Present study is designed to assess the results of primary hemireplacement arthroplasty of hip for displaced femoral neck fracture in the elderly patients by Austin Moore prosthesis. To reduce the hospital stay and provide better functional results like early easy mobilization and weight bearing and to reduce the incidence of Fracture Disease. To assess the complications of primary hemireplacement arthroplasty by Austin Moore prosthesis. Materials and Methods: In this study, primary hemireplacement arthroplasty of the hip was done in 30 patients with fracture neck of femur using Austin Moore prosthesis who were admitted in the Department of Orthopaedics in Konaseema institute of medical science amalapuram, from August 2014 to August 2016. Intra capsular fracture neck of femur in patients of age 60 years and above, Non-united fracture neck of femur, all types of fracture under Gardens classification are considered. Result: In 43% (13) patients fracture was in right side and 57% (17) patients fracture was in left side. Garden type-4 fracture was present in 14(46.7%) patients, Garden type-3 fracture was present in 12(40%) patients. 13.3% (4) patients were having Garden type 2 fracture. Average blood loss during the procedure was less than 500ml in 20pts, between 500-700ml in eight patients, and more than 750ml in 2 patients. The final Harris hip score was excellent in 46.7% of the patients, good in 8(26.7%) patients, fair in 6(20%) patients and poor in 2 (6%) patients. Discussion and Conclusion: We conclude that hemiarthroplasty for fracture neck of femur is a good option in elderly patients. The mortality and morbidity are not high, operative procedure is simple, complications are less disabling. Patients presents with swelling and pain, and sometime also with neurological symptoms when median nerve get involved. Patient evaluated clinically for the diagnosis, but the MRI is the gold standard tool for the diagnosis. Histopathological evaluation is also needed to confirm the diagnosis. Management is mainly surgical followed by full course of anti tubercular therapy. It is a rare condition with worldwide prevalence of about 1 case in 2 million individuals. No ethnic, racial, sex or geographical predisposition is noted. Myositis Ossificans Progressiva term is misnomer and not used now. The case report herein describes this rare condition in a child of one and half year age having the classical clinical presentation and radiological features. MIPPO is one of such emerging operative procedure which does give excellent results over conventional ORIF platting. We are comparing overall functional outcome and surgical complexity of MIPPO (Minimal Invasive Percutaneous Plate Osteosynthesis) vs conventional ORIF (Open Reduction and Internal Fixation) by posterior approach in this study. Materials and Methods: A retrospective clinical study was done on 50 patients with shaft humerus fractures treated with MIPPO anterior bridge plating and posterior platting by trained single surgeon at a tertiary trauma care centre in the Department of Orthopaedics, BJ medical college, Civil hospital Ahmedabad between October 2016 and May 2018. Follow up data was collected and patients were called for clinical and radiological evaluation. Results: 24 of the 25 (96%) MIPPO patients and 22 of the 25 (88%) ORIF patients achieved union. Average union times for the MIPPO and ORIF groups were 19.3 and 16.1 weeks respectively. At the end functional outcome of both the technique is as follows: the Mayo Elbow Performance Score (MEPS; range 95–100) were 90% in 20 cases (n = 20) operated with conventional ORIF posterior plating and 20 cases operated with MIPPO plating which is excellent scores. Two cases operated with ORIF posterior plating and three cases with MIPPO plating are showing a good MEPS score. Back pain due to musculoskeletal cause varies from mechanical pathologies, juvenile spondyloarthritis, biomechanical strain and metabolic causes like osteomalacia/rickets. Skeletal dysplasia is uncommon in general population and metabolic bone diseases usually manifest in childhood. Rare genetic diseases are increasingly identified because of definite genetic studies and inheritance. Mucopolysaccharidosis is one among the metabolic bone diseases where genetic defect causes enzyme deficiency thereby resulting in accumulation of glycosaminoglycans in the cell, accounting for the skeletal and systemic manifestations. Unrecognized patients may mimic as juvenile spondyloarthritis or JIA. HIV infection and ART are included as risk factors for Osteoporosis and fragility fractures by National Osteoporosis Foundation in US. In our country, not many reports are available in this regard. The present article reports two HIV + patients of different age group, on ART for variable periods, who developed bilateral fracture neck femur without any trauma. The cases were managed suitably and are being reported due to its rarity and to stress the need of careful evaluation in HIV+ patients. Case Reports: Case 1: A HIV+ male patient of age 37 years on regular ART presented with pain in his both hips region for 1-year duration without any trauma or fall. X-ray pelvis with both hips showed transcervical minimally displaced fracture neck of both femora. The young patient was managed with multiple cancellous screw fixation in situ and is under regular follow up for 15 months. Case 2: A HIV+ male patient of 65 years under regular ART presented with the complaints of pain right hip with difficulty in walking for 1 month without any trauma. X-ray pelvis showed displaced fracture neck right femur and incomplete fracture of superior cortex of neck left femur. The patient was treated by Austin Moore prosthetic replacement of head femur on right side and observation for the left femur neck incomplete fracture. The case is under follow up and continuing ART also. Clinical Message: Spontaneous nontraumatic fracture should be suspected in HIV + patients having persistent localized pain in an area even without history of any trauma or classical features of fracture on physical examination. They are more common in children than adults. Majority of cases require surgical management especially in adults. Surgically preferred management comprises of closed/open reduction and internal fixation using various types of plates/wires .We have performed Metaizeau technique in seven adults with successful treatment and functional outcome. This technique is very well described in children. Background In adults, isolated radial neck fractures are very rare with incidence of 1% of all fractures. The prognosis depends upon the degree of displacement, the age of the patient, associated elbow injury and the method the treatment. Less than 30 degree of angulation is generally acceptable. Restoration of radial neck angulation and displacement is essential to restore the normal biomechanics and stability of the elbow. Open reduction and internal fixation (ORIF) is associated with non-union, implant related complications, reduced range of motion, heterotrophic bone formation and avascular necrosis of the radial head. The standard procedure accepted for isolated radial neck in pediatric age group is centromedullary pinning (Metaizeau technique). Methodology Between January 2016-January 2017, 7 patients were operated with TENS(Metaizeau technique).Chosen patients were diagnosed with isolated radial neck fractures without elbow joint instability. Fracture morphology in all the patients had angulation more than 30 degrees and were displaced. All the patients had type 3 fracture according to Mason’s classification and type 2 or 3 according to Judet classification. The mean age of the patients was 32 years old and all were operated with 7 days of suffering the injury. The mean followup was for the duration of 9 months. Achieving this remission or very low disease activity has become increasingly possible with USA FDA approved first JAK inhibitor, Tofacitinib in November 2012 (not in Europe). This new small molecule JAK inhibitors are oral drug working intra-cellularly in comparison to TNFα inhibitors targeting a singly cytokine within extra-cellular space confining themselves to cell surface receptors. The second JAK inhibitor Baracitinib approved in Europe in February 2017 and in USA in May 2018 has greater promises. Once daily 2 mg oral dose used either as monotherapy or in combination with methotrexate in moderate to severe active rheumatoid arthritis in adults have provided remission, even in patients who have responded inadequately or are intolerant to one or more DMARDs / bDMARDs. It is a safe second line therapy after methotrexate.
