The treatment algorithms for anal fissure proposed in the literature agree on the initial conservative approach. In the case of conservative treatment failure, however, other therapies are introduced, including botulinum toxin, followed by invasive and surgical techniques. This paper presents the possibilities of different combined approaches, including botulinum toxin injection into the anal canal combined with conservative treatment, a dilator or pneumatic dilation, and surgery. Therapies combining pneumatic dilatation with conservative treatment are also presented. The publication is based on both our own practical observations and current reports. The first symptoms tend to be non-specific, which can cause diagnostic difficulties. Full-thickness inflammation of the bowel wall, which may involve any part of the gastrointestinal tract, is a landmark of this disorder. Terminal ileum is the most commonly affected area, as also described in the presented case. Surgical treatment should be as least invasive as possible due to the high probability of another surgical intervention. We present a case of a patient previously repeatedly treated for anal abscess in an outpatient setting in the USA. The patient was admitted to the surgical ward due to perineal phlegmon in the course of recurrent anal abscess. He was qualified for abscess incision. During hospital stay, the patient developed symptoms of gastrointestinal bleeding. Diagnostic endoscopy of the large bowel revealed macroscopic lesions typical of Crohn’s disease. Repeated attempts to transfer the patient to a unit specialised in the treatment of inflammatory bowel diseases (IBDs) have been unsuccessful. Due to the deteriorating general condition and progressive anaemisation, the patient was qualified for surgery. The end section of the ileum was resected and an end ileostomy was created. Initially the surgeons and the anatomists accumulated isolated personal experience and then they came to the realization of the possibility of exertion of feces bypassing the rectum. Creating an artificial anus as a treatment of various coloproctological diseases has been formed without ether anesthesia nor antiseptics. Despite this, many patients managed to prolonged or save their lives. French anatomist Alexis Littrè was the first who put forward two plans for dealing with rectal atresia. The first was to open the closed ends and stich them together; the second was to create an artificial anus. Another French surgeon Jean Mèry, probably for the first in the world formed a stoma in 1701. Jean Pillore performed the first elective colostomy (cecostoma) in 1776 on the patien with rectal cancer. Fistulas with high purulent reservoirs in diagnostic imaging are particularly challenging. Surgical opening of high branches and purulent reservoirs is a key therapeutic element in the management of complex fistulas. Access to these reservoirs should be established via the simplest route, hence for example, the deep postanal space should be reached from the outside by separating the external sphincter fibers, while the shortest access to high intersphincteric reservoirs is through the anal canal. The paper discusses two surgical approaches: a historical method proposed many years ago by Hanley and involving a wide opening of the deep postanal space, and TROPIS, which is a new, recently published method for accessing high intersphincteric fistulas. We describe the techniques, indications and contraindications for these two approaches. We concluded that proper patient qualification for the above therapeutic approaches is crucial. Statistics still indicate that even up to 40% of surgeries are done unnecessarily. The importance of this issue is reflected in the fact that new scales and expert statements are continuously being established in various surgical organizations in the world. The authors of this paper report a post-appendectomy complication in the form of fecal fistula. In the case description, it was attempted to present the order of the diagnostic and therapeutic process. Fecal fistula is a very rare and severe post-appendectomy complication, linked with high mortality. Medical imaging in the form of fistulography and CT allows rapid identification of this complication. It is reported in approximately 1% of inguinal hernias. In this hernia, the vermiform appendix is located within the hernial sac. The authors present a case of a 43-year-old man who reported to hospital due to incarcerated right inguinal hernia. The hernia was returned to the peritoneal cavity and subsequently repaired in a laparoscopic procedure. The inflamed vermiform appendix was found trapped in the hernial sac intraoperatively. The clinical state was assessed as type 2 based on the Losanoff