Its novelty was that based on the studies of the latest 3 years the LDL-cholesterol target level had been decreased. Being aware of the present daily practice for the attainment of the new goals more attention has to be paid by the doctors and would require a better adherence from patients. It has to be underlined that the achievement of 1.4 mmol/l LDL-cholesterol level or that of 1.0 mmol/l in the case of recurrent cardiovascular event within 2 years is a great challenge for the statin and combined statin+ezetimibe therapy as well. Beside the more frequent administration of the ezetimibe the use of PCSK9-inhibitors would be needed. {"title":"Adatok és gondolatok a szívinfarktusos betegek rövid és hosszú távú prognózisáról, az intézeti rehabilitáció hazai helyzetéről","authors":"A. A revaszkularizációs beavatkozások gyakorisága az ST-elevációval járó infarktus (STEMI) esetén 80,6%, ST-elevációval nem járó szívinfarktus (NSTEMI) 44,6% volt. Az észt, norvég, svéd adatokkal összehasonlítva STEMI-infarktusban a primer PCI aránya Magyarországon volt a leggyakoribb, míg az NSTEMI-betegcsoportban elvégzett PCI tekintetében a svéd gyakorlat volt a legoptimálisabb, ott ez a beavatkozás a betegek több mint a felénél megtörtént (55,3%). A másodlagos prevenció szempontjából fontos gyógyszeres kezelést Svédországban és Magyarországon a legtöbb beteg megkapta, Észtországban és Norvégiában ezen kezelések javallata több készítmény esetén elmaradt az optimális értéktől. Az életkorral korrigált 30 napos és 1 éves halálozási adatok Svédországban voltak a legkedvezőbbek (8,8%, illetve 13,1%), a hazai adatok pedig a legkedvezőtlenebbek (15,2%, illetve 23,3%). Magyarországon az infarktust túlélt betegek egyharmadánál kerül sor intézeti rehabilitációra, férfiaknál ez az arány kissé magasabb, mint a nőknél (32,2% vs. 34,7%). Az intézeti rehabilitáció aránya jelentősen összefügg a beteg lakóhelyével: egyes megyékben a betegek több mint felénél sor kerül a rehabilitációra, máshol ez az arány még az 5%-ot sem éri el. {"title":"Súlyos szívelégtelenség hátterében álló transthyretin-amyloidosis elkésett diagnózisa","authors":"V. Iványi, Róbert Sepp","doi":"10.26430/chungarica.2021.51.2.131","DOIUrl":"https://doi.org/10.26430/chungarica.2021.51.2.131","url":null,"abstract":"Several heart muscle diseases can cause heart failure, the etiological diagnosis of which may serve the basis of targeted, tailored, effective personalized therapy. Such a heart muscle disease is transthyretin amyloidosis (ATTR) which is an infiltrative disorder affecting most frequently the heart and peripheral nerves, due to the accumulation of transthyretin protein. Deposition of mutant transthyretin, due to mutations in the TTR gene encoding for transthyretin, occurs in the familial form of the disease (hATTR), while in senile amyloidosis, wild-type transthyretin accumulates (wtATTR). We report on a 79-years-old male patient who presented 9 years before because of novel onset atrial fibrillation. Echocardiography at that time showed severe concentric left ventricular hypertrophy which progressed through the years. Laboratory values displayed increased liver enzymes and troponin T levels. He was hospitalized several times because of left- and later right-sided heart failure, with increasing tendency towards hypotension. At last admission he was admitted to us after suffering syncope at home necessitating cardiopulmonary resuscitation. Transthoracic echocardiography showed severe, concentric biventricular hypertrophy (LV wall thickness 22 mm, RV wall thickness 12 mm), with depressed LV and RV function, apical sparing, and low cardiac output. Taking low voltage, seen on ECG, and other anamnestic data into consideration, the suspicion of ATTR amyloidosis was raised. The patient died after a short observational period due to intractable heart failure. Autopsy and histology revealed systemic amyloidosis, affecting predominantly the heart which was interpreted as wtATTR according to immunohistochemistry, negative TTR gene sequencing and the lack of monoclonal gammopathy. Our case illustrates the importance of the diagnostic delay which is very frequently encountered in TTR amyloidosis. During the search for improved efficacy of pulmonary vein isolation (PVI) interesting