Abbas, Amr M Elsayed","doi":"10.21608/aeji.2022.123517.1208","DOIUrl":"https://doi.org/10.21608/aeji.2022.123517.1208","url":null,"abstract":"Background and study aims: A novel severe acute respiratory syndrome corona virus (SARS COV2) outbreak resulting in corona virus disease (COVID- 19) which is known as a pandemic infectious disease. Chronic hepatitis C virus (HCV) is a well-known major cause of chronic liver disease and its subsequent major complications. Telemedicine refers to the remote practice of medical care for patients when the provider and patient are not physically present with each other. We aim to assess use of telemedicine in the follow up of chronic HCV patients during treatment by direct antiviral therapy in this era of COVID- 19. Patients and Methods: this study included cross matched 300 patients consulted for HCV treatment. 150 patients were treated in 2019 and followed by routine classic visits (routine classical group). Another 150 patients were treated in 2020 and followed up via virtual visits by telemedicine (telemedicine group). Results: There is no significant statistical difference between the two groups regarding the sustained virological response (SVR) and the recorded side effects during treatment (P= 0.275, P= 0.523) respectively. Results: The most common organisms isolated from sputum samples in the present study were E coli (19.39%), Klebsiella spp (15.31%), Staph aureus (14.29%) and Pseudomonas (9.18%) while no growth of organisms observed in 28.58%. The antimicrobial-susceptibility for Enterobacteriaceae species isolated from sputum showed higher sensitivity to Imipenem (88.2%), Piperacillin – tazobactam (73.5%), and Ampicillin (70.59%) while Ceftriaxone , Ceftazidime and Cefotaxime showed higher resistance respectively (64.71%, 58.82% and 55.89%). The antimicrobial-susceptibility results for S.aureus species isolated from sputum showed higher sensitivity to Vancomycin (100%) followed by Oxacillin ( 64.29%) while Penicillin G showed complete resistance 100%, followed by Tetracycline 92.86% ,and Co- trimoxazole 85.71%. Conclusion: Gram-negative bacteria were the cause of bacterial infections in significant proportion of patients with increased sensitivity to B-lactam antibiotics and ampicillin. Results: By histopathological examination, six patients (7.5%) were diagnosed to have EoE. Endoscopic esophageal furrows, fissures/wrinkles; white exudate/granularity and concentric rings (trachealization) were significantly more frequent in patients with EoE than non EoE. Median concentration of serum IL-5 in EoE patients was 222.2pg/ml with range 187.5-307pg/ml while, it was 32.1pg/ml with range 15.6-113.6pg/ml in non EoE patients and 11.8pg/ml with range 13.4-38.2pg/ml in controls with P value < 0.001. ROC curve analysis showed that serum IL-5 can significantly predict EoE at a cut-off value of ≥103.2pg/ml with sensitivity 0.989 and = 0.96-1.0.
