Portnov","doi":"10.17816/117758","DOIUrl":"https://doi.org/10.17816/117758","url":null,"abstract":"BACKGROUND: Motor disorders as a result of аcute cerebrovascular accident are the most common cause of disability and not only significantly reduce the quality of life of patients, but also cause restriction of social and household independence of patients in a familiar environment. \u0000AIM: the evaluation of the effectiveness for restoring the social and domestic activity skill ― shopping in the supermarket in patients undergoing stroke in the right middle cerebral artery after course of the special apparat trainings with audio-visual reconstruction of the environment. \u0000MATERIALS AND METHODS: The study included 65 participants, they were divided into 3 groups, the first and the second group ― the patients who undergoing the stroke (main and control groups): the main group of patients received a base standard rehabilitation program (kinesiotherapy, mechanotherapy on cyclic apparatгs, massage) and additional the social and domestic trainings with audio-visual reconstruction of the environment. The control group ― a base standard rehabilitation program. The duration of the rehabilitation course was 21 days. The third comparison group are the healthy participants, who were necessary for defining \"etalon\" options for training of socially important skill. \u0000RESULTS: As a result of the study in both groups (main and control) as compared with the initial findings the lowering of the heaviness ischemic stroke outcome (NIHSS): 6.2±0.4 in the first group and 6.8±0.7 in the second group vs. 4.2±0.2 and 5.9±0.8 (р ≤0.05). By comparison the first and the second groups with the health participants were defined the statistically significant changes of the studing time options in the main group during making these skills: take the bag and sling it through the shoulder, open the pockets of the bag (2 pockets), take the jacket from the clothes hanger and put on. Although in patients of the first group after restoring threatment course with using method of functional spatially-oriented rehabilitation it was authentic lesser disturbance of everyday life activity: in estimating Bartel Index 61.3±4.9 vs. 43.75±5.1 (initial) (р ≤0.05), what was signuficantly higher be comparison with control group results. In estimating functional independence by Modified Scale FIM: 71.4±4.9 in the main group vs. 53.7±3.1 (initial) (р ≥0.05), what was although significantly higher in comparison with the second group. By HADS it was lowering of the anxiety symptoms in the first group from 13.02±1.9 till 10.12±1.2 (р ≤0.05) as compared with the control group where the level of anxiety was staying quietly high and was 12.6±0.6 by comparison the initial findings 13.14±1.5 (р ≥0.05). \u0000CONCLUSION: The obtained resalts point at the effectiveness of using functional spatially-oriented rehabilitation in restoring threatment programs for patient undergoing stroke for elevation of the independence level in real life terms. Boyarintsev","doi":"10.17816/569340","DOIUrl":"https://doi.org/10.17816/569340","url":null,"abstract":"BACKGROUND: Previous studies on the comparative evaluation of the effectiveness of the course application of therapeutic physical factors during the complex medical rehabilitation of patients with polypous rhinosinusitis showed a rather high efficiency of the additional use of complex physiotherapy (alternating magnetic field and low-intensity laser therapy) in enhancing the regression of the main clinical manifestations of polypous rhinosinusitis and improving the quality of life (QoL) of patients. Considering the chronic and often recurrent nature of polypous rhinosinusitis, along with the relief of the main clinical manifestations of the disease and improvement in the quality of life of patients after treatment, medical rehabilitation of patients should also cover the long-term period. \u0000AIM: Evaluation of the results of complex medical rehabilitation of patients with polyposis rhinosinusitis in the long-term period (after 6, 12 and 18 months) and determination of the optimal time parameters for a repeated course of physiotherapy aimed at maintaining the achieved clinical effect and the level of quality of life of patients. \u0000MATERIALS AND METHODS: The study was performed with the participation of 120 patients with moderate to severe polypous rhinosinusitis and uncontrolled course. All patients were divided into 4 groups by simple fixed randomization. The first group (control, 30 patients) received only basic therapy, including endonasal application of Nasonex 2 doses 2 times a day and daily twice daily lavage of the nasal mucosa. Patients of the second group (comparison group 1, 30 patients) were additionally injected subcutaneously with dupilumab at a dose of 300 μg once every two weeks. In the third group (comparison group 2, 30 patients), against