and Basson classification. The inguinal hernia was repaired using the TAPP approach. Subsequently, laparoscopic appendectomy was performed. There were no complications in the postoperative period. Diet was introduced on day 1 after surgery. The patient was discharged from the surgical department on day 4 after surgery. The follow-up at 5 months showed no recurrent hernia. The authors emphasize the role of laparoscopic techniques as an alternative to classical surgery also for emergency conditions. Ciesielska","doi":"10.25121/nm.2021.28.3.110","DOIUrl":"https://doi.org/10.25121/nm.2021.28.3.110","url":null,"abstract":"Eduard Pernkopf (1888-1955) was an Austrian professor of anatomy who later served as a dean of the medical school and rector of the University of Vienna. Pernkopf himself joined the Nazi Party’s foreign organization in 1933. He is best known for his seven-volume anatomical atlas Topographical Anatomy of Man (“Topographische Anatomie des Menschen, Lehrbuch und Atlas der regionar-stratigraphischen Praparation”) often colloquially known as the Pernkopf atlas or just “Pernkopf”. It is considered a scientific and artistic masterpiece. It has been in recent years found that Pernkopf and the artists working for him: Erich Lepier, Ludwig Schrott, Karl Endtresser and Franz Batke used executed political prisoners as their subjects. Using a special treatment of the paper used for watercolor they created the images that look like living tissue in print. But they also used the Nazi symbols in their work for the atlas. In 1995 Pernkopf and his atlas came into the focus of a controversy in scientific ethics following the publication of a paper by Professor Edzard Ernst revealed that the subject bodies may have in some cases been those of executed prisoners and children killed in a Viennese hospital. A year later Dr. Howard Israel discovered many of the Nazi symbols in the artists’ signatures. Since then physicians have discussed whether it is ethical to use the atlas. This resulted in the establishment of the Senatorial Project of the University of Vienna in 1997. Imaging scans demonstrated the presence of numerous pus collections at various sites, but did not reveal the cause of phlegmon and fistulas. The patient was deemed eligible for an urgent surgery. The crypt-related etiology of fistulas was ruled out intraoperatively. The numerous pus collections and cutaneous fistulas were incised, cleansed and drained. Subsequently, treatment was continued using negative pressure wound therapy. The treatment led to wound healing. The retrospective analysis demonstrated that the most probable cause of phlegmon was spontaneous rectus sheath hematoma that had an atypical manifestation due to pathological obesity. The most common causes of gas and faecal incontinence include obstetric injuries (mechanical trauma to the sphincter muscles or stretching of the pudendal nerve during vaginal delivery) and complications after anorectal procedures. The incidence of faecal incontinence is estimated by various authors between 2.2 and 25%. The problem affects all age groups, but mainly the elderly, with higher rates among women than men. The paper presents a review of diagnostic and therapeutic methods for incontinence based on the latest reports. Most researchers agree that, except for extensive sphincter injuries, conservative methods are the mainstay treatment for faecal incontinence, while instrumental or surgical procedures are used in the event of their failure. Despite the multitude of therapeutic methods for this difficult disease, there is no ideal procedure as the aetiology of incontinence is usually multifactorial, hence the therapeutic management should be incorporated on several levels. The grade of the disease is an important aspect in qualification for treatment. Banov grading system, which distinguishes 4 grades of disease based on the degree of haemorrhoid prolapse, is the most commonly used classification system. While the majority of researchers are in agreement as to the use of conservative treatment in the initial stage of the disease, the treatment of grade II and III patients raises much controversy, and the developed therapeutic standards differ from country to country. We present the therapeutic standards applied in the United States, some European countries and in Japan. The initial conservative treatment of the disease is common for the presented algorithms of therapeutic management. Interestingly, infrared photocoagulation, which is very rarely performed in Poland, has its permanent place in the treatment of moderately advanced hemorrhoidal disease in most European algorithms. Some of expert opinions seem extreme, e.g. adding sphincterotomy to haemorrhoidectomy to reduce postoperative pain.
