jdmc.v30i1.56905","url":null,"abstract":"Back ground: The Visceral Adiposity Index (VAI) is an empirical mathematical model, genderspecific, based on simple anthropometric (BMI and WC) and functional parameters (triglycerides (TG) and HDL cholesterol (HDL)), and indicative of fat distribution and function.\u0000Methods: This is a Cross sectional study. It was carried out in the department of Biochemistry, BSMMU from July 2015- June 2017. Type 2 Diabetes Mellitus patients attending in outpatient Department of Endocrinology and Metabolism, BSMMU were our study population. The cutoff value for VAI was considered as 3.\u0000Results: We enrolled 105 participants in this study. Among them 41 were male and 64 were female. There was significant difference between male and female in terms of height, weight and BMI. Male had greater height (p =0.01) and weight (p =0.01) than female. Female had greater BMI (p =0.01) than male. The majority of male and female scored above the cut off value of VAI which were 63.4% and 59.4% respectively. The mean value of biochemical parameters TG, FBG and HbA1c were significantly different on the basis of cutoff point of VAI (d”3 and > 3). There is increased TG, FBG and HbA1c and decreased HDL-C with the cut off value more than 3 in VAI. Naphthalene mothballs are potent hemolytic agents specially for pediatric group and Glucose 6 phosphate dehydrogenase (G6PD) deficient individuals. Our patient, a 14-year-old boy got admitted in our institution with progressive pallor, jaundice, hematuria and oliguria. He used to chew naphthalene mixed flavored raw rice for the last six months. On investigation he was found to have features of intravascular hemolysis and AKI necessitating hemodialysis with blood transfusion. His G6PD activity was below normal. After seven sessions of Hemodialysis (HD) his renal function recovered and discharged accordingly. It is also known that blood glucose level increases during acute stroke in non diabetic patients.\u0000Objective: Objective of this study was to assess the association between family history of diabetes and newly developed diabetes, impaired fasting glucose, impaired glucose tolerance during acute stroke in non diabetic patients.\u0000Methods: The purposive sampling method was applied to identify the patients by appropriate history taking about diabetes among the patients and their 1st degree relatives, documentary papers and laboratorial support evidenced by HbA1c level less than 6.5% (This means patient to be non diabetic). Data were collected from 100 patients. The data were collected by questionnaire, pre-tested earlier and data collection sheet illustrating the family history of diabetes mellitus and glycemic status during acute stroke among the nondiabetic patients. It was conducted from January, 2021 to July, 2021 in the Department of Medicine of Dhaka Medical College Hospital.\u0000Results: The study found that, mean age of the patents was 61.76±7.33 years. Family history of Diabetes Mellitus (first degree relative) was found among 48% patients.52% patients did not give family history of Diabetes Mellitus. During analysis of the lab investigations among the participants, mean HbA1c level was found 5.69± 0.65 mmol/L which was found normal among the entire patient group. Patients Fasting Blood Sugar (FBS) was tested and mean FBS level was 5.94±0.86.Among the respondents 48% gave family history(1st degree relative) of Diabetes of which 8.3% revealed normal glycemic status , 66.7% showed IGT and 25.0% were diabetic. Haemodialysis (HD) catheter-related bloodstream infections (CRBSIs) are a major complication of long-term catheter use in HD.\u0000Aims: This study was conducted to identify the rates, risk factors and spectrum of infecting organisms of Hemodialysis catheter-related blood stream infection (CRBSI).\u0000Methods: This prospective Observational study was carried out in the Department of Nephrology, Dhaka Medical College Hospital, Dhaka during January 2018 to July 2019. A total of 125 patients with Central Venous Catheter for Hemodialysis were included in this study.\u0000Results: Twenty four patients with a mean age of 42.9±17.5 years were confirmed to have CRBSI with a rate of 6.6 per 1000 catheter days in Temporary Uncuffed Catheter group and 1.4 per 1000 catheter days in Permanent Cuffed Catheter group. Independent risk factor associated with CRBSI was diabetes. 70.8% patients had gram-negative infections, 4(16.7%) patients had gram-positive infections and 3(12.5%) patients had polymicrobial infections. Pseudomonas species (37.5%) were the most common identified bacteria.\u0000Conclusion: The rate of CRBSI in Hemodialysis patients was significant and the only identified risk factor was diabetes mellitus. A renewed interest has been emerging on the human epidermal growth factor-2 (HER2) pathway. Aim of this present study was clinicopathological correlation of HER-2/neu expression by IHC in NSCLC.\u0000Method: This was a Cross sectional and observational study done at Sir Salimullah Medical College and National Institute of Disease of the Chest and Hospital (NIDCH) and other private hospitals in Dhaka city during July / 2014 to June / 2016. Adult of both gender with histologically diagnosed as a case of NSCLC was include in the study. Immunohistochemistry was done to see the positivity for HER/neu and clinical characteristics were observed.\u0000Result: A total 45 patients with NSCLC were enrolled in the study. Male was 77.8% (n=35) and female was 22.2% (n=10). The mean age was 55.67 (SD± 12) years and mean age of male was higher compared to female (57.54±11.64 years verses 49.1±11.38 years). Most of the male were smokers (71.1%) and female were nonsmokers (90%). 57.78% (n=26) of patients and 42.22% (n=19) of patients had adenocarcinoma and squamous cell carcinoma respectively. Most squamous cell carcinoma patients were elderly, had wasting and having higher TNM staging. Serum LDH level was higher with advance staging and grading. Only 8.89% (n=4) had HER-2/neu positive expression affected by male. Patients with HER 2/neu positive expression were relatively older (mean age 62.5±17.08). However, there were equal in histopathological categorization (50% SCC and 50% Adenocarcinoma). Mean LDH was slightly higher in HER-2 positive patients was compared to HER-2 negative patients (569.5±232.6 versus 469.7±181.8).\u0000Conclusion: About nine percent of patients having HER-2/neu positive was relatively older and had more high level of LDH. SLE is a rare autoimmune disease. Concomitant presence of both diseases may worsen COVID-19.\u0000Objective: To characterize patients with Systemic Lupus Erythematosus (SLE) affected by COVID- 19 and to observe medications on COVID-19 outcomes.\u0000Methodology: SLE with confirmed COVID-19 patients on Hydroxychloroquine admitted in COVID dedicated hospital in Dhaka was included. Data were prospectively collected via a structured questionnaire form and review of medical records. Only hospital outcomes were observed.\u0000Results: Total 17 patients were included, 11 of them had only SLE, 5 had lupus nephritis and only 1 had CNS lupus. All of them had confirmed COVID-19 detected by RT-PCR. 17 (100%) were taking immunomodulator (Hydroxychloro-quine), 7 (41.2%) steroids, 5 (29.4%) taking Mycophenolate mofetil before having COVID-19. Eleven (64.7%) of them had mild to moderate severity where 4 (23.5%) had severe & 2 (11.8%) had critical conditions. Of those 17 patients, 11 required supplemental oxygen (64.7%) during hospitalization, 2 (11.8%) admitted into ICU and required mechanical ventilation. Fifteen of them were discharged after 11 days (minimum 9, maximum 21) where 2 (11.8%) died due to hypoxic respiratory failure.
