The differential diagnosis comprises the exclusion of many non-infectious diseases and the effort to isolate the microbic agent in the case of septic shock. Between bacterial causes, Klebsiella pneumoniae is one of the bacteria that can trigger SPG through hypervirulence and hypercoagulopathy mechanisms. This case report details the situation of an 87-year-old man, currently partially self-sufficient and partially autonomous, without cognitive impairment, who has been affected with recurrent right knee osteomyelitis since the age of 6. A diagnosis of hepatitis C virus (HCV)-related liver cirrhosis at 41 led to liver transplantation at 56. The immunosuppressive anti-rejection therapy caused a relapse of chronic osteomyelitis and HCV infection, a major cause of graft loss and shortened survival in HCV liver transplant recipients. Other pathologies were also diagnosed during his lifetime. A 78-year-old man, who had been vaccinated against SARS-CoV-2, was hospitalized for a flu-like infection and abdominal pain, revealing bilateral hemorrhagic adrenal infarctions with secondary adrenal failure. Imagery examinations showed adrenal necrosis and multiple ischemic cerebral strokes. A multiplex retro-transcriptase polymerase chain reaction (RT-PCR) became positive for HCoV-OC43. Cardio-vascular, infectious, and auto-immune exams were negative. A steroid medication finally allowed clinical improvement, and the diagnosis of reactional vasculitis was retained. An unknown pathogenicity of HCoV-OC43 is possible, considering that RT-PCR research is rarely made. The hypothesis of cross-reactions with SARS-CoV-2 antivaccine antibodies is also feasible. At last, the possible former virulence of HCoV-OC43 is being researched, in the event that this virus is connected to the 1889 Russian influenza pandemic. Matos","doi":"10.4081/gc.2023.10972","DOIUrl":"https://doi.org/10.4081/gc.2023.10972","url":null,"abstract":"The objective of this study is to investigate the presence of sarcopenia and the risk of oropharyngeal dysphagia (OD) in a geriatric population and to analyze the relationship between the factors associated with sarcopenic dysphagia. The cross-sectional study was carried out in two nursing homes. The presence of OD was screened using the gugging swallowing screen, and sarcopenia was assessed according to the European working group on sarcopenia in older people criteria. The sample (N=36; 23 women, 13 men) presents a mean age of 88.0±5.6, with 55.6% being at risk of OD, 52.8% with sarcopenia, and 36.1% with probable sarcopenic dysphagia. The score of the simple questionnaire to rapidly diagnose sarcopenia (SARC-F)≥4 was a significant predictor (odds ratio=9.0; confidence interval 95%=1.285-63.025) for the risk of having sarcopenic dysphagia. It was observed that sarcopenia was associated with higher odds of being at risk of OD. Also, an increase in age, poorly fitting prostheses, a higher level of dependence during activities of daily living, and a risk of malnutrition or malnourishment raised the potential of having probable sarcopenic dysphagia. Data from 144 patients admitted to the general geriatric outpatient services of Luigi Vanvitelli University Hospital (Naples, Italy) and San Felice a Cancello Hospital (San Felice a Cancello, Italy) were collected between January 1 and December 31, 2017. Cognitive function was evaluated through the mini-mental state examination and nutritional status was assessed through mini nutritional assessment. Logistic regression was used to assess the association between clinical characteristics and nutritional status, and it is reported as unadjusted, adjusted for age and sex, and adjusted for other potential confounding factors. In the unadjusted model, poor nutritional status was associated with a worse cognitive deficit (odds ratio (OR)=2.36; 95% confidence interval (CI)=1.77-3.55). The association remained significant after adjusting for age and sex (OR=2.36; 95% CI=1.77-3.55) and in the fully adjusted model (OR=2.92; 95% CI=1.90-5.64). Irradiation is also commonly associated with chronic lymphedema of extremities. It generally occurs about 10 years after the mastectomy and/or radiotherapy. The prognosis is very poor with a mean survival of 20 months from the diagnosis especially when radical surgery is not possible. We present the case of STS in an 89-year-old female who underwent left upper outer quadrantectomy for invasive ductal carcinoma in 2009 followed by axillary lymphadenectomy, radiotherapy, and hormonal therapy until 2014. She presented swollen upper left limb associated with hemorrhagic red papular lesions. Skin biopsy revealed the presence of lymphedema-associated angiosarcoma. Radical surgery was not possible, so she underwent conservative therapy with pazopanib, a tyrosine kinase inhibitor, with benefit. The case report describes a married 78-year-old white rural male worker with low level of education. Given the social isolation brought on by the COVID-19 pandemic, the patient initially received antidepressants for a history of adynamia, muscle weakness, and inappetence for a year. Four months before admission, he had an aggravation of the clinical picture, presenting mental confusion, and becoming dependent for all daily life activities. He was hospitalized with adynamia and intense fatigue, muscle weakness, inappetence with weight loss, splenomegaly and severe hypercalcemia (15.9 mg/mL; corrected by serum albumin - reference value: 8.8 to 10.3 mg/dL) and low parathyroid hormone level.
