There are various pharmacologic therapies recommended for OA knee. Intra-articular Platelet Rich Plasma (PRP) and Intra-articular Triamcinolone have been shown to relieve pain and improve quality of life in patients with OA knee. This study is conducted to compare the effectiveness of PRP and Triamcinolone intra-articular injections in Grade 1 & 2 OA knee.We conducted a randomized control study including 70 patients with Grade 1 & 2 (Kellgrenn & Lawrence grading) OA knee. 35 patients each were divided into the PRP group and Triamcinolone group. Intra-articular PRP 5ml and Intra-articular Triamcinolone 80mg were injected twice 3 weeks apart. The effectiveness of the treatment was evaluated by using VAS, KOOS, and WOMAC scores at 3 weeks, 3 months, and 6 months of the follow-up period.At 3 weeks follow up both the groups showed similar results decreasing pain and improving quality of life. At 6 months follow-up, the PRP group showed better results, and the same effect was observed at 6 months of follow-up. Overall, the PRP group showed better VAS, KOOS, and WOMAC scores compared to the Triamcinolone group.In the present study both the groups showed improvement in function and reduction in pain in patients with Osteoarthritis of the knee but Triamcinolone had a short-term effect compared to PRP. {"title":"Ochronotic Knee Arthropathy: A rare cause of arthritic knee pain and literature review of surgical considerations during knee arthroplasty","authors":"L. Rarely the arthritis of knee may be due to ochronotic arthropathy. It is often missed or misdiagnosed as osteoarthritis. An autosomal recessive metabolic disorder characterized by the deficiency of homogentisic acid oxidase enzyme preventing metabolism of homogentisic acid lead to homogentisic aciduria. Alkaptonuria progresses to ochronotic degeneration and arthritis. Early management of ochronotic arthropathy is challenging and is mainly limited to control of symptoms. Advanced cases may necessitate surgery.\u0000Case Report: A 65-year-old male presents with persistent progressive pain in both knees, not responding to conservative measures. Patient required simultaneous bilateral cemented total knee arthroplasty for management of arthritic knee pain. Arthrotomy revealed black joints with pigment deposition throughout the articular cartilage and neighboring tissues. Meticulous surgical technique allowed uneventful procedure. Postoperatively patient gained pain-free functional knee movements and was able to ambulate without support within six weeks of arthroplasty. The Knee Society Score improved to 95 bilaterally. At two years,\u0000radiologically there was good alignment with no loosening.\u0000Discussion: Arthroplasty is an excellent choice for patients with ochronotic arthropathy. However, more information and experience are needed for uniform implant selection and techniques. We report this rare, often missed cause of common knee pain. Due to global lockdown and restricted social movements, there is a rising curve in the morbidity of health of non- SARS-CoV-2 orthopaedic rheumatology patients and an impairment in the functional quality of life necessitating a pandemic response protocol into action in all fields of patient care. The development of ideal strategies and recommendations for the management of orthopaedic rheumatologically predisposed population and principles to be followed in their practice is the need of the hour. We hereby give a comprehensive list of strategies that can be followed in orthopaedic rheumatology practice by encompassing the recommendations given by the European League Against Rheumatism (EULAR), The American College of Rheumatologists (ACR), and Indian Association of Orthopaedic Rheumatology - Indian Orthopaedic Association (IORA-IOA). SARS-CoV-2 pandemic has put enormous pressure on patients with Rheumatic Diseases (RD) due to their innate weakness and shared treatment protocols in management. Often, they are asymptomatic, associated with growing bone and their discovery is incidental. Osteochondromas in the talus are quite rare and their place as a cause for tarsal tunnel syndrome is rarer still. Here we report a case of a 10-year-old female patient having an osteochondroma of the talus that was presenting with the symptoms of tarsal tunnel syndrome along with tibio-talar arthritic changes. Local steroid injection and surgical release forms the mainstay treatment in the conservative and surgical line of treatment respectively till date. This study aims to bring out the effectiveness of local steroid injection given accurately under ultrasound guidance making it comparable to surgical release in the short term period, thereby emerging as an immediate and cost – effective line of management. \u0000Materials and Methods: Twenty three patients diagnosed with de Quervain’s tenosynovitis were included in this study triamciniolone and 1 ml of 1% lidocaine hydrochloride was taken under aseptic precautions and was injected into the involved compartment under expert guidance with help of ultrasound imaging. Clinical improvement was later evaluated by analyzing the pre procedural and post procedural DASH (Disabilities of the Arm, the Shoulder and the Hand) score indexes comparing it to surgical release.8whom did not show any relief of symptoms when conservative mode was used namely rest and analgesics and splinting in certain cases. Clinically patients were assessed using DASH score based on the severity of condition before the procedure and after the procedure. A mixture of 1 ml of \u0000\u0000Results: 22 patients (96% were symptom-free) out of the 23 patients that were included, after the 1st injection at two weeks. At the end 6 months 22 (96%) patients were symptom free and were fully satisfied with the gratifying outcome. No recurrence in these patients after a 1 year of follow-up was recorded. \u0000The one failed patient underwent surgery for release of the first extensor compartment and was symptom-free at the 2 weeks post –op after assessment. Among many HIV HBV & HCV are deadliest diseases. Among the other surgical specialities, orthopaedics has the highest rate of glove perforation. Hence this study was conducted to study the incidence of glove perforation & their pattern in our institute.\u0000Materials and Methods: all the gloves (1444) collected from 286 operations which occurred during 3months period were tested, out of which 172 were major procedures and 114 were minor procedures. During the procedure, if any perforation was detected to naked eye examination then they were labelled as perforated. And rest the gloves were tested as per American society for testing and material standard (ASTM d5151 - 06) guidelines for any perforations. \u0000Results: In our prospective study, perforation occurred in 74(26%) surgeries. Total of 173(12%) gloves were perforated, 51(17.83%) from major & 23(8.04%) from minor procedures, the primary surgeon was most commonly involved (68%). Only 53(30.6%) glove perforation was detected during the procedure. Among the double glove users, inner glove perforation was seen in only 5% of cases. The most common fingers involved was left index (38%), then right index finger (28%) followed by left thumb (20%) and right thumb was (14%).the primary surgeon was most commonly involved (68%). The mean duration of the surgery in which perforation was noticed was 76+6minutes.\u0000Conclusion: Double glove practice is better than the use of the single glove.
