Methods: Document based categorization of thyroid cancer patients, like- low risk, high risk and clinically extremely vulnerable groups who are at greatest risk of severe illness from coronavirus are to be done accordingly. Individuals with thyroid cancer are mostly not susceptible to COVID- 19. However, patients with thyroid cancer (papillary or medullary) having lung metastases or undergoing certain types of cancer treatment might be at increased risk of viral infection or complications. Risk benefit ratio is checked and explained to the patient and their attendants. \u0000Recommendations: As COVID-19 is spreading across the Bangladesh, hospitals are being forced to reallocate resources to the care of critically ill. So it is crucial to adhere to the advice from experts to reduce the risk of infection. All non-essential surgeries and hospital admissions can be postponed to make sure that hospitals are not unnecessarily occupied. \u0000Conclusion: In the face of the COVID-19 pandemic, cancer care has had to adapt rapidly given recommendations to postpone nonurgent surgeries with active surveillance of thyroid cancer patients except thyroid tumors requiring acute airway management. \u0000J Dhaka Medical College, Vol. 29, No.2, October, 2020, Page 178-181","PeriodicalId":320976,"journal":{"name":"Journal of Dhaka Medical College","volume":"31 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-01-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115920823","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Essential trace elements like iron (Fe), copper (Cu) undergo redox cycling and have physiological significance in inflammatory process. This study is aimed at measuring the level of copper, ceruloplasmin, iron and transferrin in psoriasis patient and to assess its relationship with the severity of the disease. This is an observational cross sectional study. It was conducted at the department of Dermatology and Venereology, BSMMU, Dhaka. Age range of the patient was 18 to 65 years. The mean age of the patients was 39.1±13.54 years, 57.9% patients were male and 42.1% were female. Male: female ratio was 1.4:1. Male patients were predominant. Mean duration of disease 5.36±4.05 years with range from 1.0 to 14 years. Most of the (76.3%) patients had mild disease followed by 23.7% had moderate to severe disease. Serum level of trace elements was compared between mild and moderate to severe group of psoriasis patients but difference were not statistically significant (p>0.05). Weak negative correlation was found between PASI score and serum levels of copper (r = “0.134, P = 0.423), iron (r = –0.080, p = 0.632), transferrin (r = –0.079, p = 0.638) and weak positive correlation was found with ceruloplasmin (r = 0.228, p = 0.168). The results of the present research provide valuable information and correlation between the measured biomarkers and severity of psoriasis. This randomized, clinical trial aimed to compare the effectiveness of Platelet Rich Plasma (PRP) and Hyaluronic Acid (HA) as individual treatments and PRP in combination with HA in the treatment of mild to moderate knee Osteoarthritis (OA). The study was conducted among 89 patients with mild to moderate knee osteoarthritis in the Department of Physical Medicine and Rehabilitation, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. Patients were randomly allocated to one of the three interventions: HA (n=30), PRP (n=30), or HA+PRP (n=29). Patients in HA group received 3 intra-articular knee injections with 1 week interval while patients in PRP group and PRP+HA group received 2 intra-articular knee injections with 2 week interval. Functional outcome of the treatment were evaluated using the Western Ontario and McMaster Universities Arthritis Index (WOMAC) and Visual Analogue Scale (VAS) questionnaire at baseline and after 1,3,6 and 9 months of treatment. Majority of the patients in all groups were female where mean age of the patients were about 50 years. Pain scores in VAS scale significantly reduced in PRP group when compared to HA group and also in PRP+HA when compared to HA group which persist till nine months (p<0.05). No significant statistical difference was found in VAS scores between PRP and PRP+ HA group. WOMAC pain scores also significantly reduced in PRP group when compared to HA group and also in PRP+HA when compared to HA group (p<0.05). WOMAC stiffness significantly reduced in PRP+ HA group when compared to HA group at three month (p<0.001) and at six month (p=0.011). At nine month, physical activity scores significantly reduced in PRP group compared to HA group (p=0.002) and in PRP+HA when compared to HA group (p<0.001). No significant reduction was observed in WOMAC scores when the outcome of PRP+HA was compared with PRP alone. PRP provides better functional outcome than HA. The combination of PRP and HA also provides better outcome than HA alone but does not provide better outcome than PRP alone.","PeriodicalId":320976,"journal":{"name":"Journal of Dhaka Medical College","volume":"91 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-01-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124674479","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Immunofluorescence assay (IFA) is the gold standard serological test for diagnosis of Rickettsial diseases. For serological diagnosis of rickettsial diseases, use of filter paper is an inexpensive and convenient method for collecting, storing, and transporting of blood samples with little space without refrigeration. Current study shows that the rickettsial diseases were present in febrile patients in Bangladesh. The study also compared the results of direct serum by IFA and filter paper serum spot followed by IFA as an alternative way of testing frozen samples. Materials and