{"title":"INVESTIGATION OF THE IMPACTS OF ECONOMIC CRISES ON THE HEALTH SYSTEM IN TURKEY: AN ARDL BOUNDS TESTING APPROACH","authors":"Ümit Çirakli","doi":"10.31201/IJHMT.819982","DOIUrl":"https://doi.org/10.31201/IJHMT.819982","url":null,"abstract":"In this study, the impacts of 1994, 2001 and 2009 economic crises on 9 indicators related to the health system in Turkey between 1974-2015 were investigated through the ARDL Cointegration approach. It was found that the 1994 economic crisis affected the total number of other health personnel significantly (p<0.05) in a negative way. The 2001 economic crisis was found to have a significantly negative effect on public health expenditures, total health expenditures and number of beds. For the 2009 economic crisis, private and total health expenditures decreased significantly (p<0,05), while public health expenditures increased significantly (p<0.05). In this study, data collected by TurksStat, Turkish Statistical Institute, through Turkey Health Survey which was applied across the country was analyzed. This survey was carried out every two years, for the first time in 2008 and the last time in 2016. The aim of this study is to examine the level of access to health services in terms of some socio-demographic variables such as gender, age, education, health status and income in Turkey between the years 2008-2016. The results obtained with the analysis show that all variables differ according to years and they are statistically significant. According to the analysis conducted as part of the study, it was concluded that, in Turkey, the status of income and health insurance do not constitute a risk factor on the access to health services. It is thought that the presence of a social security system involving the whole country has an effect on this result. Considering from this point of view, in countries that do not have a strong social security system, out-of-pocket health spending and private health insurance increase the inequalities among the social segments. Therefore, private health insurances and income systems to reduce dependence on out-of-pocket health spending should be developed, and access to health services should not be an indicator of income. Adongo","doi":"10.31201/ijhmt.740557","DOIUrl":"https://doi.org/10.31201/ijhmt.740557","url":null,"abstract":"This study primarily focused on “medical tourists”, that is, patients who were referred or travelled from other districts/regions of Ghana and/or other countries to receive medical treatments at the Tamale Teaching Hospital (TTH). A total of 120 patients who were referred from other health facilities came seeking healthcare from 10 surveyed departments. These medical tourists were contacted over a one month period through structured questionnaire. Information was also sourced pertaining to the most visited departments and units in the hospital and the human resource capacity of the facility. Data relevant to this study about TTH were collated from management through in-depth interview (IDIs) schedules. The study revealed some departments in the facility with complements of qualified staff were heavily patronized. The main traffic to the facility came from the Upper East and Upper West Regions including other districts within the northern region. The facility had also hosted patients from other countries such as Togo and Burkina Faso on the continental front and the USA, UK and Cuba on the foreign arena. Questionnaire method was used to collect the research data. ANOVA and Pearson Correlation Analysis were used to analyze the data. As a result of the research, it was determined that the general adequacy perceptions of medical tourists for the treatment services they received within the scope of health tourism and the general satisfaction levels of the medical tourism services showed a significant difference according to the monthly total income, insurance status and nationality. If there was any difference, which direction it effects and what kind of health care it directs individuals toward. A questionary was created by author depending on literature review. There were questions about participants’ socio-demographic, economic and health status and the usage of their health services in the last year. The sample was consisted of 852 people, 459 of whom do not have private health insurance and 393 of whom have private health insurance. The data was analyzed with SPSS.v2 package program. Non-parametric tests such as Mann Whitney U test and Chi square were used for the analyzing. Significant differences in terms of outpatient treatment, surgical procedure, vaccination, dentist visits, glasses-contact lenses supply and family health center (FHC) visits were found between the groups. It was found that individuals who had private health insurance tend to use therapeutic health services rather than preventive health services.