topics, such as first-line cryoballoon ablation, use of visualizable steerable sheaths, ablation on the intervenous carina, high-power and very high-power short-duration ablation techniques were investigated. We also learned important lessons from studies investigating the relationship between left atrial anatomy assessed by cardiac computed tomography (CT) and acute/long-term outcomes of PVI procedures. In the field of ablation of supraventricular tachycardias (SVT) researchers demonstrated better outcomes with prophylactic ablation of the peri-incisional isthmus in postoperative atrial flutters, limitations of ventricular pacing maneuvers in paroxysmal SVT diagnostics, the superiority of intracardiac echocardiography (ICE) guided slow pathway ablation, and the feasibility and safety of zero/minimal fluoroscopy approach. Detailed baseline characteristics of patients from the BUDAPEST-CRT Upgrade Trial (NCT02270840.) were reported. Regarding cardiac resynchronization therapy interesting details were also published about reverse electrical remodeling, and the impact of premature ventricular contractions on atrial remodeling. We got valuable data on the comparative efficacy and safety of laser vs. mechanical lead extraction sheaths. In the field of genetics of cardiac arrhythmias, a robust systematic review was performed and published by a Hungarian multicenter collaboration about the different phenotypic and genotypic manifestations of the CACNA1C gene mutations. It is known that obesity is associated with several diseases and metabolic abnormalities – such as hyperinsulinemia, insulin resistance, type 2 diabetes mellitus, dyslipidemia, cardiovascular diseases and cancer – and may predispose to various respiratory, endocrine and immune disorders. SARS-CoV-2 infection can range from asymptomatic or mild respiratory disease to severe, life-threatening pneumonia. It has been observed that COVID-19 infection is more often associated with poor prognosis in obese individuals. Respiratory failure was more common, mechanical ventilation was required at a higher rate, and mortality increased significantly in obesity. After examining the cause of this phenomenon, several studies have suggested that the overexpression of the RAAS system and the angiotensin converting enzyme 2 (ACE2) receptor in the obese, that is required for SARS-CoV-2 virus to enter the cell, may be an important pathogenic factor. Furthermore, the endothelial dysfunction, the hypercoagulability and the chronic low-grade inflammation associated with obesity may also contribute to a more severe prognosis of COVID-19 infection. Between 2017–2019 the implantation numbers have remained on a steady state level, the numbers of ILR, CRT-D and conventional ICDs have shown a slight increase, moreover the ratio of CRT-P against CRT-D has decreased. Year of 2020 was a special year due to the SARS-CoV2 pandemic, since the primary implantations without urgent indication have been limited and been postponed. In the last 2 years close speciality cooperation between the National Health Fund and the Board of the Hungarian Society of Cardiology Working Group of Cardiac Pacing and Electrophysiology established the reimbursement of implantation of leadless pacemakers and subcutaneous ICDs. Az eddigi eredmények arra utalnak, hogy a pozitív pszichológiai tényezők független módon kapcsolódnak a kardiovaszkuláris előnyökhöz, túl a negatív állapotok egyszerű hiányán. Célkitűzés: Pilot vizsgálatunkban szívinfarktuson átesett személyek és egészséges kontrollszemélyek mintáján elemeztük az élettel való elégedettség, a pszichológiai jóllét, a társas támogatás, és a reziliencia szintjét, összehasonlítva a rizikótényezők közül a stressz-szel és a szorongással. A mintában összesen 60 fő vett részt, csoportonként 30-30 fő (átlagéletkor: 55 év). A betegcsoport papír-ceruza formában, a kontrollcsoport online töltötte ki a kérdőíveket. Eredmények: Az élettel való elégedettség, a pszichológiai jóllét és a multidimenzionális társas támogatás barátok alskálája esetében a szívbetegek alacsonyabb értéket mutattak a kontrollcsoporthoz viszonyítva. A társas támogatás mértéke mindkét vizsgálati csoportban negatív kapcsolatban állt a szorongással és az észlelt stressz-szinttel.
