Chronic shoulder pain is a common reason for outdoor parients in rheumatology. Magnetic Resonance Imaging (MRI), which is its main diagnosis tool, is not accessible for the majority of Congolese patients. The objective was to describe diseases that lead to non-traumatic shoulder pain (NTSP) using the radiography-ultrasound pair and to assess the performance of X-ray compared to that of ultrasound. Methods. This is a cases series of 222 patients for whom an X-ray and an ultrasound were carried out for an NTSP at the Gombe imaging center in Kinshasa and at the Intercommunal Hospital of Fécamp in France, during the period from January 2015 to October 2016. The variables of interest were demographic, clinical, radiographic and ultrasound data. Results. Two hundred and twenty-two patients were included in the study, including 113 women (50.9%) and 109 men (49.1 %). The mean age of the patients was 55.7 ± 14.3 years. A normal shoulder was observed in 122 patients (55 %) on the radiography versus 48 (21.6 %) on the ultrasound. Tendinopathies, calcifications and acromio-clavicular arthropathy were the most common forms of NTSP. The X-ray was more informative than ultrasound for the diagnosis of capsulitis and subacromial conflict. In all other cases it was less effective than ultrasound. The sensitivity and negative predictive value of X-ray were lower than that of ultrasounds. Conclusion. The coupling of X-ray with ultrasound helps to diagnose a greater number of entities than if we considered each examination in isolation. Ultrasound presented a better diagnosticu performance than standard radiography. Major handicap for operational conditioning of troops, hypertension requires innovative approaches for its prevention and management. The present study aimed to evaluate the impact of adapted physical activity (APA) on BP level of sedentary soldiers from Kinshasa garrison and the rate of hypertension control in those with high BP. Methods. Open, parallel randomized controlled trial carried out at Camp Lt-Colonel Kokolo (CVEC) from June 2016 to October 2017) in sedentary soldiers (57.6 %, hypertensives) allocated for 8 weeks to APA (n=119) or control (n=110). The randomization procedure used permuted blocks of four consecutive participants. The outcomes were baseline-adjusted betweengroup difference in BP level (all participants), in rate of BP control among hypertensives. Results. At the last available visit in 226 participants (119 vs 107), the baseline-adjusted BP difference between active and control group by intentionto- treat was 5.1 (95 % CI 1.2 -10.8)/3.0 (0.1-6.9) mmHg lower in the active group. The effect of APA was also significant across pre-specified categories of participants based on age, officers’ rank, and hypertension status. Among 129 analyzed drug treated hypertensives (68 vs 61), the rate of BP control remained unchanged in the control group (43.8 to 44.3%) but increased (43.5% to 85.3 %) in the active group yielding a baseline-adjusted between group difference of 40.7 (32.2; 49.2) %. The probability to achieve hypertension control was greater (HR: 3.38 [95% CI: 1.48- 4.84] in the active group. PP analysis of 122 soldiers (80 vs 42) with data at all scheduled visits yielded confirmatory results for BP reduction and for hypertension control by APA. The changes in BP were positively correlated with concomitant reductions in heart rate. Conclusion. Nous rapportons une observation clinique d’une méningite purulente dans le contexte d’un lupus érythémateux systémique, chez une jeune patiente de 29 ans, faisant suspecter à priori un neurolupus. L’examen clinique a retrouvé des lésions cutanées lupiques, un syndrome méningé dans un contexte fébrile. Le bilan biologique a révélé la présence des anticorps antinucléaires, une hyperneutrophilie, un liquide céphalorachidien d’aspect trouble caractérisé par une hypoglycorachie, une hyperprotéinorachie et la présence des méningococques. Le diagnostic d’une méningite bactérienne sur un terrain de lupus érythémateux a été retenu. Sous l’antibiothérapie prescrite, l’évolution du syndrome méningé était favorable. Cette observation clinique en discussion, démontre que tout syndrome neuro-méningé chez un patient lupique n’est pas forcément un neurolupus. \u0000 \u0000English title: Purulent meningitis occurring during lupus simulating neurolupus \u0000Neurolupus is a frequent and severe complication of lupus disease with heterogeneous clinical manifestations and difficult diagnosis. We report a clinical observation of purulent meningitis in the context of systemic lupus erythematosus in a 29- year-old female patient, raising a priori suspicion of neurolupus. The clinical examination revealed skin lesions of lupus; a meningeal syndrome in a febrile context. The biological