{"title":"THE INTER-RATER AND INTRA-RATER RELIABILITY OF GLENOHUMERAL JOINT POSITION AND MOVEMENT SENSE TESTS APPLIED USING AN ISOKINETIC DYNAMOMETER","authors":"S. Arslan, G. Yapali","doi":"10.21653/tjpr.997172","DOIUrl":"https://doi.org/10.21653/tjpr.997172","url":null,"abstract":"Purpose: The aim of this study was to test the inter-rater and intra-rater reliability of glenohumeral joint (GHJ) position and movement sense tests of an isokinetic dynamometer in healthy individuals. \u0000Methods: The study included 17 healthy subjects. GHJ position and movement sense tests were applied with an isokinetic dynamometer to the dominant extremity. \u0000Results: The different error types calculated for position sense were in the range of mean 0.02±1.62 - 5.42±1.74. The ICC value for the different error types was determined in the range of 0.038 - 0.657 for the intra-rater tests, and 0.095 - 0.779 for the inter-rater tests. The movement sense test results for different directions and angles ranged from 9.61±2.61 to 11.18±2.89. The ICC values for movemeent sense were in the range of 0.687 – 0.912 for the intra-rater tests, and 0.844 - 0.925 for the inter-rater tests. \u0000Conclusion: The isokinetic dynamometer showed moderate-good intra-rater and inter-rater test reliability in the measurement of the GHJ internal rotation movement sense. The intra-rater test reliability was poor in external rotation but the inter-rater test reliability was moderate-good. However in the measurement of movement sense, the isokinetic dynamometer showed intra-rater and inter-rater test reliability at an excellent level. Demographic characteristics, COVID-19 infection diagnosis and quarantine due to contact were questioned via a structured online form. COVID-19 related fear was assessed by using the Fear of COVID-19 Scale. \u0000Results: No statistical differences were detected regarding demographical characteristics and COVID-19 infection rates between the groups (p> 0.05). The virtual reality training group performed virtual reality training using the XBox One Kinect gaming system, and the Control group performed neurodevelopmental training. Outcome measurements were Pediatric Balance Scale, Gross Motor Function Measurement-88, Quality of Upper Extremity Skills Test, Functional Reach Test, Sit to Stand Test and Pediatric Disability Evaluation Inventory. Each training program was carried out for eight weeks, two times per week and 45 minutes per session. \u0000Results: All measurements were statistically increased in both groups. Exercise has generally been associated with improving psychosocial aspects of pain, such as kinesiophobia, depression, and anxiety in LBP patients, but its effect on the paraspinal muscles needs to be focused on. For this reason, the aim of our study is to evaluate different types of exercises that will help increase neuromuscular facilitation and core stability in paraspinal muscles by considering patient satisfaction. \u0000Methods: A total of thirty-seven female patients with chronic low back pain were included in the study. The Oswestry Low Back Pain Questionnaire was used to assess functionality. Core stability was evaluated with a stabilizer. Pain intensity and satisfaction were measured with visual analog scale. Patients' kinesiophobia was assessed by using Tampa Kinesiophobia Scale. Patients were randomized into three groups according to the interventions as; proprioceptive neuromuscular facilitation (PNF) exercise group (n = 9), core stability exercise group (n = 14) and control (n = 14) group. The duration of applications was 3 days per week with a total of 6 weeks. \u0000Results: There was a group difference for core muscle strength (p = .045), Oswestry score (p = .001), pain intensity score (p = .003) and Tampa score (p = .001). There were significant gains for core muscle strength and Oswestry scores for proprioceptive neuromuscular facilitation and core stability groups (p < .05). \u0000Conclusion. New York Postür Derecelendirme Ölçeği (NYPD) ile genel vücut postürü, fleksible ruler ile torakal kifoz açısı (TKA)- lumbal lordoz açısı (LLA) değerlendirilmiştir. Skolyometre kullanılarak olası skolyoz varlığı ve pelvik asimetri değerlendirilmiştir. Üst Ekstremite (ÜEFS) ve Alt Ekstremite Fonksiyon İndeksi (AEFS) ile hastalık şiddeti, 2 Dakika Yürüme Testi (2DKYT) ile fonksiyonel kapasite, Zamanlı Kalk Yürü Testi (ZKYT) ile denge ve düşme riski, Fonksiyonel Bağımsızlık Ölçeği (FIM) ile günlük yaşam aktiviteleri kaydedilmiştir. \u0000Sonuçlar: Postüral deformitelerin değerlendirilmesi sonucunda hastaların NYPD’si ortalamasının 40,24±9,56 olduğu belirlenmiştir. Yapılan ölçümlerde hastaların %46,34’ünde TKA’nın, %24,39’unda ise LLA’nın açısının norm değerlerin dışına çıktığı ve skolyometre ölçümleri sonucunda 30 hastada (%73,2) olası skolyoz varlığı, 11 hastada (%26,8) spinal asimetri olduğu, 27 hastada (%65,9) ise pelvik asimetri olduğu bulunmuştur. All the patients were informed about bruxism and its effects and were given home exercises. The patients in the ST group were instructed to use a static splint for one month. The MT group patients received a total of 8 sessions, 45 minutes twice a week of treatment, including joint and soft tissue mobilizations to the temporomandibular joint (TMJ) and cervical region. There are many methods in the literature about the evaluation and treatment of this disorder. This study aims to create a consensus on the opinions of physiotherapist with certification Mulligan Concept Practitioners about Mulligan Concept and other frequently used physiotherapy approaches in people with cervical headache. \u0000Methods: This study was planned to be completed in 3 internet-based survey rounds. The surveys were sent to physiotherapist with Certified Mulligan Practitioner. The consensus was achieved when 70% of the experts agreed. \u0000Results: Consensus was reached on 16 key messages. The most frequently preferred assessment methods are Visual Analogue Scale, Neck Disability Index, Flexion Rotataion Test, and Range of Motion.
