{"title":"Effect of swallowing training on dysphagia patients after anterior cervical decompression and interbody fusion","authors":"Huan-Chieh Chen, Chang-jiang You, Yan Zhao","doi":"10.3760/CMA.J.ISSN.1672-7088.2020.07.006","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1672-7088.2020.07.006","url":null,"abstract":"Objective \u0000To explore the effect of swallowing function training on dysphagia patients after anterior cervical decompression and interbody fusion. \u0000 \u0000 \u0000Methods \u0000From January 2014 to January 2018, 119 patients with anterior cervical decompression and interbody fusion and difficulty in swallowing were divided into intervention group (59 cases) and control group (60 cases) by random digits table method. Patients in the control group were given routine hospital nursing and health education, while patients in the intervention group were given swallowing function training for 6 months, and swallowing function and quality of life were evaluated by Water Swallow Test and M.D.Anderson Dysphagia Inventory(MDADI) and compared between the two groups. \u0000 \u0000 \u0000Results \u0000There was no significant difference in swallowing function and quality of life between the two groups(P> 0.05). After intervention, Water Swallow Test score was 2.00 (1.00, 3.00) in the intervention group, and 3.00 (2.00, 4.00) in the control group, the difference was statistically significant (Z value was 3.205, P < 0.01). Then the General Self-Efficacy Scale(GSES) and visual simulation were used to evaluate self-efficacy in six groups, and the evaluation results were compared to analyze the influence of age on treatment efficacy in patients with limb fracture. \u0000 \u0000 \u0000Results \u0000There was no significant difference in GSES scale and self-efficacy between group A and group B (P > 0.05). The proportion of low score and low self-efficacy score of group A and group B was significantly lower than that of group C, D, E and F, the proportion of high score of GSES scale and high score of self-efficacy was significantly higher than that of group C, D, E and F, the proportion of those with low GSES scores and low self-efficacy scores in group C, D, E and F increased in turn, while those with high GSES scores and high self-efficacy scores decreased in turn, and the difference between the two groups was significant (χ2 value was 10.09-50.86, all P < 0.05). \u0000 \u0000 \u0000Conclusions \u0000The treatment efficacy of patients with limbs fracture in different ages have obvious differences,and the treatment efficacy of patients should be paid more intervention with the age increasing. They were investigated by the Jalowiec Coping Scale, The Eysenck Personality Questionnaire-Revised Short Scale for Chinese, General Self-Efficacy Scale, Social Support Rating Scale. \u0000 \u0000 \u0000Results \u0000Optimistic coping in gynecologic tumor patients scored (1.61±0.36). The main influencing factors included residence place, education level, general self-efficacy score, personality trait score, they could explain 25.2% variation of optimistic coping of gynecological tumor patients. \u0000 \u0000 \u0000Conclusions \u0000Optimistic coping in gynecologic tumor patients scored is at a low level. Residence and education level, general self-efficacy score and personality trait score are important factors that affect the level of optimistic coping in gynecologic tumor inpatients. Eighteen of the operating room nurses were interviewed by phenomenological research methods in qualitative research. \u0000 \u0000 \u0000Results \u0000The obstacles to the professional management of nurses in the operating room were mainly reflected in the difficulty of doing only the work of specialist nurses (96.46%,218/226), \"there is no clear role positioning (93.81%, 212/226)\", and \"responsible for the work of several departments(89.82%, 203/226).\" The refinement and analysis of the interview data showed that the management of specialist equipment could not be arranged and managed in a coordinated manner; the surgeons were over-reliant on the specialist nurses; the problem of setting up the positions of the senior nurses was outstanding; the nurses who were not the specialists have insufficient competence. With the sense of belonging; specialist nurses would have a slack situation. \u0000 \u0000 \u0000Conclusions \u0000There are still many problems in the implementation of specialist management in the operating room of general hospitals. A total of 80 handoffs was evaluated to test the reliability and validity of the Chinese version of the Handoff CEX. \u0000 \u0000 \u0000Results \u0000A total of 320 evaluations of handoff was obtained. The Cronbach α coefficient was 0.85 for the handoff provider, and that was 0.80 for the handoff recipient. Weighted kappa scores for provider evaluations ranged from 0.31-0.52, and that for recipient evaluations ranged from 0.35-0.55.
