Pain and inactivity in CLBP patients can lead to tightness and weakness of gluteus maxmimus muscle fibres. Limited data from the studies are available on Saudi military population, hence we intend to know the relationship of gluteus maxmimus insufficiency in relation to CLBP as it contributes larger burden of disability, loss of work days, diminished quality of life and considerable health care cost in military personnel. The objective was to study the relationship of gluteus maximus insufficiency in the patients with chronic mechanical LBP in king Khalid military city Saudi Arabia.\u0000Materials and Methods: A prospective observational study was done on 100 patients with CLBP in king khalid military city hospital. The intensity of pain was measured by visual analog scores, Strength of Gluteus maximus muscle was measured by manual muscle testing, and the length of the muscle was measured by hip extension range of motion (ROM) using goniometer. Khanna","doi":"10.18231/j.ijor.2019.012","DOIUrl":"https://doi.org/10.18231/j.ijor.2019.012","url":null,"abstract":"Introduction: WHO defined Osteoporosis as a bone mineral density (BMD) of 2.5 SD or more below the mean peak bone mass (average of young, healthy adults) as measured by dual-energy X-ray absorptiometry. FRAX is a computer-based algorithm developed by the World Health Organization Collaborating Centre for Metabolic Bone Diseases and first released in 2008. The output of FRAX is the 10-year probability of a major osteoporotic fracture and the 10-year probability of hip fracture. Objective: To assess the outcome of combined bisphosphonate, vitamin D and calcium therapy in patients with established osteoporosis at the end of 15 months, to assess the relationship between vit D, Calcium and osteoporosis with reference to existing FRAX model and to analyze whether any correlation exists between serum Vitamin D levels and BMD as established by DEXA Scan. Materials and Methods: A prospective observational study was performed in 128 patients who underwent screening with Vitamin D levels and DEXA scan to confirm osteoporosis. DEXA scan was employed primarily to establish a case as osteoporosis according to the definition. These patients were started on treatment with combined Bisphosphonates standard dosage of alendronate 70 mgs once per week with standard instructions for 15 months, Vitamin D was administered to patients with established hypovitaminosis as standard therapy with oral sachet of 60,000 I.U administered with milk for 8 weeks. Calcium supplements were given as calcium carbonate 1G per day for 15 months. The final assessment was done by the end of 15 months by analyzing the results of BMD and Vitamin D levels to look for any improvement. Results: In our study of 128 patients we found that, there was statistically significant improvement in mean ± SD Vit D and FRAX hip score at end of study while comparing with pretreatment scores. There was a strong positive correlation between FRAX score hip and FRAX score osteoporosis. Seronegative spondyloarthropathy or seropositive rheumatoid arthritis and their variants once diagnosed should be treated early to prevent the bony erosion. Delay of up to nine months in initiating Disease-Modifying Anti-rheumatic Drug (DMARD) definitely impacts clinical outcome several years later. During the last decade, introduction of biologics like TNF inhibitors have revolutionized the management of rheumatic diseases leading to early and prompt control over disease activity. One of the TNF inhibitors, Etanercept over the time has proven itself to be a reliable option for treatment of rheumatoid arthritis, spondyloarthropathy, psoriatic arthritis and other rheumatic diseases. Major drawback with biologics like Etanercept is their cost. Indian population differs substantially from western population in terms of height, weight, phenotype as well as genotype. Due to this difference in demographic profile and cost-affordability, many drugs have been used in low dose in Indian population. Etanercept can be a safe and effective option in rheumatic diseases even at half its recommended dose in countries like India where the average weight of people is less as compared to western population. Low dose also improves affordability, reduces risk of infections and may be, minimizes need for vaccination. In the modern era of biologicals, biosimilars have a huge potential to treat rheumatic diseases in India where affordability is a real challenge. Thus, we have performed this study to evaluate the outcome of non-patella resurfacing total knee arthroplasty. Materials and Methods: A total of 50 patients in series who came to our institute with clinical signs and symptoms of osteoarthritis confirmed radiologically, were operated. We measured the outcomes with Knee society score and VAS score. Results: There was significant difference in outcome of non-resurfaced patella pre-operatively and post-operatively. There was significant improvement in mean range of motion (ROM) 87.2 pre-op vs 104.4 post-op, KSS pre-op mean 39.66 vs post op mean 83.26. The improvement in functional score were pre-op 52.5 to post-op 83.36. The mean VAS score decreased from 7.98 to 2. Materials and Methods: The study was conducted in department of orthopaedics, S.R.G. Hospital and Medical College, Jhalawar (Rajasthan). The study comprised of 78 patients (48 males and 30 females) with intertrochanteric fractures. Observations and Results: Out of total 78 patients taken in study 61 were found to be hyponatremic. Pain typically comes on rapidly, reaching maximal intensity in less than twelve hours. The first metatarsophalangeal joint is affected in about half of cases. It may also result in tophi, renal stones, or urate nephropathy. Commonly, but very large tophi are unusual in chronic gout. Here we present 52 years male patient presenting with unusually large tophi over bilateral foot, with raised serum uric acid levels. In a untreated cases of chronic gout, patients can develop severe destructive poly articular involvement and tophi. Treatment aims at control of serum uric acid levels which can cause regression of tophi.
