Prevention of the first exacerbation of chronic obstructive pulmonary disease and alleviation of exacerbations are key goals in the treatment of the disease. Due to the fact that immune mechanisms occupy the central place in the pathogenesis of chronic obstructive pulmonary disease, the aim of the study was to study immunological parameters in patients with chronic obstructive pulmonary disease at the beginning of exacerbation and in the period of exacerbation termination, as well as the way how changes in immunological indicators affect the dynamics of pulmonary functional parameters in the process of treating patients with exacerbation of chronic obstructive pulmonary disease. The study included 116 patients hospitalized in the pulmonology department due to the first exacerbation of chronic obstructive pulmonary disease. In addition to the standard set of studies, patients underwent lymphocyte phenotyping using CD3, CD4, CD8, CD20 monoclonal antibodies, evaluation of neutrophils' phagocytic activity in a latex test, determination of immunoglobulins – IgG, IgA, IgM concentrations, circulating immune complexes and cytokines (interleukin (IL)-1β, IL-4, IL-8, interferon-γ (IFN-γ) in the serum. The above-described range of studies was carried out on the 2nd day of inpatient treatment and on the 10th–12th days of treatment. The results of the study showed that COPD exacerbation is associated with quantitative insufficiency of the cellular link in adaptive immunity against the background of activation of its humoral link. The features of the cytokine profile in patients with chronic obstructive pulmonary disease in exacerbation stage testified to the activated production of proinflammatory cytokines (IL-1ß, IL-8) and the main cytokines associated with the activation of humoral (IL-4) and cellular (IFN-γ) components of adaptive immunity. At this, the function of the cellular link was activated to a greater extent, as indicated by an increase in the ratio of IFN-γ/ IL-4. Analysis of indicators of external respiration function enabled us to establish that all speed indicators in patients were lower than in healthy ones. A retrospective independent continuous blind study of ultrasound examination protocols was conducted in 665 patients, 241 patients had benign non-cancerous thyroid diseases, 86 patients had benign tumors (adenomas), 338 patients had thyroid cancer. During the preoperative examination, all patients underwent a multiparametric ultrasound examination of the neck organs according to the standard procedure, with the registration of identified thyroid nodes, with examining the prognostic capabilities of stratification systems – TIRADS, EU-TIRADS, TLA_RU. It was found that the TIRADS system has a sensitivity of 91.04%, specificity – 91.41%. Focusing on the scale of AUC values reflecting the quality of the diagnostic test, it can be stated that TIRADS is a test with excellent quality (AUC =0.972). Basing on the results of the analyzing the data obtained by the EU-TIRADS system, it was revealed that it is a high-quality test (AUC=0.826), but its predictive capabilities are worse than those of TIRADS. The original TLA_RU model has 87.5% sensitivity and 95.7% specificity. During the ROC analysis, it was found that the AUC is equal to 0.954±0.00894, which suggests that the TLA_RU model is an excellent quality test in the differential diagnosis of thyroid nodular masses. The review presents the main methods for determining the centric jaw ratio using deprogramming of masticatory muscles, as well as indications for the use of these methods. In complete adentia, the most acceptable are the method of non-forced control of the mandibular movements, the Dawson method, the Schiler method, the method of recording the Gothic angle, transcutaneous electroneurostimulation. The above-listed methods can be used in partial adentia as well, and in the presence of all teeth in the oral cavity. In the pathology of the temporomandibular joint, deprogramming methods using the Lucia jig, a leaf gauge, Sliding Guide, as well as the method of recording the Gothic angle will also perform a diagnostic function. With the help of these methods, it is possible to change the design of long-wearing deprogrammers and medical devices. The majority of patients (34%) were hospitalized in the department with multisystem and concomitant injuries. The remaining patients were taken to the medical institution with different diagnoses (urolithiasis, liver cirrhosis, pancreatic lesion of various types, poisoning, peptic ulcer, sepsis). The data of coagulogram and thromboelastography at different stages of treatment were compared. In patients with the development of traumatic shock, the coagulogram parameters were changed to varying degrees depending on the stage of shock. At the first stage of shock, only an increase in soluble fibrin-monomer complexes by almost 2 times and a slight increase in fibrinogen dynamics were noted in the analysis. In a patient with stage 3 traumatic shock, the coagulogram parameters were within the normal range, but according to thromboelastography (EXTEM and FIBTEM tests), hypocoagulation due to the platelet link was noted. Only the coagulogram was evaluated in dynamics, hypocoagulation was noted in the indicators of internal and external hemostasis pathways: lengthening of the activated partial thromboplastin time, a decrease in the prothrombin index and an increase in the international normalized ratio, an increase in fibrinogen A and soluble fibrin-monomer complexes. In the group of male patients with closed craniocerebral trauma, an increase in soluble fibrin-monomer complexes in the coagulogram was always combined with changes in the FIBTEM test during thromboelastography. In most patients, no changes in the classical coagulogram tests immediately after the injury are noted. All segments of the population are sensitive to it, regardless of gender, age, belonging to various social groups. This virus creates certain difficulties in detecting and treating the disease. It is associated with the risks of developing critical clinical conditions of infected people. There are several versions regarding the origin of the coronavirus. The first theory is the natural origin