Merkely","doi":"10.26430/chungarica.2022.52.3.208","DOIUrl":"https://doi.org/10.26430/chungarica.2022.52.3.208","url":null,"abstract":"A kardiovaszkuláris megbetegedéseken belül a krónikus szívelégtelenség az egyik vezető oka az időskori morbiditásnak, mortalitásnak és hospitalizációnak. Számos randomizált, kontrollált vizsgálat (RCT) igazolta, hogy a kardiális reszinkronizációs terápia (CRT) hatékonyan csökkenti a morbiditást és a mortalitást megfelelően szelektált betegekben. Azonban a legtöbb RCT-ben az idős populáció alulreprezentált volt, ezért az idősekben kialakult, a CRT-re adott válaszkészség leírására ezen vizsgálatok kevésbé alkalmasak. Ugyanakkor a népesség elöregedésével emelkedik a szívelégtelenség prevalenciája, valamint a fejlődő terápiás lehetőségeknek köszönhetően növekszik a betegek várható élettartama, aminek következtében emelkedik az idős, 70–75 év feletti szívelégtelen betegek száma. Fontos kérdés tehát, hogy az életkor negatívan befolyásolja-e a CRT-re adott válaszkészséget. Célul tűztük ki, hogy a nemzetközi szakirodalom legfrissebb, 2000 után publikált eredményei alapján életkor szerint megvizsgáljuk a CRT effektivitását, a peri- és posztprocedurális komplikációk arányát és a hosszú távú kimenetelt. Korábbi kutatások eredményei alapján CRT-implantációt követően szignifikánsan nő a bal kamrai ejekciós frakció és hasonló a reszponderek aránya valamennyi életkorban. A peri- és posztprocedurális komplikációk tekintetében nem találtak statisztikai különbséget az egyes korcsoportokban. A hosszú távú klinikai kimenetelt illetően, statisztikailag szignifikáns növekedést írtak le az összmortalitás tekintetében az idősebb csoportokban, ami a komorbiditások növekvő prevalenciájával magyarázható. Azonban az összhalálozás vagy szívelégtelenség-esemény miatti hospitalizáció kompozit végpontját, valamint önmagában a hospitalizációt vizsgálva nem találtak különbséget az életkor szerint. Az eddigi nemzetközi publikációk eredménye alapján, megfelelő indikáció esetén a CRT életkortól függetlenül hatékony terápia. Az idős, 70–75 év feletti betegekben a reszponderitás mértéke, a szívelégtelenség miatti hospitalizáció gyakorisága, valamint a szövődmények előfordulása összevethető a fiatal korcsoportokban leírtakkal. Paragh","doi":"10.26430/chungarica.2021.51.2.125","DOIUrl":"https://doi.org/10.26430/chungarica.2021.51.2.125","url":null,"abstract":"Series of clinical studies proved the beneficial effect of lipid lowering therapy on the process of atherosclerosis and the frequency of the cardiovascular events, and according to these it plays an awarded role in the preventive treatment recommendations. In the most recent European dyslipidemia guidelines a lower LDL-cholesterol target has been established. Despite the many evidence and the guidelines’ recommendations, the quality of lipid-lowering and also the goal attainment rate is inadequate. What are the reasons of the inertia? From the patients’ side the appearance of the different side effects is a factor for decreasing their compliance. The statin intolerance is believed to occur in 10-15% of cases. Many patients complain of muscle pain when taking statins, but the actual relation with medications is significantly less common than it appears based on the patients’ symptoms. The STOMP study was designed to examine the side effect, in the statin naive patients the occurrence of myalgia in the group taking 80 mg atorvastatin was 9.4%, in the placebo group 4.6%. In clinical endpoint HOPE-3 study, the incidence of muscle pain was 5.8% in case of patients taking 10 mg rosuvastatin and 4.7% for placebo group. According to a recently presented, very well-designed, randomized, double-blind SAMSON study, 90% of complaints that occur when taking statins are not due to a drug effect. Improving the patients’ adherence might be achieved through continuous education, the use of electronic devices, and simplifying the treatment. Data are scarce concerning the right ventricular (RV) morphological and functional remodeling in HFrEF in relation to the severity of TR. Accordingly, we aimed to characterize RV remodeling in HFrEF patients with and without significant TR using three-dimensional (3D) echocardiography. We retrospectively identified 138 patients with HFrEF. In this cohort, we graded TR severity according to current guidelines and compared patients with no to mild TR (non-significant TR, n=78) versus patients with moderate to severe TR (significant TR, n=60). All patients underwent clinically indicated 3D transthoracic echocardiography. 3D LV and RV end-diastolic volumes (EDVi) and ejection fractions (EF) were measured. To characterize RV mechanical pattern, the ReVISION method was used to quantify the contribution of the longitudinal, radial, and anteroposterior motion components to total RV EF. Patients with significant TR had higher LV EDVi and lower LV EF compared with patients with non-significant TR (LV EDVi: 117.2±34.9 vs. 102.6±39.6 ml/m2; LV EF: 27.0±6.6 vs. 30.2±7.7%, both p<0.05). Concerning the right heart, RV EDVi was significantly higher in patients with significant TR compared with those without (92.1±32.4 vs. 74.1±26.9 ml/m2; p<0.01). RV EF was lower in patients with significant TR (36±10.3 vs. 42.5±9.3%, p<0.05). Regarding RV mechanics, anteroposterior and longitudinal components were significantly decreased in patients with significant TR compared with patients with non-significant TR (anteroposterior relative contribution: 10.0±4.5 vs. 11.7±3.7%; longitudinal: 8.2±3.8 vs. 11.1±3.8%; both p<0.05). On the other hand, the radial component did not show a difference between patients with or without significant TR (17.8±6.9 vs. 19.8±6.2%; p=NS). By assessing RV mechanics using 3D echocardiography, we have shown that HFrEF patients with significant TR presented with reduced RV global function, mainly attributable to the deterioration of the longitudinal and anteroposterior motion components. The number of people with metabolic syndrome (MS) is increasing year after year. It is known that metabolic syndrome is a risk factor for cardiovascular disease and type 2 diabetes, so it is important to identify the risk factors for MS and improve the quality of life who is suff ering from this syndrome. Among the psychosocial risk factors we focused on sleep disorders and investigated their relationship with metabolic syndrome. Several studies have confi rmed that short sleeping time and poor sleep quality are associated with higher risk for metabolic syndrome. The relationship between long sleeping time and MS is less clear. Further longitudinal studies are needed to investigate the relationship between sleep disorders and metabolic syndrome, where sleeping is measured by objective means, and participants' age, gender, place of residence and socioeconomic status are taken into account.
