In general, as treatment, compression therapy and indifferent ointments/cremes are used, often together with topical steroids, though the latter may exhibit considerable side effects. This study aims to explore the effect of Medical Moisture Retention Cream (MMRC = ALHYDRAN®) on the symptoms and complaints of patients suffering a venous eczema, often next to an existing VLU. The working mechanism of MMRC involves a combination of the moisturizing effect of Aloe Vera gel and the moderate occlusion effect of added fatty acids. \u0000Method: In an open case series, 18 patients attending an outpatient wound clinic with moderate venous eczema (maximum TIS score<3) participated. MMRC was applied twice a day and its use was clinically assessed for 4 consecutive weeks. Next to the registration of patient characteristics and capturing the clinical details of the skin lesions in weekly pictures, a VAS scale to assess the patients’ and caregivers’ experience with the treatment was used. \u0000Results: During the application of MMRC, the signs and symptoms of a dry, itchy, scaly, crusty and erythematous skin, fainted in all patients. The skin condition of most participating patients improved and there was also a visibly better skin hydration status in all patients. Relevant aspects such as ‘night rest’, ‘mood’ and ‘social participation’ improved, except for 2 patients. The clinical skin condition of one of these deteriorated in a week, which was not related to the use of MMRC. No side effects to the use of MMRC were observed. The wound care experienced nurses assessed MMRC as effective and feasible. \u0000Conclusion: This study shows that MMRC is effective, safe, and feasible in the treatment of venous eczema. Future Randomized Controlled Trials are necessary to compare the efficacy and feasibility of MMRC with the application of other hydrating creams/ointments and topical steroids.

Besides, the variable pharmacokinetics and pharmacodynamics, numerous other factors need to be considered for any surgical procedure or during treatment of critically ill geriatric patients in intensive care units. Besides taking care of clinical comorbidities, many other preventive measures are necessary to minimize the morbidity and mortality in this subset of population. The concept of ‘quaternary prevention’ has recently gained popularity but its application in geriatric population presenting for surgery has been very limited. The current brief review discusses various aspects of quaternary prevention applicable to anesthesia procedures in geriatric patients during peri-operative period.

The Older People in Acute Care Survey (OPACS) is an instrument measuring hospital nurses attitudes regarding older patients and validated in Australia and the USA. The OPACS is translated in Dutch language and content validity of this translation is previously assessed, presenting questionable results. Measurement instruments, however, cannot be “validated” based on content validity evidence alone. Judgmental evidence and statistical analysis should be combined to fully evaluate content domain definition and representation and guide further development. \u0000Objective: Assess structural validity and reliability to fully evaluate the OPACS for use in the Netherlands, complementing previous conducted content validity results. \u0000Design: Cross-sectional. \u0000Setting: Three general hospitals in the Netherlands. \u0000Participants: 201 registered nurses. \u0000Methods: Confirmatory factor analysis was used to assess the structural validity. Reliability was assessed with Cronbach’s alpha. \u0000Results: OPACS Section A (measuring practice experiences) demonstrated to have acceptable structural validity- and good reliability outcomes after exclusion of two items (model fit: x² [df=537]=8475.40, p |z|)= 0.551 -0 .788). \u0000Conclusion: Even though structural validity for section A was acceptable, content validity scores of a majority of items in this subscale were low, resulting in questionable use of this subscale for the Dutch context. The findings of this study, in relation to the earlier findings regarding content validity, justify the conclusion that use of the Dutch OPACS in clinical practice and research is not recommended. Given these findings, future research should pursue the development or (cross-cultural) validation of other instruments measuring hospital nurses attitudes towards older patients for the Dutch cultural context. Furthermore, this study demonstrated the influence of cultural differences on measurement instruments and the need for rigorous research before using a measurement instrument in a new culture or context.

According to WHO, the elderly population will almost double from 12% to 22% within a span ranging from 2015 to 2050. A similar trend is anticipated for the US. The data seems to be encouraging in terms of increase in life expectancy and reduced mortality due to advancements in medical science. But the flipside is that, the increased number of elderly commands increase in number of, or visits by caregivers to manage the increased demand for Quality of life for the elderly. Another study focused on the increased stress level with emotional and physical fatigue among care givers. A partial solution to this problem can be provided by technological interventions through the branch of Gerontechnology. This paper is an attempt to enlist and analyze the technology, which can augment the Quality of life of elderly along with reducing the burden on formal and informal caregivers.
