Materials and Methods: Eleven patients with neurogenic detrusor overactivity were enrolled in this study. All patients underwent more than 16 SMS sessions, two times per week, with the use of an arm-type magnetic stimulator. Objective success was defined as a significant reduction (≥50%) in the total number of leakage episodes per 24 hours, whereas subjective success was defined as the request to continue treatment. Results: Five (71.4%) of 7 patients who had urinary incontinence before treatment reported objective success, and 3 patients showed no incontinence. A total of 6 (75.0%) of 8 patients showed increased average voiding volume. One patient showed an adverse effect of back pain after SMS and dropped out. Seven patients (70.0%) of 10 patients wanted to continue this treatment. They showed significant improvements in frequency/volume chart data, quality of life scores, and urodynamic data. Conclusions: SMS can be a safe, non-invasive and effective option for the treatment of neurogenic detrusor overactivity. We evaluated the efficacy and safety of cystocele repair using monofilament polypropylene mesh (Gynemesh PS). Materials and Methods: This study was performed in 53 patents who underwent cystocele repair using monofilament polypropylene mesh between January 2006 and January 2009. According to the ICS (International Continence Society) stage classification, 33, 17 and 3 women had stage II, III and IV cystocele. The operation were performed through the vaginal approach. Patients were followed up for 9 to 36 months. We defined the cure of cystocele as stage 0, improvement as stage I, and failed as stage II or greater Results: The mean follow-up was 23.8 months. At follow-up, 41 women were anatomically cured (77.4%), 12 women were improved as stage I (22.6%) and no one was failed. Six cases were previously ICS stage II, 5 cases were stage III and 1 case was stage IV in improved group. No significant intraoperative complications occurred. The postoperative complications were de novo urgency (4 cases, 7.6%), erosion of mesh (2 cases, 3.8%) and anterior vaginal wall hematoma (1 case, 1.9%). Conclusion: The use of polypropylene mesh for correction of cystocele by transvaginal route with tension free technique seems to be a safe and effective procedure. Since sonography has been the standard tool for the assessment of PVR, a portable, automated device (BladderScan) produced measurements of PVR that correlated well with catheterized volumes in most studies [1-4]. However, portable ultrasound is not as specific for urinary retention as it is sensitive, of which false-positive rate was upto 9% even in the most favorable series [4]. This relatively low specificity is maybe because BladderScan does not appear to distinguish between residual urine and their fluid collections in the pelvis and lower abdomen [5]. We investigated the possible effects of NAC after ischemia/reperfusion (I/R) of rat bladder. Materials and Methods: I/R injury was induced by abdominal aorta clamping and ischemia for 60minutes, followed by 120minutes reperfusion. Twenty rats were divided into four groups: sham operation + saline group (S+S), sham operation + NAC group (S+NAC), I/R + saline group (I/R+S), I/R + NAC group (I/R+NAC). Blood levels of reactive oxygen species (ROS) were determined using the free oxygen radical tests (FORT). Superoxide generation was measured based on lucigenin-enhanced chemiluminescence. The level of malondialdehyde (MDA) was analyzed in order to measure lipid peroxidation. Results: In I/R+S group, the isometric contractile responses to carbachol were significant lower than other groups and were reversed by the pretreatment with NAC. The level of FORT and MDA showed a marked increase in I/R+S group compared with S+S group. NADPH-stimulated superoxide production was also significantly increased. I/R+NAC decreased these parameters compared with I/R+S group. Conclusion: Our results suggest that treatment with NAC reversed the low contractile responses of rat bladder and prevented oxidative stress following I/R. The aim of our study was to observe the experimental conditions required to reproduce the clinical pharmacological effects of tolterodine, as seen in humans, to decrease bladder pressure or increase bladder capacity. Materials and methods: We studied the effects of the most widely used antimuscarinic drug, tolterodine, on cystometric parameters via two different administrations (oral and intravenous) in awake SHRs. Results: Oral administration of tolterodine 10 mg/kg body weight in awake rats did not change any cystometric parameters significantly. Intravenous administration of tolterodine 0.3 mg/kg body weight significantly decreased basal pressure (BP) and micturition pressure (MP), but showed no