of the virus. The second theory is the artificial origin of the virus. Currently, the evidence for both the artificial origin of the coronavirus and its natural origin is indirect, so it is impossible to reject both hypotheses yet. So, in Russia, currently, women begin to bring their reproductive function into action on average at the age of 26-35. This leads to the fact that obstetricians and gynecologists in their practice increasingly face with pregnancy and childbirth complicated by a uterine scar after a previous cesarean section or myomectomy. The formation of a uterine scar after any intervention entails the likelihood of long-term complications, such as a rupture of the uterus along the scar and pregnancy in the uterine scar. According to the latest clinical recommendations for ectopic pregnancy, pregnancy in the uterine scar has been added to the anatomical classification of ectopic pregnancies. The article describes a clinical case of an undeveloped pregnancy in the uterine scar after a cesarean section, it shows the difficulty of timely diagnosis of such pregnancy, which often results in improper management of patients and loss of fertility by women of childbearing age. Vaccination against COVID-19 with ChAdOx1 nCoV-19 (AstraZeneca) in a number of patients turned out to cause thrombocytopenia and thrombi formation in atypical (cerebral venous sinus, portal, abdominal, and hepatic veins) and typical (deep femoral and sural veins, pulmonary artery thromboembolia, acute arterial thrombosis) places. And the use of mRNA vaccines (Moderna and Pfizer) was sometimes accompanied by thrombocytopenia and hemorrhage, but without thrombi formation. This circumstance gave rise to the search for mechanisms of thrombi formation when using previously never-used vaccines developed against COVID-19. The purpose of the paper is to inform the medical community about the mechanisms of thrombi formation in COVID-19; to discuss possible pathogenetic ways of rapid amyloid formation and amyloidogenic stimulation of the coagulation hemostasis system. The only study completed to date provides information on the launch of rapid amyloid formation with the formation of dense large fibrin clots in the whole blood of both healthy people and those who were in the acute period of COVID‑19 disease by the spike protein of the COVID-19 virus capsid. The authors, having discovered the fact of the direct influence of spike protein on the formation of blood clots, nevertheless did not investigate possible pathogenetic ways of triggering thrombi formation by the spike protein. Since the authors directly pointed out the role of rapid amyloid formation in triggering coagulation, the mechanism of which is unknown to practitioners, it makes sense to discuss the issues of rapid amyloid formation in the vascular bed and the role of amyloid as a factor in triggering coagulation hemostasis. The publication under discussion is confirmed by previous studies of other authors on the influence of b‑amyloid and AA‑amyloid on the formation of blood clots in Alzheimer's disease and systemic amyloidosis. A promising method of therapy for myofascial pain syndrome (MFBS) is the photobiomodulation method (FBM), which uses non-ionizing light sources (lasers, LEDs) and radiation in the visible, infrared and near-infrared spectral ranges for therapeutic purposes. The review briefly discusses the general understanding of FBM and the results of studies on the effectiveness of FBM in MFBS, it discusses possible mechanisms of the therapeutic effect of low-intensity optical radiation and the need to choose the optimal exposure parameters. The method of eye tracking (ET) opens up great opportunities for studying social attention in this contingent of people with disabilities. The aim of the study is to form a set of stimulus materials for dynamic ET-testing of eye movement patterns that are sensitive to changes in the level of social attention in preschool-age children with ASD. The selection of stimulus content included the definition of requirements for the quality of the images presented, storage and encoding of stimuli, the development of the procedure for presenting stimuli and the methodology for processing the ET-data. The stimulus material was tested on a group of healthy adults. Thirty-two patients aged 19 to 52 years underwent surgical treatment for clinically proven chronic tonsillitis. The aim of the study was to detect the fact of amyloid tonsillar lesions formation in chronic tonsillitis. We performed histological examination of tonsillo-ectomized material fixed in 10% formalin solution and filled in paraffin on 4-micron histological sections: 1) according to the conventional technique, stained with hematoxylin-eosin, 2) Congo red staining and 3) using our own original trichromatic method (two-hour incubation of paraffin sections in acidin-pepsin, sequential staining with picro-fuchsin according to Van Gieson, Сaracсi hematoxylin, Congo red according to H. H. Benhold, picro-indigocarmine). All 32 patients showed a clinical presentation of chronic tonsillitis without exacerbation with stroma sclerosis and atrophy of the lymphatic glandular system. Local amyloidosis of the tonsils was detected in 10 out of 32 cases. As a result of trichromatic staining, unchanged connective tissue fibers stained green, changed tissues (hyalinosis or amyloidosis) stained in various shades from yellow, yellow-orange to red, in addition, lymphocytes, neutrophils and mast cells were clearly detected in the tissues of the amygdala. For comparison, a section of the palatine tonsil was taken from a 49-year-old man who committed suicide by hanging, who had no chronic tonsillitis. Forensic diagnosis: mechanical asphyxia by hanging. Macroscopically: the tonsils are not enlarged, folded, the size of one of them together with the soft tissues was 2.5×1.5×1 cm. It is concluded that a significant number of amyloidosis cases that did not lead to severe damage to the vital organs remain unaccounted for due to the lack of specialists' awareness and, as a result, recommendations for screening for the presence of amyloidosis. At least 30% of patients with chronic tonsillitis are observed to have at least local tonsillar amyloidosis.
