Semiological accuracy, with a careful anamnesis plus a systematic and detailed physical examination, is mandatory. Only the use of an accurate and standardized language will allow correct communication so as not to incur ambiguities of definition and interpretation and consequent omissions or overvaluations of clinical manifestations, culminating in diagnostic errors. This manuscript intends to gather and define the multiple concepts of musculoskeletal pain with the aim of standardizing and clarifying medical terminology. However, most studies used as reference for fever assessment had a cross-sectional design and were conducted in adults. Different and more precise fever definitions exist within the field of knowledge known as clinical thermometry. Aims: To assess basic knowledge of health professionals working in Pediatrics regarding fever physiopathology and clinical thermometry. Material and Methods: A cross-sectional analytical study was performed between February and July 2014 through application of an anonymous closed-end questionnaire to health professionals. Results: From 426 questionnaires applied, 29% were completed by nurses and 71% by physicians. Within the whole group, 89% did not know how human “normal temperature” was determined, 70% did not recognize the “individual definitions” of fever, 33% acknowledged a “subfebrile” status, 39% did not recognize the most and least accurate anatomical sites for temperature measurement, and 57% did not recognize the dynamic difference between core and peripheral temperatures. Hyperthermia and fever definitions were confounded by 78% of nurses and 56% of physicians. Conclusions: Most health professionals surveyed had a limited knowledge of fever and clinical thermometry. The patient typically refers a diffuse pain along the anteromedial side of the tibia, which is worse in late afternoon and associated with over-exercising days. Diagnosis is confirmed by a prototypical history and physical examination findings. Radiological evaluation assists in differential diagnosis. Treatment consists of adequately resting or changing training routines, together with analgesic drugs. Herein is presented the case of an adolescent referred to our Pediatric Rheumatology Unit with diffuse pain in the right pretibial region due to intensive exercise. Laboratory study and imaging exams were unremarkable. Since 2002 it has allowed active euthanasia in children aged 12 and over. The Groningen Protocol, established in 2005, introduced the possibility of ending the life of newborns who fulfill certain specific criteria. It was drafted by Verhagen and Sauer at the University Medical Centre in Groningen and was granted authorization for national implementation from the Dutch Association of Pediatric Care. Methods: A literature search was conducted to analyze the Groningen Protocol and arguments supporting and opposing it. Results: Seemingly competing tenets of principalism – respect for autonomy, beneficence, nonmaleficence, and justice – are invoked as core arguments both for and against the protocol. The scale hangs in the sense of opposition to the protocol, essentially because of the weight of some of the arguments presented. Conclusion: From our perspective, the Groningen Protocol seems to have been built primarily to allow deliberately ending the life of a newborn baby without fear of criminal prosecution. In addition, included criteria are prone to subjectivity and may lead to abuse. Reducing time to diagnosis and engaging health professionals in early treatment are key when dealing with pediatric cancer. In this study, time to diagnosis and its determinants were assessed. Methods: A descriptive and observational study in the Pediatric Department of a Level II hospital was conducted using clinical records of children diagnosed with cancer between 2007 and 2016. Using Mann-Whitney’s test, Kruskal-Wallis’ test, and Spearman’s correlation test, differences in time to diagnosis across subgroups of children according to age, gender, parental age, type of first medical visit, and diagnosis were assessed. Results: One hundred and five pediatric tumor cases were included in the analysis: 48 (46%) brain and central nervous system tumors, 32 (30%) hematological tumors, and 25 (24%) solid tumors. In the hematological subgroup, older age was associated with longer time to medical services demand (r=0.38, p=0.04) and children initially observed in primary health care exhibited longer time to diagnosis compared with children initially observed in Pediatric emergency services (median 1.9 vs 0 weeks, p=0.01). The median number of medical visits before the definitive diagnosis was one (min=0, max=7). Conclusions: Differences found in time to diagnosis between different types of medical services disclose the need for improving diagnosis in the primary care setting. In toddlers, the most likely causes of gait alterations can be divided into five groups: inflammatory/infectious; neurologic; developmental/congenital; neoplastic; and post-traumatic. Case report: Here in is reported the case of a three-year-old boy with claudication diagnosed with bilateral proximal femoral transphyseal fractures by plain radiograph and magnetic resonance imaging. Conclusion: When a non-ambulatory child presents with femur fractures, clinical history should address specific questions to determine whether the explanation put forth can reasonably explain the injury. The estimated prevalence of PFS in patients with pollen allergy is 50–70%. Typically, PFS-induced symptoms are restricted to the oral cavity (oral allergy syndrome – OAS). The authors present the case of a female adolescent with grass pollen allergic rhinitis treated with oral immunotherapy for four years and four months, who developed OAS to fresh fruits and walnuts. Diagnostic workup showed sensitization to several allergens, particularly to profilin, which seems to be responsible for PFS.