assessment found the presence of antinuclear antibodies, a cerebrospinal fluid with a turbid appearance characterised by hypoglycorrhagia, hyperproteinorrhagia and the presence of meningococci. The diagnosis of bacterial meningitis in lupus erythematosus was retained. The patient was treated with antibiotics and the meningeal syndrome evolved favourably. Although neurolupus is a major complication of lupus disease, it should be recognised that not every neuro-meningeal syndrome in a lupus patient is neurolupus. L'adoption de techniques moléculaires pour la détection de P. falciparum a révélé, la présence de nombreuses infections de faible densité indétectables par des techniques de routine et pourtant transmissibles. Dans cette revue, nous avons analysé, la situation actuelle des infections submicroscopiques et discuter ; de leurs intérêts, dans le développement des stratégies de lutte, visant à éliminer le paludisme. Souvent rapportée comme asymptomatique, l’infection submicroscopique à P. falciparum peut de manière imprévisible, donner des accès palustres simple. La proportion de ces infections est généralement plus élevée dans les milieux à faible transmission. La rupture temporaire de la transmission du paludisme pour des raisons thérapeutiques, prophylactique ou climatique est souvent, en lien avec l’augmentation du taux des infections submicroscopiques. Des données montrent une persistance des infections submicroscopiques dans des pays en phase de pré-élimination du paludisme et tout au long de l’année dans des zones à transmission pérenne. However, these tests do not detect all cases of malaria infection. The adoption of molecular techniques to detect P. falciparum infection has revealed many low-density infections that are not detected by routine techniques but are yet transmissible. In the present paper, the current status of submicroscopic infections was analyzed nd their relevance to the development of control strategies to eliminate malaria discussed. Often reported as asymptomatic, P. falciparum submicroscopic infection can specifically trigger ncomplicated malaria in both adults and childhood. The proportion of this infection is typically highest in low transmission settings. The occurrence of submicroscopic infection is influenced mainly by climate notably the dry season and the intermittent preventive treatment. There is evidence that submicroscopic infections persist in countries in the pre-elimination phase of malaria and throughout the year in areas with perennial malaria transmission. L’objectif de la présente étude était de décrire les aspects cliniques et évolutifs de l’ARA. \u0000Méthodes. Il s’agissait d’une étude de suivi longitudinal analytique avec recueil prospectif des données, entre les 1er janvier et 30 septembre 2019,incluant tous les patients admis au CHUD-Borgou pour une altération de la fonction rénale et ayant donné leur consentement. L’ARA était retenue et classée selon les critères de KDIGO. L’issue était favorable devant une normalisation des valeurs de la créatininémie avant un délai de trois mois. L’évolution vers une insuffisance chronique était considérée comme défavorable. \u0000Résultats: Au total, 578 patients sur les 7975 admissions avaient une ARA soit une fréquence de 7,2 %. Les stades 1, 2 et 3 étaient rencontrésrespectivement, dans 53,8 %, 30,9 % et 15,2 %. L’âge moyen était de 46,9 ± 19,9 ans [extrêmes 5 ans et 93 ans]. Sur les 221 patients ayant été suivis, l’évolution était favorable (33,9 %) et défavorable dans 49,3 % des cas avec 17 % de décès. Le jeune âge (p=0,0253) et un bon état d’hydratationextracellulaire (p=0,0023) étaient associés à une évolution favorable de la fonction rénale, tandis que la présence de l’hypertension artérielle était péjorative (p=0,0147). \u0000Conclusion: L’ARA est fréquente. Son évolution est émaillée par le decès ou le passage à la chronicité, surtout chez les patients hypertendus. The objectiveof the present study was to describe the clinicaland evolutionary aspects of AKI. \u0000Methods: This was a longitudinal analytical follow-up study with prospective collection of data, between January 1st and September 30th, 2019, including all patients admitted to CHUD-Borgou for impaired renal function and having given their consent. Ce mécanisme, cependant, par défait de synthèse de l’hormone adrénocorticotrope secondaire à l’aplasie hypothalamo-hypophysaire fait défaut chez le foetus anencéphale. En effet, l’anencéphalie s’accompagne presque toujours du dépassement de terme de la grossesse en absence de l’hydramnios. Par contre, associée à ce dernier, elle conduit à un accouchement prématuré. Nous rapportons un cas de déclenchement spontané du travail d’accouchement sur une grossesse de trente-huit semaines et un jour. La