It is reported in approximately 1% of inguinal hernias. In this hernia, the vermiform appendix is located within the hernial sac. The authors present a case of a 43-year-old man who reported to hospital due to incarcerated right inguinal hernia. The hernia was returned to the peritoneal cavity and subsequently repaired in a laparoscopic procedure. The inflamed vermiform appendix was found trapped in the hernial sac intraoperatively. The clinical state was assessed as type 2 based on the Losanoff and Basson classification. The inguinal hernia was repaired using the TAPP approach. Subsequently, laparoscopic appendectomy was performed. There were no complications in the postoperative period. Diet was introduced on day 1 after surgery. The patient was discharged from the surgical department on day 4 after surgery. The follow-up at 5 months showed no recurrent hernia. The authors emphasize the role of laparoscopic techniques as an alternative to classical surgery also for emergency conditions.
