We present a case of postpartum emphysematous cystitis after Casarean section (C-sec) in a young female without underlying disease. Therefore, the authors evaluated the effect of tolterodine and oxybutyninin on nocturia in BPH patients. Materials and Methods: From September 2006 to March 2007, 82 patients who presented over than 2 in nocturnal bladder capacity index (NCBI) in spite of having alpha blockers for 6 months were enrolled. Group I (n=38) took alpha blocker with tolterodine, group II (n=44) took alpha blocker with oxybutynin. The number of their nocturia episodes was separately evaluated by the time before and after the medication. The complications were assessed using a questionnaire. Results: The number of nocturia episodes decreased by at least 1 in 68.4% (26/38), 84.1% (37/44) of patients in group I, II, respectively, and decreased by 2 or more, 1 and were unchanged or increased were 36.8, 31.6, 31.6% in group I patients and 45.5, 38.6, 15.9% in group II patients, respectively. In baseline nocturia ≥6 group, the nocturia decreased by 1 or more in 66.7%, 77.8% in group I, II, respectively. Adverse events, including dry mouth, dizziness, headache, etc, occurred in 21.1% (8/38) in group I and 27.3% (12/44) in group II patients. The complications between two groups showed no significant differences. Conclusions: Alpha blockers with tolterodine or oxybutynin can be effectively combined as a treatment option for patients with BPH complaining of unresolved nocturia. Materials and Methods: All voided volumes, times and frequencies were recorded in 249 women with overactive bladder. The subjects age related changes in bladder capacity, urinary volume and frequency were evaluated. The causes of nocturnal urinary frequency and its increase with age in older women with OAB were evaluated using 3 days of voiding diaries. Nocturia was devided into three types: nocturnal polyuria, decreased nocturnal bladder capacity and mixed type. Results: Total voided volume, daytime urine volume, functional bladder capacity were decreased with the age. Nocturnal urine volume and nocturnal urinary frequency were increased with age. Nocturnal polyuria was the major cause for nocturia, and mixed type was increased with age. Conclusions: Voiding symptoms as well as storage symptoms were increased with age in OAB female. In most cases, nocturia was caused by nocturnal polyuria. There are 5 types muscarinic receptor subtypes cloned (M1-M5). Among them, the M2 subtype distributed dominantly in amount in the detrusor and its role seems to be primarily by blocking stimuli from cAMP-coupled receptors that induce relaxation. In contrast, through M3 receptors on the detrusor the strong contractile stimulus evoked even if its distribution in secondarily common. The excitatory M1 and inhibitory M2 and M4 subtypes are also expresswed prejunctionally. Like detrusor, urothelium also expressed all subtypes of muscarinic receptors, and on stimulation it releases factors affecting bladder afferents and smooth muscle. During inflammation, the expression of muscarinic M5 receptors is increased, particularly in the urothelium, together with a cholinergic-induced production of nitric oxide in the mucosa. This review will be discussed expression and functional effects of muscarinic receptors in the lower urinary tract. Clinical aspects of the different muscarinic receptors and roles in the pathologic conditions included in this discussions. Materials and Methods: Thirty-three patients whose symptoms persisted after more than three months of first-line drug therapy were chosen to receive additional COX-2 inhibitors or antidiuretic hormones orally. Seven patients (group 1) were given 80mg of zaltoprofen at night, while 15 (group 2) were given 100mg of nimesulide at night. Desmopressin acetate (0.2mg) was administered at night to 11 patients (group 3) as a control group. Median follow up was 35 days (range, 28∼90 days). Results: In 25 patients (75.8%), the severity of nocturia was reduced. The median decline of nocturia in the COX-2 inhibitor groups (groups 1 and 2) was once, and it was statistically significant (p<0.001), while the median decline in each of these groups was twice (p=0.026) and once (p=0.002), respectively. The reduction of nocturia in the control group was once (p=0.011). The differences in reduction between the COX-2 inhibitor group and the control group were not statistically significant (p=0.418). Conclusions: The effects of the COX-2 inhibitors were not significantly different from those of desmopressin. Combination therapy with COX-2 inhibitors can effectively reduce nocturia in patients with refractory nocturia, following first-line drug therapy. Tolterodine tartrate (detrusitolR) is a widely known selective anticholinergics to bladder, which does not cause a cognitive dysfunction. This study was designed to analyze the change of cognitive function of brain disease patients, whom are taking anticholinesterase inhibitor with tolterodine for overactive bladder (OAB). Material and methods: From January 2001 to December 2004, with the patients whom have been followed for the brain disease in the department of neurology, we have analyzed 79 patients with tolterodine administered for OAB. We used K-MMSE (Korea minimental status examination) and SNSB (seoul neuropsychological screening battery) to analyze the state of cognition. Mean age of patients was 67.3±4.5 (yrs), mean administration period was 4.7±9.5 (mon). Results: 7 patients made complaints for the decline of memory, 2 of them with Parkinsonism and 2 with cerebral infarction, 1 with progressive supranuclear palsy and, 2 dementia with lewy body (DLB). Patients with DLB was excluded in this study because the disease had fluctuation of cognitive function. Conclusions: The result of these studies demonstrates tolterodine tartrate caused the decline of cognitive function in only a few patients with brain disease. We concluded that prospective studies are needed to change the cognitive functions of the brain disease patients with OAB after administration of tolterodine tartrate. Material and methods: A total of 230 women who underwent urodynamic studies for evaluation of overactive bladder were retrospectively reviewed. The patients were classified as having obstruction based on two criteria, including one pressure flow cutoff point criteria (free Qmax < 12ml/ sec and pdetQmax ≧ 20cmH2O) (group I) and moderate or severe obstruction by the Blaivas- Groutz nomogram (group II). Urodynamic characteristics of the patients in Group 1 and 2 were compared with those of 42 patients having only overactive bladder without any voiding symptom (OAB group). Results: Of the patients 31 (13.5%) were in group I, 42 (18.3%) in group II. Qmax of group I was the lowest among 3 groups. Detrusor pressure at maximum measurable flow and maximum detrusor pressure during voiding in group 1 and II were significantly higher than in OAB group. Detrusor overactivity was more observed in group I and II, and there was no signigicant difference in the presence of voiding symptoms between group I and II. Only 9 patients (4%) had obstruction by both of criteria. Conclusions: Considering the high incidence of bladder outlet obstruction in the patients of overactive bladder and non-specificity of the symptoms by bladder outlet obstruction, thorough urodynamic study may be helpful in detection and treatment of bladder outlet obstruction of the overactive bladder patients. The further study for the criteria of female bladder outlet obstruction should be necessary to make its accurate diagnosis. The International Continence Society (ICS) prefers the term Painful Bladder Syndrome (PBS) defined as \"the complaint of suprapubic pain related to bladder filling, accompanied by other symptoms such as increased daytime and night-time frequency, in the absence of proven urinary infection or other obvious pathology\". Interstitial cystitis is a clinical diagnosis primarily based on symptoms of urgency/frequency and pain in the bladder and or pelvis. The pathogenesis of IC is still not completely understood, but it is likely multifactorial. The major etiologic theories include abnormality of the bladder urothelium, bladder mast cell activation, bladder inflammation, and altered bladder innervation. The sensitivity and specificity of urinary markers and the potassium sensitivity test have not been prospectively studied. Antiproliferative factor and Tamm-Horsfall protein are novel tests that may prove to be worthwhile pending future studies. Management includes patient education, dietary and lifestyle counseling, oral therapy, intravesical therapy, and surgery. Recently, the European Society for the Study of Interstitial Cystitis (ESSIC) proposed a new nomenclature and classification system. This article discusses recent data and outlines current concepts of IC/PBS.
