Patients and Methods: The study included diagnosed cases of mycetoma foot which were treated in the facility. Epidemiological data and detailed clinical data were noted in all cases. The discharging materials: purulent exudates with grains (granules) were sent for staining and culture to the microbiology department and microbiological data were noted. All patients were commenced on treatment as per the standard protocol. Results: Our study noted 10 cases of mycetoma. The mean age of presentation was 38.4 years with male preponderance. Most of the cases affected right foot. On microbiological examination, gram-positive narrow filamentous branching bacilli were seen. Surgical debridement was necessitated in all cases except one, and bone debridement was needed in three cases. Conclusion: Delayed diagnosis or inadequate treatment can lead to severe deformities, disabilities, and even limb loss. Results: Prevalence of L. pneumophila in Zagazig university hospitals was 17% detected by L. pneumophila urinary antigen testing (LPUAT), 15% of samples were positive by q RT PCR based mip gene and 12 % positive by respiratory sample cultures. In comparison to respiratory sample culture, the gold standard, q RT PCR evaluation showed sensitivity 83.3%, specificity 94.3%, positive predictive value 66.7%, Negative predictive value 97.6% and accuracy of 93.0%. Based on the current study, predictors of failed cannulation of major papilla were endoscopists experience < 5 years and malignant obstruction. Conclusion: ERCP still has some sort of difficulty during canulation. Early prediction of those patients who are vulnerable to failure of cannulation would have a great effect on their outcome with a reduction in the frequency of expected complications. Alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin, direct bilirubin and international normalized ratio (INR) after infection were significantly higher than before infection in both groups (p <0.001). Group 2 had significantly higher mortality rate (47%) than group 1 (32%) (P= 0.02). MELD score was significantly higher in died patients than recovered patients in group 2 (p <0.001). After COVID-19 infection, hepatic encephalopathy (HE) and aggravation of ascites was found in 34% and 36% of patients in group 2.
