Enlargement of optic nerve sheath and increased subarachnoid fluid around the optic nerve are uncommon magnetic resonance (MR) findings of the patients with pseudotumor cerebri. To the best of our knowledge, these findings were not reported in pseudotumor cerebri due to Behcet's disease. Lumboperitoneal shunting surgery may be performed in cases which have visual symptoms and refractory to the medical treatment. The systems designed for surgical patients include the ASA and P-POSSUM. NRS-2002 score is suggested for screening of nutritional risk. We aimed to assess the performance of prognostic models, and to compare the reliability of NRS-2002 with those, in major gastrointestinal surgical patients. APACHE II and III, SAPS II, MPM II and P-POSSUM scores, ASA grading, and NRS-2002 scores of the patients underwent major gastrointestinal surgery were collected on admission. Calculations were repeated for APACHE II and III, SAPS II, and MPM II at postoperative 24 th hour. Discrimination and calibration characteristics of the scoring systems were evaluated.APACHE II-III, and SAPS II at postoperative 24th hour, and P-POSSUM on admission, had reliable power of discrimination and calibration for mortality prediction in patients undergoing major gastrointestinal surgery. APACHE III, SAPS II and PPOSSUM had the best performance on complication prediction. ASA, MPM II, and NRS-2002 had insufficient discrimination statistics. APACHE II and III, SAPS II, and P-POSSUM are superior to other systems in predicting mortality in gastrointestinal surgical patients. {"title":"Ocimum gratissimum impairs gut glucose absorption but enhances water absorption in Streptotozocin induced diabetic rats","authors":"U. Okon, E. E. This study therefore assesses the effect of diabetes mellitus (DM) and treatment with Ocimum gratissimum (OG) on gut glucose and water absorption. Phytochemical analysis and LD50 of OG was conducted as preliminary studies. Diabetes was induced in group 2; diabetic untreated group (DM group) and 3; diabetic treated group (DMTgroup) with a single i.p dose of 65mg/kg of Streptotozocin (STZ), with the later treated with 1500mg/kg of OG. Tissue sections for histology were obtained by standard method and absorption of fluid and glucose conducted by the everted sac technique. The result showed that glucose uptake was significantly lower (P<0.05) in the DM and DMT groups compared to the control while the fluid uptake in the DMT group was significantly lower (P<0.05) than the DM group and lowest in the control group. OG treatment reduces glucose absorption but increases gut fluid intake. It is possible that the hypoglycemic effect of OG may be related to reduction in intestinal glucose absorption. The aim of this study is to evaluate the characteristics of patients and results of port catheter insertions. We retrospectively reviewed demographic and clinical properties of 224 children who underwent implantable port catheter insertions in order to receive long-term intravenous treatment between 2003 and 2013. Port catheters were implanted in 224 patients (124 female and 100 male). The mean age of patients was 5.04±3.75 years (2 months-18 years). The most common oncologic diagnose was acute lymphoblastic leukemia. Seventy-two port catheters were removed due to infection in 40 and completion of treatment in 32 patients. 25 of 152 port catheters inserted via subclavian puncture and 15 of 90 port catheters inserted via jugular cut down were removed due to infection (P=0.965). Port catheters were inserted 3 times in 3 patients and 2 times in 15 patients. The mean duration of usage was 442.5±381.3 days among patients whose catheters were removed due to infection and 964.4±298.8 days among patients whose treatments were completed (P<0.001). Pneumothorax or hemothorax were observed in none of the cases. Four patients needed revision of eroded skin over the chamber. We were not able to remove the catheter from right jugular vein in a patient due to firm adhesion of catheter to the vessel wall. Catheter migration occurred in one patient and removed by endovascular procedure. Since the clinical outcome of breast cancer may differ among some women who have the same clinicopathological stage, researchers focused on additional prognostic parameters to predict the tumor behavior. The aim of this study was to investigate the prognostic value of Wilms Tumor 1 (WT1) expression in tumor tissues and to compare it with known prognostic variables in patients with breast cancer. In patients with breast cancer, we investigated the relationship between (WT1) protein expression in tumor and surrounding tissues and prognostic variables including age, pathologic type, axillary node involvement, estrogen receptor (ER) status, menopausal status, stage (TNM), tumor grade and treatment. Borderline significance was detected between WT1 monoclonal antibody (mAb) staining and premenopausal state (p=0.051). Additionally, surrounding tissue staining showed significant correlations with grade (p=0.045), stage (p=0.026), lymph node status (p=0.026), and axillary involvement (p=0.02), respectively. No correlation was demonstrated between relapse free survival, relapse sites and WT1 mAb staining of tumor and surrounding tissues (p=0.36). WT1 mAb staining was demonstrated in human breast cancer tissues, and in this study we have used monoclonal antibody against WT1 on paraffin-embedded tissue samples. The results indicate that WT1 expression by tumor is more evident in premenopausal state rather than postmenopausal period. To this day, games and conventional treadmill exercise tests have not been carried out in pre-pubertal children. Children cannot easily adapt to the treadmill or cycle ergometer. We therefore aim to the use of video games as a form of exercise tests in pre-pubertal children. Twenty healthy children (10 girls and 10 boys, aged 5-11 years old) were enrolled in the study. The physical examination, 12-lead electrocardiography and echocardiography were performed. The heart rate and respiratory rate were measured with systolic blood pressures. Treadmill exercise testing was performed, and electrocardiographic changes were studied during both the treadmill exercise test with Bruce protocol and an activity-promoting video game (Nintendo Wii Boxing), in different time periods. Exercises were stopped at the target heart rate.The mean duration of exercise was 15 minutes in both the Bruce protocol and the activity-promoting video game. Although the data of healthy children (including heart rate and blood pressure responses to exercise) were consistent with the results from several countries using the Bruce protocol, the mean maximal heart rates for all groups were slightly lower than those obtained with the video game. The data obtained from this new exercise test may be used to determine the diagnosis and activation of cardiovascular disease in pre-pubertal children. Being the most common among oral cancers, 95% of lip cancers are located on the lower lip, 4% on the upper lip and the remaining one percent at the commissure. The purpose of this article was to analyze retrospectively in lower lip cancer patients who were operated in our clinic. Between October 1999 and December 2009, 57 patients with lip cancer were analyzed retrospectively, with regards to their age, state of oral hygiene, etiological factors, nodal status at the first visit, period from the onset of symptoms to the patient's first application to our clinic, follow-up period, neck dissection, postoperative radiotherapy and the TNM classification of the tumors. Hereby, we aimed at presenting our results and comparing them with the current data. The mean ages of the patients were 63.21 years and 70.17% were male and the remaining 29.83% were female. General oral care and hygiene was good in 8.77% and bad in 49.12% of males and good in 5.35% and bad in 19.29% of females. 52.64% of males and 5.35% of females were active smokers, and 44.5% of all patients had lymphadenomegaly at first admission. According to TNM classification, 43.88% of patients had T1, 33.32% had T2, 17.53% had T3 and 5.27% had T4 tumors. The tumors were excised with 1 cm surgical margins and 39 patients had cervical lymphatic dissection. 33.34% of the patients were given postoperative radiotherapy. During the first two years postoperatively, 89.48% of the patients visited the clinic on a regular basis, however, the intervals between visits increased thereafter. Lip cancers are commonly seen in males of advanced age. Following treatment, a long survival is evident in cases of early staged tumors.
