It is clinically characterized by fever, skin lesions, arthralgia/arthritis, lymphadenopathy, and nephritis and by presence of immune complexes. Serum sickness-like reaction mimics serum sickness and its pathophysiology is not well understood. It is more common in pediatric age and often associated with drug administration. This study aimed to characterize serum sickness-like reaction cases evaluated at a Drug Allergy Clinic. Material and methods: An analytical, retrospective, longitudinal study of a sample of 39 children (56% male) with diagnosis of serum sickness-like reaction evaluated at a Drug Allergy Clinic between January 2007 and December 2017 was performed. Results: Children had an average of 4.8 years at the time of reaction. In most cases (97%), beta-lactam antibiotics were the suspected triggering drugs. On average, clinical manifestations developed 7.5 days after treatment start. Skin lesions developed in all patients, and arthralgia/arthritis in 92.3%. Fifteen percent of children were hospitalized. Penicillin-specific IgE was evaluated in 87% of patients, but only tested positive in two. Skin prick tests and intradermal tests were performed in 46% of cases and were positive in seven. Oral challenge test was performed in 26% of patients and was positive in three. Conclusion: Serum sickness-like reaction is a rare and poorly understood entity. Diagnosis is essentially clinical, requiring a high index of suspicion. Some of these tumors secrete catecholamines, being a rare cause of hypertension. Clinical Case: A female adolescent, with 14 years old and unremarkable medical history, was referred to the Pediatric consultation due to episodes of holocranial headache associated with blurred vision with six months of evolution. Physical examination and brain magnetic resonance imaging revealed no abnormalities. Physical examination showed stage 1 hypertension, later confirmed by ambulatory blood pressure measurement. Analytical study, chest X-ray, electrocardiogram, and echocardiogram were normal. Renal echography revealed a heterogeneous mass contiguous to the left renal sinus and abdominal computed tomography showed a retroperitoneal lesion in the medial side of the left perirenal space. Due to retroperitoneal paraganglioma suspicion, the patient was referred to Porto’s Portuguese Institute of Oncology, where surgical excision of the lesion was performed. Anatomopathological examination confirmed diagnosis of sympathetic paraganglioma. Conclusion: Although rare in pediatric age, paragangliomas represent the most common endocrine tumor. Biochemical and imaging evaluation are key for diagnosis, after which a genetic study is recommended due to its relevance for patient management and follow-up. The clinical case of an adolescent with AN and ASD is presented, and its specific treatment challenges discussed. Case Report: Female, 16 years old, with AN and a diagnosis of ASD since the age of two. Cognitive-behavioral therapy, motivational techniques, and family involvement were used, with some adaptations. Discussion/Conclusions: Challenges were found which hampered treatment, such as rigid thinking patterns and routines, including very rigid rules around food. Migraine diagnosis in childhood rests on criteria similar to those used in adults but with some particularities, as duration of the attack, which is often much shorter than in adults, and location of the attack, which in many children may be bilateral. Despite its high prevalence, pediatric migraine remains undertreated, sometimes due to fear of caregivers and physicians and lack of studies about its treatment. Although treatment options for pediatric migraine are increasing, they remain limited. In this article, the authors review approved and “off-label” drugs currently used in migraine and status migrainosus acute treatment in pediatric patients. In migraine treatment, nonsteroidal anti-inflammatory drugs (NSAIDs) should be used. In moderate-to-severe migraine unresponsive to analgesics or NSAIDs, triptans may be used, alone or in combination with the former. Rescue medication, including dihydroergotamine and sodium valproate, can be used in hospital setting for intractable migraine. Severe hypokalemia must be immediately managed, as it can have important cardiac repercussions. Clinical case: A previously healthy eight-year-old female, with normal growth and normal-to-low blood pressure, was observed due to persistent abdominal pain, anorexia, and fever two days after overcoming a mild gastroenteritis episode. Serum biochemistry revealed severe hypokalemia (1.8 mmol/L), hypomagnesemia, and metabolic alkalosis. The patient was admitted to the Pediatric Intensive Care Unit for correction of electrolyte imbalance, cardiac monitoring, and investigation. Complementary studies included a spot urine ionogram that revealed inappropriate potassium wasting. Ionic correction was achieved by both intravenous and oral supplementation plus spironolactone. Genetic testing was positive for Gitelman syndrome.
