Little is known about shift work in Pakistan, especially in the Peshawar region. Implication for individual Nurses’ need to care for their own health as well as with patient care. The objective of this study was to identify the effects of shift work on the health of nurses working in public and private hospitals of Peshawar.\u0000Methods: The study design was cross-sectional study. In May 2017, 199 nurses participated in this study by completing a self-administered questionnaire which comprised of demographic data health outcomes after taking consent from the participants in 2 public and 1 private hospital in Peshawar.\u0000Results: The study found that Rotating shifts affect nurses physically as well as psychologically; there is a positive association between shift work and health problems such as sleep disturbances and sleep-related impairment, fatigue, anger, satisfaction with social roles outside of work, and lower cognitive abilities.\u0000Conclusion: Shift work is essential for any organization to provide 24 hours services to the patient. Farooq, Mudassir Abbasi, Ayesha Ayaz, Nadia Rehman Alvi, Sarah Ali","doi":"10.32413/pjph.v12i2.920","DOIUrl":"https://doi.org/10.32413/pjph.v12i2.920","url":null,"abstract":"Background: The main purpose of this study is to evaluate the oral health of diabetic patients through the application of Knowledge, Attitude and Practices (KAP), since periodontitis considered to be one of the complications of diabetes.\u0000Methods: This cross-sectional study was conducted at THQ hospital, Kahuta district Rawalpindi, where 104 diabetic patients were selected as study participants. Data was collected by using a structured questionnaire and further analysis was done by using SPSS 21.\u0000Results: The study showed that only 38.5% of the participants had adequate knowledge regarding oral manifestations of diabetes. The oral health attitudes and compliance of suggested oral cleanliness practices were also poor. The study results showed that 55% of the diabetic patients brushed their teeth once a day. While only 16.3% brushed twice a day. 39.4% of the participants did not visit the dental clinic within the last 1 year.\u0000Conclusion: The study concluded that knowledge about the oral complications was deficient in the diabetic patients. Cross infection can be defined as “the spread of infection between staff members & patients in clinical setting.” Precautions should be adopted while treating or checking the patients.\u0000Methods: This cross-sectional study of 3 months duration was carried on dental house officers of Multan Medical and dental college Multan. 60 dental house officers willingly participated in our study. Universal sampling technique was utilized in order to collect the data. Informed consent was sign up from the partakers.\u0000Results: Out of sixty dental house officers, males were twenty seven while females were thirty three . 85% participants consider that dental clinics/hospitals are more predisposed to the infection/contamination than other medical fields. 73.3% partakers wash the hands regularly after providing the treatment to every patient. All the partakers reported that usage of PPE has a foremost part in the prevention of cross contamination. Majority of the partakers use gloves as well as facemask but goggles or protective eyewear, head caps & aprons were not in use by most of participants.\u0000Conclusion: Adequate awareness/knowledge but less satisfactory practice about infection control was reported. Knowledge acquired must also be transferred into everyday practice. Continuous infection control education through lectures/ tutorials or seminars must be organized. The purpose of this study is to determine the workload of physicians working in various departments of a tertiary healthcare centre, as well as the link between physician job load and specialty.\u0000Methods: This Cross-Sectional study was carried out from October 2020 to November 2020 at KRL Hospital Islamabad. Hospital records were used to gather information about the number of patients. Four parameters, such as mental demand, physical demand, time and effort demands were evaluated.\u0000Results: Gynecology had the highest patient load (23 percent), followed by pediatrics (21%) and surgery (14 %). Physically and mentally taxing specialties were urology and maxillofacial surgery. Psychiatry had the lowest demands in terms of mental, physical, time, and effort.\u0000Conclusion: Our study shows that Urology and Internal Medicine are more time-consuming and stressful for the physician to perform. Eman","doi":"10.32413/pjph.v12i1.961","DOIUrl":"https://doi.org/10.32413/pjph.v12i1.961","url":null,"abstract":"This narrative review of the public private mix (PPM) TB plan by comprehensively evaluated the performance of this initiative in case reporting, management, treatment outcome, affordability, and accessibility from the district health system data to the TB Control Programs. Comprehensive literature review was done from the peer-reviewed journals; reports including global and national TB control programs along with technical guidelines on PPM were reviewed.\u0000This paper focusses on PPM model in the TB case notification since its establishment in Pakistan based on the data reported by national TB control mechanisms by reviewing the involvement of and TB case contribution by private health care providers, active case finding through chest camps, TB case finding through enhanced case findings in large private hospitals, and case finding through lady health workers. PPM is a promising strategy for TB care and control and scaling up of PPM should include financial support in addition to material inputs. Moreover, improving program governance and training for the health care providers with the help of integrated collaborative mechanisms is warranted. Haq","doi":"10.32413/pjph.v12i1.955","DOIUrl":"https://doi.org/10.32413/pjph.v12i1.955","url":null,"abstract":"This paper reviews the overarching strategies, implementation rigour, achievements, strengths and weaknesses, and challenges and opportunities faced by Pakistan’s National Tuberculosis Control Program (NTP) during the period 2011-2020. NTP’s annual reports, Global Tuberculosis (TB) Report (2019 & 2020), peer-reviewed journal articles, NTP and NSP plans, along with voluminous programmatic data reviewed. Pakistan’s national and provincial tuberculosis control program has treated around four million people and gained more than 90% treatment success. Iterative planning, partnership with the private sector, strategic advocacy, communication, social mobilization, operational research, and increasing domestic funding are essential to improving case notification and treatment success. Lack of adequate political commitment, over-and under-reporting, lack of a systematic mechanism for sputum transport, and inefficient coverage from the private sector are the main areas for improvement. Local and national strategic planning in funding, program development, and implementation is imperative from a multisectoral perspective for ending TB. Memon","doi":"10.32413/pjph.v12i1.974","DOIUrl":"https://doi.org/10.32413/pjph.v12i1.974","url":null,"abstract":"This is an opinion paper on the process leading to the initiation of a model multisectoral accountability framework (MAF) project in the coastal district of Sindh. Dopasi Foundation in collaboration with Stop TB Partnership Pakistan conducted several meetings with senior policymakers of Sindh province drawing their attention towards the international commitment for establishing a Multisectoral Accountability Framework (MAF) to control TB. Thereafter, operational meetings were held with key government functionaries and district Badin. Subsequently the first sensitization meeting of stakeholders comprising of the district MAF team was convened and a problem analysis encompassing the current case notification, intersectoral challenges were drawn from the discussion these includes; likely benefits, zakat provision, mapping of community services, augmenting the role of drug inspectors, sputum transport and various dimensions related to the control and elimination of TB was conducted. This problem analysis, MAF implemented in the district and found effective in case detection likewise; during 2020, 2,519 TB cases were notified as compared to 2,464 in 2019, depicting a nominal increase, despite an overall decline of 17% in TB notifications owing to COVID-19 nationwide, while promoting self-reliance, pooling of resources and comparative advantages of each partner. This extremely cost-effective exercise based on WHO principles showed promising results, although the COVID-19 related disruption rendered a proper program evaluation impracticable.