Methods: This observational study for evaluation of rickettsial diseases from direct serum by IFA and sera dried on blotting paper followed by IFA at the department of Virology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. A total of 152 patients’ sera were selected by systemic random sampling of every 3 sera of 556 febrile patients who were tested negative for dengue and or Chikungunya virus infection. Forty cases were confirmed by IFA from direct sera. In addition, 60 samples were selected from 152 sera by systemic random sampling of every 3 sera for blotting on filter paper for further IFA test. Out of 60 sera, 35 were confirmed by IFA from direct sera and of the 35 confirmed positive cases, 31(88.6%) were positive by IFA after blotting paper assay. Taking the IFA of direct sera as gold standard test, the sensitivity, specificity, positive predictive value and negative predictive value of IFA from blotting paper eluted sera were 88%, 100%, 100% and 86% respectively. It was previously known as Migraine with cerebrospinal pleocytosis; pseudo migraine with lymphocytic pleocytosis. It was first described in early 19801. It is a rare disorder with Migraine-like headache episodes (typically one to twelve) accompanied by neurological deficits including Hemiparaesthesia, hemiparesis and/or dysphasia, but positive visual symptoms only uncommonly, lasting several hours. There is lymphocytic pleocytosis. The disorder resolves spontaneously within 3 months2. As an example of a rare disease. We described a case of 50 years old lady presented with acute onset headache, right hemi sensory tingling and bilateral papilloedema. Her blood test, MRI of brain and MRV was normal. CSF study revealed lymphocytic pleocytosis. Case Description: A 50 year old lady admitted in the inpatient of the Neurology Department of Dhaka Medical College on 15.02.20 with the history of severe throbbing head ache for 2 days associated with tingling sensation on the he right side of her body. The headache was global, throbbing in nature, severe in intensity and was unremitting. It was associated with several episodes of vomiting. There was associated hemiparaesthesia on her right side. Materials & Methods: This study was carried out in the Department of Pediatric Hematology & Oncology, Dhaka Medical College Hospital, Dhaka, over a period 12 months from the day of approval of the protocol. All acute lymphoblastic leukaemia children admitted in Pediatric Hematology & Oncology Department in Dhaka Medical College Hospital and receiving chemotherapy of standard protocol for induction of remission were the study population. A total of outcome variable was hepatotoxicity resulting from chemotherapy given for induction of remission. Result: The mean age of the children was 4.4 years (range 28 years). Males were a bit higher in the series with male to female ratio being 11:9. Liver function tests before therapy revealed that none of the children exhibited raised serum billirubin and only 2(4.5%) children had increased SGPT. However, 50% of the children had raised prothrombin and 43.2% had reduced serum . albumin. Liver function tests after therapy after induction of remission shows that 9(20.5%) children exhibited raised serum billirubin, the proportion of children with raised prothrombin remained almost same as before but the status of serum billirubin improved to some extent. However, proportion of children with raised SGPT was increased to 25%. Comparison of liver function in children after therapy during induction of remission with that before induction did not show any significant difference, except that the serum SGPT was significantly raised during induction of remission (p < 0.001). Conclusion: The study concluded that the current therapy for induction of remission of ALL cases does not produce any toxic effect on liver. Although, enzymes like SGPT take a sharp rise during induction of remission, it is transient and does produce any deleterious effect on liver. The objectives of this cross sectional study are to determine the mean bone mineral density (BMD) and to identify osteopenia and osteoporosis in patients of CKD on maintenance hemodialysis. Methods: Twenty three male and 18 female patients with age between 18 and 50 years were enrolled in this study. The BMD of the lumbar vertebral spine (LV) and the neck of femur (FN) were measured in all patients. Data were analyzed using SPSS version 20.0 software and the level of significance was considered as P <0.05. Results: The mean BMD in the LV (L2-L4) was 1.18 ± 0.19 gm/cm2 in male and 1.04 ± 0.13 gm/cm2 in female patients (P =0.011). The mean BMD in the FN was 0.90 ± 0.19 gm/cm2 in male and 0.77 ± 0.15 g/cm2 in female patients (P = 0.022). Based on the World Health Organization criteria, 26.0% of the male and 22.2% of the female patients in our study had normal BMD; 39.2% male and 38.9%female patients had osteopenia, while 34.8% male and 38.9% female patients had osteoporosis .This study showed a marked decrease in mean BMD in the cortical bone (FN) compared with trabecular bone (LV) (P = 0.001) as well as in female patients on maintenance hemodialysis compared with male patients . Significant negative correlation (r= -0.480; p=0.001) was found between duration of hemodialysis and bone mineral density (BMD) in lumbar spine and femoral neck. Conclusion: The measurement of BMD is a good non-invasive screening test for renal bone disease and that a high number of patients with CKD stage 5 on maintenance hemodialysis have markedly decreased BMD.","PeriodicalId":320976,"journal":{"name":"Journal of Dhaka Medical College","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-01-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125819232","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
