Clinical findings range from mild edema to severe heart failure and pleural effusion. Diagnosis is clinical and other edema causes should be excluded. Since it is a self-limited condition, there is no need to modify insulin regimen. In more severe cases, diuretic therapy may be required.Clinical case: A 14-year-old female was brought to the Emergency Department due to weight gain and leg swelling with three days of evolution. She had been diagnosed with type 1 diabetes at the age of nine. The girl had poor metabolic control which led to multiple hospitalizations, the last one occurring the week before with a glycated hemoglobin higher than 15%. In physical examination, lower limb edema and a 7-kg weight gain were noted. No jugular vein distension, hepatomegaly, or other forms of edema were found. The girl was hemodynamically stable, with diuresis within the normal range. Analytical study consisting of hemogram, ionogram (sodium and potassium), and renal and hepatic function was normal. Urine analysis revealed glycosuria and chest x-ray showed no abnormalities. Prepubertal growth hormone administration results in increased growth and musculoskeletal development. The present case report illustrates the negative impact of growth hormone disruption in a 6-year-old patient, resulting in impaired growth and development. The girl presented with ecchymotic lesions on the anterior region of the thorax and antero-proximal region of the upper limbs, which had apparently worsened in the last 24 hours. She denied trauma or impact sports, fever or recent infections, and blood loss or other symptoms. Laboratory evaluation showed no alterations and urine toxicology screening was negative. When confronted with normal laboratory evaluation, the girl confessed intentional self-inflicted injuries following a discussion with her mother. Parents were made aware of the situation, and a pediatric psychiatry consultation was recommended. When used for short periods of time, it is considered a healthy habit. Acute otitis media is one of the most frequent infections in pediatric age and has been associated with pacifier misuse.Objectives: To review available evidence regarding the association between pacifier use in the first years of life and otitis media risk. Methods: A literature search was conducted on several databases using MeSH terms “otitis” and “pacifiers” for guidelines, systematic reviews, randomized controlled trials, and observational studies over the last 20 years. SORT scale of the American Family Physician was used to evaluate evidence levels.Results: A total of 56 articles were retrieved, of which four guidelines, one systematic review, and one original article were selected. According to guidelines, there is no reason to discourage pacifier use, as it can be particularly beneficial in the first six months of life. Afterwards, pacifier use should be limited to moments of falling asleep. Its use should also be avoided in infants with chronic or recurrent otitis media. The systematic review stablished pacifier use as a risk factor for recurrent acute otitis media (RAOM) which is susceptible to intervention. In the observational study, a statistically significant association was found between pacifier use and RAOM risk.Conclusions: Pacifier use should not be actively discouraged in the first semester of life, as it can have beneficial effects for children (SORT A). O objetivo deste estudo foi caracterizar a populacao referenciada a uma consulta pediatrica do sono de um hospital terciario por insonia comportamental. Metodos: Estudo retrospetivo e descritivo baseado na revisao de processos clinicos de doentes seguidos por insonia na referida consulta especializada durante um periodo de oito anos (2008−2016). Os resultados sao apresentados como mediana (minimo–maximo), media (desvio padrao) e percentagem. Resultados: No total, 964 criancas foram referenciadas a consulta de sono, 162 (16.8%) das quais por insonia e 137 (14.2%) por insonia comportamental. A maioria (58.4%) era do sexo masculino e a mediana de idades aquando da referenciacao foi de 45 meses (5 meses–18 anos). A mediana da hora de deitar foi 22:00 (20:00–4:00), tendo sido frequentes os despertares noturnos, sobretudo em idade pre-escolar. Relativamente as rotinas na hora de deitar, 62% das criancas nao adormeciam sozinhas, 48.9% tinham televisao no quarto e 43.1% (11.9% das quais, adolescentes) precisavam de um objeto e 42.3% de luz acesa para adormecer. A maioria (62%) das criancas tinha uma rotina para adormecer, dependente do cuidador em criancas mais novas e de televisao e leitura nos adolescentes. Os sintomas diarios mais frequentemente referidos foram sonolencia em criancas mais velhas e irritabilidade em criancas mais novas. Relativamente ao tratamento, 29.9% encontrava-se a receber tratamento farmacologico antes da referenciacao. Conclusao: A maioria das criancas neste estudo encontrava-se em idade pre-escolar, a qual constitui uma importante janela de atuacao. A hora de deitar tardia, presenca de televisao no quarto e dependencia dos pais para adormecer foram frequentes, o que revela a necessidade de intervencao na area.