the background of the administration of dupilumab, exposure to an alternating magnetic field was performed. using the apparatus MAGNIT Med TeCo. Patients of the fourth group (main group, 30 patients), in addition to the volume of treatment of patients of the third group, received low-intensity laser radiation of the red spectrum (633 nm) using the AZOR-2K-02 apparatus. \u0000RESULTS: Positive dynamics in terms of assessed clinical indicators throughout the entire observation period was noted only in the comparison groups and the main group. The maximum severity of positive changes was determined in the main group after 6 and 12 months of rehabilitation, exceeding the achieved values in the group with dupilumab. The follow-up period made it possible to establish that 18 months after the start of rehabilitation, there were no significant differences between these groups. Kaisinova, Yu V Bobrik","doi":"10.17816/321299","DOIUrl":"https://doi.org/10.17816/321299","url":null,"abstract":"BACKGROUND: In modern medicine, among the main indicators of the effectiveness of therapeutic measures, the quality of life of the patient is singled out, which allows determining the degree of recovery of physical and mental health after treatment of acute conditions. \u0000AIM: to evaluate the effectiveness of complex balneopeloid and enzyme therapy with longidase according to the monitoring of the quality of life of the patients suffering from chronic bacterial prostatitis. \u0000MATERIALS AND METHODS: There have been observed 54 patients with chronic prostatitis. They were divided into 2 groups by simple randomization: in the comparison group (n=26) the patients were prescribed exercise therapy, internal and external administration of mineral water of Zheleznovodsk type, peloidotherapy with silt sulfide mud topically, antibiotic therapy with ceftriaxone, phytomicroclysters; in the main group (n=28), the patients additionally underwent antibiotic and enzyme therapy by peripheral indirect lymphotropic therapy. The efficiency was assessed using validated Quality of Life questionnaires: Chronic Prostatitis Symptom Scale (NIH-CPS) and SF-36 General Health Questionnaire (Social Functioning-36). \u0000RESULTS: The analysis of the clinical effect of the new treatment technology revealed its advantage over the standardized therapy, which is proven by the monitoring of QoL indicators for 1 year after the spa treatment: recovery of QoL in the main group immediately after the treatment occurred by 59.7% (p 0.01), 6 months later ― by 72.7% (p 0.01), with a slight decrease in the rate in 12 months ― by 64.9% (p 0.01). The dynamics of physical and mental health indicators on the SF-36 scale was also observed with the same degree of confidence: compared with the baseline, the average improvement was 25.5% (p 0.01), 33.6% (p 0.01) and 27.3% (p 0.01) respectively. There is no doubt that rehabilitation is a necessary component of the complex treatment of the patient and plays an important role in the postoperative period. Physiotherapeutic methods of treatment occupy a special place in rehabilitation programs. Cryotherapy has been used for many years in the pathology of the musculoskeletal system and has proven its clinical effectiveness due to analgesic, anti-inflammatory and decongestant effects, which gives grounds for its use after surgical interventions. \u0000AIM: Based on the above, the purpose of this study was to study the effectiveness and rationale for the use of cryotherapy in combination with therapeutic gymnastics procedures in patients after total knee replacement for the third the rehabilitation stage. \u0000MATERIALS AND METHODS: The study included 60 patients after total knee replacement at the age of 57 to 79 years. All patients were admitted for outpatient treatment 3 months after surgery and were divided into 2 groups. The first group consisted of 30 patients who had therapeutic gymnastics classes with a physical therapy instructor (physical therapy), for a course of 10 daily procedures. The second group consisted of 30 patients who underwent a comprehensive rehabilitation program, including therapeutic gymnastics and air cryotherapy, for a course of 10 daily procedures. \u0000RESULTS: In all patients, before the start of the rehabilitation course, the intensity of the pain syndrome according to VAS averaged 4.5±0.6 points; according to the 10-meter walking test, a decrease in the level of mobility was detected, in most patients, the sum of the points on the two parts of the KSS scale averaged 62.8±4.5; according to the results of laser Doppler flowmetry, 44% of the subjects revealed spastic, and in 56% ― spastic-stagnant type of microcirculation. AIM: to study the effect of therapy with a pulsed electromagnetic field on pain, disability, psychological state and quality of life in intervertebral hernia of the cervical spine. MATERIALS AND METHODS: Patients were