The technology made it possible to open dialog channels and monitor conversations in real-time. However, a model capable of supporting and guiding this process is lacking. We present in this article the theoretical basis and the methodological path used to develop a model for the monitoring and evaluation of Public Communication of Science and Technology (PCST) programs, carried out in the digital environment, based on the Theory of Change (TM) approach. In this study, we analyzed the management mechanisms present in the context of the implementation of the Program for Improving Access and Quality of Primary Care, based on the discourse of municipal health managers, presenting analyzes anchored in post-structuralist premises. The perception of the consolidation of the formal model of work management in health services is highlighted, using performance evaluation and payment of financial rewards as motivation strategies for improving the work process and for disciplining and controlling care workers basic. The emergency of the Covid-19 pandemic made its implementation unfeasible, causing a redesign process. This article analyzes the changes promoted in the design of the program to adjust it to contextual constraints. We conducted a case study, participant observation, documental research, and content analysis. We found that the policy objectives were not changed, but the implementation instruments were. Regulatory, organizational, financial, and informational instruments were activated, which institutionalized new practices and processes, modifying the forms of interaction with families. Such changes involved adding new elements to the design without removing existing ones. Innovative initiatives to promote child development should be evaluated. In Brazil, there is still a gap regarding the assessment of the efficacy and effectiveness of innovative solutions for early childhood with large-scale implementation in public policies. The Early Childhood Innovation Lab supported innovative projects based on the IDEAS Impact Framework. This is an approach to create intervention programs involving the theory of change with expected effects, the development of materials, and the evaluation plan. Program feasibility tests should be performed in rapid, repeating cycles. In the pilot study the program is applied on a small scale to assess whether it has worked and for whom and in what context it has worked.
