{"title":"Analysis of the implementation of the Hospital Component in the Emergency Care Network","authors":"Marilei Elisangela Radel, Helena Eri Shimizu","doi":"10.1590/0103-1104202313602i","DOIUrl":"https://doi.org/10.1590/0103-1104202313602i","url":null,"abstract":"ABSTRACT This study aims to describe the implementation of the Hospital Component of the Urgent and Emergency Care Network (RUE), and to evaluate the delivery of priority Lines of Care in places that serve as gateways in Brazilian regions, between 2011 and 2019. This is a descriptive and analytical study, using data from the National Registration System of Health Institutions (CNES) of the Ministry of Health. To assess significant differences before and after the implementation of Priority Lines of Care, the Wilcoxon test was used. It was found that the Southeast region had the largest increase, including the implementation of technologically denser care points, followed by the Northeast, South, North, and Midwest regions. The South region stood out for the implementation of Priority Lines of Care. Not only did it implement the most of these lines, but it also increased the number of visits and decreased the average length of stay. The Trauma Line of Care was found to be the most effective, when verifying the number of visits with population growth. A despeito de décadas em investigações científicas sobre suas possíveis causas, apenas modestos avanços foram alcançados. Atualmente, as pesquisas indicam que a esquizofrenia é uma condição complexa e que sua etiologia é multifatorial. No entanto, o modelo biomédico em saúde mental, caracterizado pela ideia de que transtornos mentais são doenças do cérebro, frequentemente procura delimitar a esquizofrenia aos seus aspectos biológicos, menosprezando a influência do ambiente. A concepção de transtornos mentais como doenças cerebrais tem repercutido na população em geral, que, em parte, é informada sobre o tema a partir da divulgação científica pela mídia tradicional. O objetivo do artigo é analisar como as causas da esquizofrenia têm sido divulgadas pela mídia impressa. Para tanto, a partir de uma Análise de Discurso Crítica, foi conduzida uma busca no acervo digital dos três maiores jornais brasileiros, de maneira a evidenciar quem é convocado a falar sobre a esquizofrenia e quais as principais explicações causais divulgadas ao público. Essa questão raramente é examinada na literatura brasileira sobre diplomacia da saúde. A partir de revisão de literatura, análise documental e entrevistas com atores-chave, examinamos as políticas impulsionadas por complexos processos históricos de mudança no Brasil. Há importantes inter-relações entre política externa e política social, incluindo saúde. Durante os governos Lula (2003-2010), a internacionalização das políticas domésticas brasileiras, vinculadas à cooperação Sul-Sul, teve papel central. A saúde na agenda da política externa foi um importante suporte à crescente presença internacional do Brasil. Esses desenvolvimentos foram possibilitados pelo ativismo e comprometimento de diversos atores estatais e não estatais, que atuaram em dois níveis: advocacia nacional e transnacional e atividades coordenadas entre representantes do governo, incluindo diplomatas, e atores da sociedade civil. O principal argumento deste estudo é que as políticas nacionais e internacionais são interrelacionadas nesse processo, e a dinâmica doméstica e o engajamento societal são essenciais, mas não suficientes: escolhas governamentais são também determinantes. Method: qualitative study, with 6 focus groups and 37 in-depth interviews with 58 participants, from the cities of Belo Horizonte, Salvador and São Paulo, whose thematic analysis was based on the Theoretical Framework of Acceptability. Results: the HIV self-test had good acceptability, although it was not homogeneous among participating adolescents. Positive aspects include, for example, agility, privacy, autonomy, monitoring one’s health, and emotional and stigma management. In another direction are concerns about how to deal with an eventual reactive result and whether self-testing is a prevention strategy. The place where self-tests are given out is decisive to improving their use, which depends on cultural competence to accommodate sexual and gender diversities of adolescents. Conclusion: This study has shown that HIV self-testing is a fundamental strategy to increase adolescent autonomy and self-care. This article analyzed federal regulation and its implications for establishing primary care teams in Brazil. Exploratory mixed methods research involves analyzing 25 federal ordinances and secondary data of national scope referring to the teams approved in the National Register of Health Establishments from 2017 to 2021. The results indicate changes in the direction of the policy regarding the configuration, funding, and accreditation of teams. There was an expansion of primary care teams, a reduction in community health agents, and a weakening of the Family Health Support Center. An integrative review of the literature on the working and health conditions of rural women workers, published from 2010 to 2020, was carried out. The databases consulted were BVS, SciELO and PubMed, and 43 studies, 11 national and 32 internationals, were analyzed. The results point to a diversity of work activities performed by rural women, in a salaried, autonomous and cooperative way, as well as unpaid activities. Working conditions in salaried jobs are generally precarious, with temporary contracts, low wages and long working hours. In autonomous work and in cooperatives, studies report greater control of activities, financial autonomy and greater satisfaction of workers. Stood out in all forms of work, the sexual division of labor and the work overload with the addition of productive activities, unpaid reproductive activities, housework and care work. To assist in understanding reality, an Evaluability Study was carried out collaboratively, involving discussion with stakeholders, questionnaires and consultation of documents. The policy components were identified, organized and modeled. A simplified-logical-model and one of the operational-logical-models, validated by the participants, are presented in this article. For policy evaluation, the initial aspects of the modeled logical implementation chain were prioritized. It was identified as fundamental the promotion of the actors’ knowledge about the set of resources to the implementation of the Integrative Practices services and about the related intersectoral responsibilities. Formative, qualitative and collaborative assessments are suggested. The analysis focuses on the years following 2003, when a seeking to include this policy within the scope of the National Health System took office at the Ministry of Health and met with resistance from medical societies. The study adopted sources were process tracing methodology, documents, and interviews. It is based on a neo-institutionalist theoretical approach. Findings reveal long-standing stability (1960-2002) in the policy and its institutional arrangement under the policy community formed by Liberal Medicine advocates (LM-PC); a period (2003-2009) when a thwarted attempt at change by the Ministry of Health met with the opposition of the LM-PC; a period (2010-2016) when such stability was undermined, and the policy was changed despite the opposition of the LM-PC; and, finally, a period when stability was recovered favoring the policy mentioned above community again, reversing several previous changes. However, the LM-PC could not implement its propositions since it faced opposition from other actors influencing the policy.