effect on micturition interval (MI) or bladder capacity (BC). Conclusion: Despite a high dose of tolterodine via an oral or an intravenous route, a decrease in BP or MP was the only effect on cystometrographic parameters in awake rats, whereas MI and BC were not significantly affected. Therefore, it is difficult to reproduce in awake rats as an acute response the cystometric increase in the MI that is observed in humans after chronic administration of antimuscarinic agents. However, there is lack of data proving the overactive bladder (OAB) on sexual function. We evaluated the impact of OAB on sexual function. Materials and Methods: We included 82 women from 18 to 65 years old with overactive bladder symptoms including urinary frequency and urgency. A control group was recruited at the Health Care Center for health medical examination of our hospital. All patients were asked to complete the Koran version of Female Sexual Function Index (FSFI) to assess sexual function. Results: The Mean age was 47.37±8.73 and 49.90±5.36 in OAB and Control group. Each of the dimension scores in FSFI represents worse sexual function in patients with OAB than the control group. The total scores of the FSFI were significantly decreased in the OAB group 20.10±4.10 than 24.64±3.91 (p<0.05). Among the six subdomains of FSFI, four domains of desire (4.92±1.59 vs 6.80±2.19), arousal (9.87±2.87 vs 11.50±2.44), orgasm (7.22±2.09 vs 9.52±2.96), satisfaction (6.95±2.09 vs 8.45±2.28) scores were significantly decreased in the OAB group (P<0.05) than control group. But, there was no statistically significant differences in domain of lubrication (11.12±2.80 vs 11.72±2.56) and pain (6.85±1.91vs 6.92±1.92) between the OAB group and control group. Conclusion: In this study, OAB syndrome was found to cause greater deterioration in sexual function. These results suggest that OAB symptoms have a significant impact on women’s sexual function. We analyzed the clinical and urodynamic findings of elderly patients with LUTS to search for accurate diagnosis and effective treatments. Materials and Methods: We performed a retrospective chart review of 154 male patients older than 65 years old with LUTS who underwent urodynamic studies between January 2002 and December 2008. The patients were divided into irritative and obstructive symptom groups according to their chief complaints. The urodynamic findings between two groups were compared. And the prevalence of detrusor dysfunction (either detrusor underactivity; DU or detrusor overactivity; DO) according to age, history of urinary retention, the presence of an indwelling urethral catheter, neurologic disease, or diabetes was estimated. Results: The mean age of men was 70.8±4.5 years. On urodynamics, detrusor dysfunction was detected in 116 (75.4%) of 154 patients. Seventy eight (50.6%) patients showed demonstrable evidence of DU of whom 33 (21.4%) had concomitant DO, while 18 (11.7%) had concomitant bladder outlet obstruction (BOO). BOO and DO was identified in 67 (43.5%) and 71 (46.1%) patients, respectively. The prevalence of was significantly greater in the presence of history of acute urinary retention and an indwelling urethral catheter. Conclusion: One hundred forty one (91.6%) elderly patients with LUTS showed urodynamic abnormalities such DO, DU and BOO. Urodynamic study would play a important role in establishing a correct diagnosis in elderly patients with LUTS and deciding on additional treatments. Materials and Methods: From August 2006 to December 2008, we reviewed the medical records of 261 patients underwent Canal TOT in Division of Urogynecology, Department of Obstetrics and Gynecology, Kangbuk Samsung Hospital, Sungkyunkwan University School of Medicine in Seoul. This study is a retrospective analysis of the clinical characteristics (age, gravida, parity, body mass index), previous operation history, comorbidity, surgical procedure and operation outcomes. We performed stress test, one hour pad test, urodynamic study, postvoid residual assessment to all patients for diagnosis of SUI. All patients answered self assessment questionnaires (IIQ-7, UDI-6) at 6 months and 12 months after operation. Results: 55 women over 65 years were underwent Canal TOT. Mean follow up was 11±4.5 months. Mean age of patients was 70.2±3.9 years, gravida 5.8±2.3 times, parity 3.9±1.5 times and body mass index (BMI) 25.6±3.1Kg/m. 8 patients had got hysterectomy (14.5%). 16 patients (29%) had sling operation (Canal TOT alone), and 39 patients (71%) had Canal TOT combined with vaginal surgery for pelvic organ prolapse. The cure rate was 96.4% in 6 months follow up. Leakage after operation were reported by 2 patients (3.6%) and 3 patients (5.5%) transiently suffered from postoperative voiding difficulty.