gestante avait une circulation veineuse collatérale au niveau de la paroi abdominale, la hauteur utérine était de 41 centimètres, l’échographie réalisée avait objectivé un foetus anencéphale associé à un polyhydramnios. Le nouveau-né était décédé sept minutes après l’accouchement. Ainsi, bien que très rare, l’anencéphalie associée au polyhydramnios peut atteindre le terme de la grossesse. L’échographie du deuxième trimestre devrait être réalisée chez toute les gestantes pour dépister cette malformation et arrêter l’évolution de la grossesse, car cette pathologie est incompatible avec la vie extra-utérine. \u0000 \u0000English title: Spontaneous induction of labor in a full-term pregnancy of an anencephalic fetus: an unusual case report \u0000The fetal stress occurring at the end of pregnancy is linked to the peak of corticotropin-releasing hormone which is considered as the marker of placental clock. This stress is the spontaneous trigger of labor by stimulation of the corticotropic axis. In the anencephalic fetus, this mechanism is lacking due to the absence of synthesis of adrenocorticotropic hormone secondary to hypothalamic-pituitary aplasia. In fact, anencephaly is almost always accompanied by the post-term pregnancy in the absence of hydramnios. However, when associated with hydramnios, it leads to premature delivery. We report a case of spontaneous induction of labor in pregnancy of thirty-eight weeks and one day. The pregnant woman had a collateral venous circulation in the abdominal wall, the uterine height was 41 centimeters, and the ultrasound scan showed an anencephalic fetus associated with polyhydramnios. The newborn died seven minutes after delivery. Therefore, although very rare, anencephaly associated with polyhydramnios can reach term. Elle nécessite une prise en charge multidisciplinaire associant le dermatologue et le psychiatre. Nous rapportons, le cas d’une patiente âgée de 39 ans, gauchère, suivie en psychiatrie pour un trouble obsessionnel compulsif depuis deux ans sous thérapie comportementale, admise aux urgences ; pour une grosse jambe traitée initialement comme dermo-hypodermite non nécrosante sous antibiothérapie orale et locale avec un échec thérapeutique. Le diagnostic de pathomimie a été suspecté et la patiente a été mise sous pansements occlusifs. L’évolution était marquée par la cicatrisation des lésions. It represents a challenge for the dermatologist and requires multidisciplinary management involving the dermatologist and the psychiatrist. We report the case of a 39-year-old female patient, left-handed, who was followed in the Psychiatry Service for an obsessive-compulsive disorder for two years under behavioral therapy. She was admitted in the emergency for an erythematous leg and initially treated as non-necrotizing dermo-hypodermitis by oral and local antibiotic therapy with therapeutic failure. The diagnosis of pathomimia was suspected and occlusive pads were placed on the lesions. The evolution was marked by the healing of the lesions. The patient was referred to the Psychiatry Service for psychotherapy. Les paramètres d’interet englobaient : les caractéristiques sociodémographiques, les antécédents médicaux et /ou chirurgicaux, la gravité de la dysfonction érectile (DE) et l’évolution de la dysfonction érectile sous traitement. \u0000Résultats: Cent huit patients ont été recensés sur une période de 2 ans. L’âge moyen était de 45,8 ±14,3 ans. Les mariés étaient majoritaires (76,8 %). Plus de la moitié des patients (58,3 %) avaient un facteur de risque cardiovasculaire. La dysfonction érectile sévère était objectivée chez 64 patients (59,2 %). Près de la moitié des patients (47 %) avaient une fonction érectile normale après le traitement. \u0000Conclusion: La dysfonction érectile est préponderante parmi les patients ayant un facteur de risque cardiovasculaire. The aim of this study was to describe the epidemiological, diagnostic and therapeutic aspects of ED patients. \u0000 Methods: Retrospective descriptive monocentric study collecting records of patients aged over 18 years with ED, between January 1, 2018 and December 31, 2019, at the Urology Department of CHU Le Dantec, Senegal. The parameters studied were as follows: socio-demographic characteristics, medical and/or surgical history, severity of ED and evolution of ED under treatment. \u0000Results: One hundred eight patients were enrolled over a 2-year period. Their mean age was 45.8 ± 14.3 years. Married patients were preponderant (76.8 %). More than half of the patients (58.3 %) had a cardiovascular risk factor. Severe ED was diagnosed in 64 patients (59.2 %). Almost half of the patients (47 %) had normal erectile function after treatment. \u0000Conclusion: ED was common in patients with cardiovascular risk factor.
