ABSTRACT. Introduction: lung cancer (LC) is usually diagnosed late. Understanding how patients present at the time of their diagnosis it is useful for early identification of the disease. Objective: to identify the clinical profile of patients with lung cancer, allowing to suspect the diagnosis and to propose an algorithm for early referral. Material and methods: prospective case series of lung cancer. The general characteristics, smoking, exposure to wood smoke, time from symptom onset to diagnosis and the type and frequency of symptoms were studied and their association with pleural effusion was analyzed. Results: the median age was 65 years, 55% were men. History of smoking 50.2%, exposure to wood smoke 43.9%. The median time from symptom onset to diagnosis was 120 days and in 94% it was more than 3 weeks. The most frequent symptoms were cough 88%, cough for 3 weeks or more 94.6%, dyspnea 75.2%, chest pain 50%, weight loss 70%, and hemoptysis 22%. Malignant pleural effusion 44.9%. Dyspnea, chest pain and oxygen saturation < 88% simultaneously were associated with pleural effusion OR (95% CI) 7.54 (3.28-17.34). Pulmonary transplantation is a treatment that involves mastery of a complex surgical technique, for the specialist in training in chest surgery. One modality of preserving the lung before being transplanted is the ex vivo technique. Material and methods : In order to train the resident in this area, a biological model of pig was used to obtain the heart lung block and its connection to the living system. Results: 20 lung heart block procurements were carried out and connected to the lively system without complications, leaving them connected for two hours to assess, that the technique was correct. This significant learning modality, with a process of situated cognition, helps to improve the learning of residents in training. El consumo de tabaco tiene consecuencias adversas en la salud cardiovascular y representa un problema de salud pública a nivel mundial que demanda atención integral. Se realizó una revisión sistemática en PubMed de la literatura concerniente al impacto del tabaquismo sobre la salud cardiovascular reportada en metaanálisis publicados en los últimos 10 años. Doce estudios determinaron una asociación significativa entre el tabaquismo y la incidencia de enfermedades cardiovasculares, de los cuales seis demostraron una dismi nución importante de la morbimortalidad tras la implementación de políticas para el control del tabaco. Es necesario fortalecer las acciones de los gobiernos y los profesionales de la salud para disminuir la prevalencia de consumo de tabaco y proteger a la población general de las enfermedades asociadas con este producto, que afectan a todo el cuerpo y que tienden a generar enfermos con múltiples comorbilidades que se tienen que investigar. ABSTRACT. Cigarette smoking has hazardous effects on cardiovascular health and represents a worldwide great concern that demands further attention. We conducted a systematic review using PubMed database of the literature concerning cardiovascular effects of cigarette smoking reported in meta-analyses published in the last 10 years. 12 studies determined a significant association between cigarette smoking and the incidence of cardiovascular diseases, 6 of them demonstrated a substantial decline in the morbimortality after tobacco control policies were implemented. Introduction: The elderly may represent a specific group of patients at high risk for developing COVID-19 with rapidly progressive clinical deterioration. In fact, in older individuals, immunosenescence and comorbid disorders are more likely to promote a virus-induced cytokine storm resulting in life-threatening respiratory failure and multi- system compromise. Material and methods: A descriptive study of the geriatric population with COVID-19 disease attended at the National Institute of Respiratory Diseases, Mexico City, Mexico in the period March-May 2020 was carried out. 35 hospitalized adult patients over 60 years were confirmed by RT PCR. for COVID-19 disease in pharyngeal exudate. Descriptive statistics were used and results were expressed using measures of central tendency. Results: A total sample of 35 patients was analyzed. 62.85% were male (n = 22). Systemic arterial hypertension was documented in 51.42% (n = 18), obesity in 48.57% (n = 17) and diabetes mellitus in 34.28% (n = 12). The severity of the COVID-19 disease at the time of hospital admission was: mild in 22.85% (n = 8), moderate in 42.85% (n = 15) and severe or critical in 34.28% (n = 12). 80% (n = 28) consulted after presenting symptoms for more than five days. The tomographic findings were ground glass pattern in 82.85% (n = 29), consolidation in 14.28% (n = 5) and «crazy paving» in only one patient (2.8%). The mortality registered in geriatric patients with COVID-19 was 68.57%. Conclusions: The geriatric population is considered vulnerable for the development of COVID-19 disease and for fatal outcomes. Patients with moderate disease should be closely monitored as they tend to eventually develop critical forms. The speed with which telemedicine services have been implemented globally has been very different. There are still great challenges to be solved in order to provide this type of care worldwide. Introducción: La pandemia por SARS-CoV-2 es un importante problema de salud pública a nivel mundial, con altas tasas de morbimortalidad. La tomografía computarizada es fundamental en el proceso diagnóstico dada su alta sensibilidad. Objetivo: Describir los hallazgos tomográficos en la afectación pulmonar por COVID-19. Material y métodos: Estudio transversal analítico. Pacientes atendidos en el Instituto Nacional de Enfermedades Respiratorias, Ciudad de México, con definición operacional vigente de caso sospechoso para COVID-19, que contaran con prueba de RT-PCR específica y tomografía computarizada de tórax en la valoración inicial. Se realizó un análisis descriptivo y analítico mediante la prueba de χ2 y t de Student. Se empleó el programa Epi-Info versión 7. Resultados: Se analizaron 56 pacientes, con una edad promedio de 51 años, 61% fue del sexo masculino. 52% presentaron comorbilidades, siendo la diabetes mellitus la más frecuente. Los síntomas que principalmente se observaron fueron fiebre, tos y cefalea. El patrón tomográfico que predominó fue mixto, con localización subpleural y bilateral. Introducción: En los últimos años las infecciones ocasionadas por micobacterias no tuberculosas han tenido un incremento notable en varias regiones del mundo, asociado fundamentalmente a infecciones respiratorias en individuos inmunocomprometidos. La identificación de micobacterias no tuberculosas es engorrosa y puede demorar varias semanas. Objetivo: Aplicar las técnicas moleculares GenoType Mycobacterium CM/AS para la identificación de micobacterias no tuberculosas en Cuba y comparar con los resultados de la prueba convencional. Material y métodos: Se utilizaron 73 aislados de micobacterias no tuberculosas procedentes de muestras pulmonares recibidas de unidades de salud ubicadas en varias provincias de Cuba en el período abril 2017‐diciembre 2018. Para la identificación de especies se utilizaron técnicas conven‐ cionales y las técnicas moleculares GenoType CM/AS. Resultados: Las especies identificadas más predominantes fueron las pertenecientes al complejo Mycobacterium avium-intracellulare (46.6%) y Mycobacterium fortuitum (31.5%). Se hallaron cuatro aislados de M. simiae, especie poco frecuente en Cuba. Al comparar los resultados de ambas técnicas se obtuvo una concordancia de 91.3%. Conclusión: Las técnicas Geno‐ Type Mycobacterium CM/AS demostraron ser herramientas útiles en la identificación rápida de micobacterias en el Laboratorio de Referencia, brindando resultados precisos en menor tiempo, logrando acortar sig‐ nificativamente el tiempo diagnóstico.
