We aimed to compare the efficacy and outcome between transobturator and retropubic mid-urethral sling procedures. Materials and Methods: We reviewed the chart of 186 consecutive women who had sling procedures with a follow-up of at least 1 year from 2001 to 2006. The retropubic sling procedure was perfomed on 76 women assigned to retropubic group. A total 110 women received transobturator slings assingned to transobturator group. The preoperative evaluations, the indices of voiding, and postoperative complications were analysed by Student's t-test and chi-square. Results: Two groups were similar in their preoperative characteristics and the perioperative parameters except the period of hospitalization and operative time. There were significant difference between the 2 groups in terms of the postoperative obstructive and iatrogenic complications. Transobturator group had shorter hospitalization and operative time and lower complications than retropubic group. Conclusions: Because transobturator sling procedures showed better results and less complications than retropubic slings, it is a safe and efficient surgical treatment for female SUI. (J. We evaluated the relationship between detrusor overactivity and postmenopausal state and the effects of estrogen replacement for detrusor overactivity caused by the ovariectomy in rat. Material and Methods: Thirty female Sprague Dawley rats were divided into three groups: the control, the ovariectomy group and the estrogen replacement group after the ovariectomy. The ovariectomy and estrogen replacement groups had the bilateral ovariectomies at 12 weeks of age. After 2 weeks, the control and ovariectomy groups were injected weekly with saline, while the estrogen replacement group was injected with estradiol benzoate (500 / ) weekly. After 12 μg kg weeks, the voiding frequency study was recorded and a cystometrogram was performed while awake. The harvested bladders were used in the carbachol-induced detrusor muscle contraction study and the distribution of estrogen, M2 and M3 muscarinic receptors in the rats' bladder. Results: The ovariectomy group voided more frequently than the others (p=0.005). During the awake cystometrogram, the detrusor characteristics of the ovariectomy group were a higher mean intervoiding pressure and smaller bladder capacity than the others (p=0.000). There was no significant statistical difference between the control and estrogen replacement group. The other cystometric parameters were not statistically different either. The detrusor muscle contraction study showed no difference between three groups. {"title":"Impact of the Tension Adjustment of the mesh by using Babcock Clamp during Tension-Free Vaginal Tape Obturator Procedure on the Postoperative Voiding Pattern and Satisfaction.","authors":"J. Materials and Methods: Medical records of 118 women who underwent Tension-Free Vaginal Tape Obturator (TVTO) prucedure were retrospectively reviewed. Tension of the tape was controlled either by holding the 1 cm length of the mesh with Babcock clamp in 72 cases (Group I), or by placing Metzenbaum scissor between the tape and the urethra in 46 cases (Group II) before pulling on the tape. The preoperative evaluations included symptom questionnaire, history taking, physical examination, one hour pad test and complete multichannel urodynamic study. Postoperatively, uroflowmetry, postvoiding residual urine volume and symptom questionnaire were repeated. Results: The success rates of the operation were not different between groups. However, postoperative complication rates, including persistent pain and de novo urgency, in the Group II (n=9, 19.6%) was higher than in Group I (n=6, 8.3%). The rate of postoperative transient urinary retention was also higher in Group II (p<0.05). The rate of the patients who answered [satisfied] were significantly high in Group I (95.8%) compared to Group II (80.4%). Conclusions: By using the Babcock clamp technique, the complications can be minimized, and satisfaction increased. Our findings suggest that this technique does assure the proper tension for the normal voiding postoperatively. (J. Bipolar TURP can be another therapeutic option of large volume prostate, which in the past indicated open prostatectomy, to escape from higher surgical morbidity. (J. {"title":"Mesh Cutting after Midurethral Sling Procedure in Female Stress Urinary Incontinence: 1 Year Follow-up.","authors":"Dong Hwan Lee, Jo Un Jung, H. But some complications are inevitable, although the incidence is very low. Mesh cutting may be required to correct unwanted problems in some patients. We evaluated the outcome of mesh cutting in patients having mesh-related complications. Materials and Methods: Medical records of patients who underwent cutting of midurethral tape from January 2001 to December 2005 were reviewed and a detailed telephone interview was done to see if stress urinary incontinence recurred at least a year after cutting. Results: Eleven patients were included in this study. Mean age was 51.2 ranging from 41 to 70. The reasons why their meshes should be cut were as follows; eight (72.7%) had voiding difficulty, two (18.2%) had a tape erosion and one had voiding difficulty and overactive bladder. These problems were corrected by mesh cutting except one. However, a year after cutting, four out of eleven patients (36.4%) developed recurrent stress urinary incontinence. Recurrence occurred in three out of five patients (60.0%) whose meshes were cut within 1 month after implant, while occurred in 1 out of 6 (16.7%) whose meshes were cut after 2 months of implant. Three out of four patients (75.0%) who had mixed urinary incontinence developed recurrence after mesh cutting. In particular, two patients who had detrusor overactivity confirmed by cystometry before surgery showed recurrence of incontinence after mesh cutting. Conclusion: A total 36.4% of patients who required mesh cutting developed recurrence of SUI. These data demonstrate that mesh cutting may cause recurrence and urologists should be aware that mesh cutting may be disappointing. Mixed urinary incontinence and duration from implant to mesh cutting seem to be the risk factors of recurrence after mesh cutting. (J. The preoperative and postoperative evaluations analysis and urodynamic study and Korean version of I-QoL, surgical outcomes, complications and uroflowmetry were analysed. Results: All outcomes included the patients̀ states, incontinence quality of life (I-QoL)(p=0.013), the success rate of the cure (p=0.003) and the rate of the patient satisfaction (p=0.001) were not comparable statistically between two groups and there are no differences and no long-term complications such as bladder perforation or prolonged voiding difficulty. Preoperative and postoperative uroflowmetry was similar between the two groups. Mean operation time (TVT: 15.6 min, TVT-O: 13.2 min) was significantly shorter in TVT-O than in TVT. Conclusion: The observation of 1-year following-up shows the equally effectiveness between two groups (TVT and TVT-O) as the surgical treatment of stress urinary incontinence. (J.
