It is therefore essential to know which are the chronic pathologies that affect them most frequently, what impact they have on the oral cavity and what type of treatments are required. The aim of this study is to shed further light on these questions.</p><p><strong>Material and method: </strong>Articles in PubMed on chronic and oral pathologies and treatment needs were reviewed.</p><p><strong>Results: </strong>The most prevalent chronic pathologies are heart disease, chronic respiratory disease, diabetes and cancer. Anxiety and depression are also highly prevalent among the prison population, along with the consumption of tobacco, alcohol and drugs of abuse. They are responsible for the appearance of caries and periodontal diseases, the treatment of which should be carried out in the prison environment.</p><p><strong>Conclusions: </strong>The level of dental care among the prison population is very low. International guidelines recommend a thorough drug-drug interaction (DDI) risk assessment prior to starting DAA therapy and before starting comedications. This study aims to evaluate the impact of potential multiple DDIs in the real-life adverse event (AE) profile of PLWHCV treated with DAAs.</p><p><strong>Material and method: </strong>This is a retrospective, observational study using electronic medical records. Patients included were PLWHCV and were treated with either the protease inhibitor (PI) free DAA sofosbuvir/velpatasvir (SOF/VEL) or glecaprevir/pibrentasvir (GLE/PIB) between 2017 and 2020. Potential (single and multiple) DDIs were identified using the Liverpool HEP Interaction Checker. AEs potentially associated to DDIs were identified during DAA treatment period.</p><p><strong>Results: </strong>1620 patients with DAA prescriptions (SOF/VEL or GLE/PIB) were included. Of these, 123 were predicted to have multiple DDIs (multi-DDI). About 10% (123/1256) of the patients receiving ≥2 comedications were at risk of multi-DDI with DAA. Most comedication-associated AEs were recorded in this multi-DDI population (88.9%, 16/18), meaning that 13% (16/123) of the multi-DDI population suffered AEs. According to DAA regimen, more comedication-associated AEs were reported in GLE/PIB-treated as compared with SOF/VEL-treated patients (18.3% [13/71] vs 5.8% [3/52] p<0.05). These AEs were mainly reported by primary care physicians (62.5%).</p><p><strong>Discussion: </strong>PLWHCV predicted to have multiple DDIs are at high risk of AEs. When the measurement varied, concordance was checked using the kappa index (() and its clinical significance. To determine variables associated with clinical significance, bivariate and multivariate analysis was performed using logistic regression.</p><p><strong>Results: </strong>488 cases were studied. TST was positive in 20.1, 23.4 and 23.8% at 48 hours, 72 hours or any of them, respectively. There was more TST positive in foreigners or BCG vaccinated. The reading varied in 35.2% (mean deviation 5.5 +/-4.4 mm) and was higher at 72 hours (86% of different cases). Inter-reading concordance was excellent overall (( = 0.892) and in BCG vaccinated (( = 0.805), but moderate in immunocompromised (( = 0.421). Discordance was clinically significant in 3.7% and was not associated with any variable.</p><p><strong>Discussion: </strong>TST concordance at 48-72 hours is excellent, but in some patients (3.7% in this work) the score is incorrect, more so in readings at 48 hours. The aim of this study is to evaluate the effectiveness of group therapeutic patient education in 15 people diagnosed with diabetes, with poor metabolic control, inmates of the Madrid III-Valdemoro Penitentiary Center (Madrid), whose population is exclusively male.</p><p><strong>Material and method: </strong>Quasi-experimental study based on 4 training sessions aimed at a group of 15 people with diabetes and poor metabolic control. The intervention was carried out by 4 health professionals from the center. An evaluation of knowledge before and after the training and an assessment of the analytical and anthropometric parameters 5 months after the training were carried out. An anonymous self-evaluation questionnaire was also completed by the participants.</p><p><strong>Results: </strong>After the intervention, a statistically significant reduction in weight, abdominal perimeter, glycosylated hemoglobin and diastolic blood pressure was obtained. The remaining parameters decreased non-significantly. {"title":"COVID-19 and anxiety amongst health professionals in prisons.","authors":"María Pilar Molés-Julio, Miguel F Ruiz-Garrido, Martín Flores-Saldaña, María Vicenta Lucas-Miralles, Aurora Esteve-Clavero","doi":"10.18176/resp.00087","DOIUrl":"10.18176/resp.00087","url":null,"abstract":"<p><strong>Objectives: </strong>The main aim of this article was to analyze the emotional levels of anxiety amongst health professionals employed in prisons during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) epidemic.</p><p><strong>Material and method: </strong>This descriptive, cross-sectional study was conducted by means of a validated anxiety questionnaire answered by 100 clinical nurses and nursing assistants residing in Spain and working in prisons. Alongside the demographic information, the test included 14 items related to anxiety levels. The questionnaire turned out to be highly reliable after carrying out an analysis with a Cronbach´s alpha of 0.935, demonstrating high reliability.</p><p><strong>Results: </strong>Over one third of health participants presented severe anxiety levels, with a greater impact on women than on men. The situation in centers for the elderly appears to be more tense and complex than in prisons, although the latter are far more overcrowded. The risks of infection for oral health personnel are considered high, mainly from cross-infection between patients.</p><p><strong>Objectives: </strong>Differentiate between a true and false dental consultation emergency during the SARS-CoV-2 outbreak for a better and effective screening of inmates of the Social Rehabilitation Center (CERESO) of San Francisco Kobén (Campeche, Mexico).</p><p><strong>Material and method: </strong>An observational, cross-sectional, descriptive, and prospective study was designed for a sample of 100 inmates of the CERESO San Francisco Kobén, the data was collected in the prison's dental office, the participants signed a letter of informed consent to be voluntarily included in the study during the SARS-CoV-2 outbreak. The questionnaire \"Assessment of a true Dental Emergency\" previously validated for the Mexican population was applied, the personnel was standardized and an intra- examiner and inter-examiner reliability of k = 0.98 was obtained. Cases of scabies from March 01, 2021 and March 01, 2022 were analyzed according to the International Alliance for the Control of Scabies (IACS) 2020 criteria: a) confirmed cases; b) clinically probable cases; and c) suspected cases.Epidemiological, clinical, therapeutic and regimental control variables were collected. We checked whether the interventions were homogeneous in each site. The mean and standard deviation were used as continuous variables and percentages and confidence interval (95%) for discrete variables. The Chi-square test was used to determine statistical association.</p><p><strong>Results: </strong>269 cases were detected (21.2% confirmed, 50.2% clinically probable and 28.6% suspected). 61.3% were treated with permethrin, 18.6% with ivermectin and 20.1% with both. 27.1% of the cases were withdrawn.There were no significant in-site differences in the study of contacts, isolation and treatment of clothing and belongings, but there were significant differences in the prescription of drugs (P <0.001), duration of treatment (P <0.001) and disinfection of the cell (P <0.001).</p><p><strong>Discussion: </strong>The number of detected cases is estimated to be high, and the majority (71.4%) confirmed or clinically probable. Control measures are homogeneous, except for cell disinfection, which was conventional in 59.1%. Pharmacological treatment and its duration varied between prisons.