randomly divided into two groups, including group 1, who received therapy consisting of transcutaneous electrical nerve stimulation, thermal procedures, and pulsed electromagnetic field, and group 2, who received a false magnetic field in addition to transcutaneous electrical nerve stimulation and thermal procedures. Pain was assessed using a visual analogue scale (VAS). Other outcome measures were pain and disability (Neck Pain and Disability Scale), anxiety-depression (Hospital Anxiety and Depression Scale), and quality of life (Nottingham Health Profile). All evaluations were performed before treatment, at week 3, and at week 12 post-treatment. RESULTS: Significant improvements were found in scores for neck pain, disability, depression, anxiety, and quality of life in both groups after treatment compared with scores before treatment. However, when comparing changes within groups, significant improvements were only identified in VAS and Nottingham Health Profile sleep subparameters at week 12 post-treatment compared to pre-treatment. Nagornev","doi":"10.17816/rjpbr115249","DOIUrl":"https://doi.org/10.17816/rjpbr115249","url":null,"abstract":"BACKGROUND: Despite the overall success of arthroscopic anterior cruciate ligament reconstruction, the scientific literature contains studies reporting high recurrence rates (up to 30%), often accompanied by other lesions such as meniscal tears and cartilage damage. The main reasons for this development of events are considered: repeated injury of the anterior cruciate ligament, possible technical errors during surgery, as well as the timing of reconstructive operations after injury. AIM: Assessment of long-term (after 1 year) results of complex medical rehabilitation of patients after reconstruction of the anterior cruciate ligament, performed at different times after injury. MATERIALS AND METHODS: The study involved 175 patients aged 18–55 years with an isolated rupture of the anterior cruciate ligament of the knee joint, who underwent rehabilitation measures after arthroscopic reconstruction of the anterior cruciate ligament. Using the stratification method, all patients were divided into two groups depending on the time of arthroscopic reconstruction of the anterior cruciate ligament. In the group with early reconstruction (88 patients), anterior cruciate ligament reconstruction was performed early after the injury, from the 2nd to the 6th week inclusive. Patients of the second group (delayed reconstruction of the anterior cruciate ligament, 87 patients) were operated on late after the injury from the 7th week to 1 year. Each of these stratification groups was divided into two groups using a simple fixed randomization method: control (with a basic rehabilitation complex) and main (with the additional inclusion of high-frequency magnetic therapy and hydrocortisone ultraphonophoresis). The basis of the follow-up study was the results of questionnaires for subjective assessment of the functional state of joints, as well as an assessment of the level of quality of life of patients obtained using MOS SF-36. RESULTS: A comparative comparison of long-term results obtained in groups where arthroscopic reconstruction of the anterior cruciate ligament was performed early after injury made it possible to establish the advantage of the proposed complex of medical rehabilitation based on high-frequency magnetic therapy and hydrocortisone ultraphonophoresis. This was manifested by a significantly low level of pain according to visual analog scale and a statistically significant excess on scales of subjective assessment of the functional state of the knee joint. CONCLUSION: The obtained results of positive dynamics of the condition of the knee joint of patients 1 year after reconstruction of the anterior cruciate ligament and rehabilitation indicate the restoration of stability and kinematics of the knee joint. However, there is practically no information about the effectiveness of using non-thermal gentle methods of peloid therapy using peat mud in patients after spinal surgery for disc herniations, about the indications for their use depending on the existing clinical and functional characteristics. AIM: to develop and scientifically substantiate the use of resource-saving non-thermal peloidotherapy techniques in the rehabilitation of patients after spinal surgery for herniated discs. MATERIALS AND METHODS: 74 patients aged 20 to 65 years were examined 4–9 weeks after microdiscectomy. Patients underwent clinical and functional examination before and after the rehabilitation course, pain intensity was determined using a visual analog scale (VAS). Functional activity was assessed using the \"Get up and go\" test (3 and 6 meters). Infrared thermography of the back and lower extremities was performed. The study used the Oswestry questionnaire, as well as the HADS Anxiety and Depression Scale. By simple randomization, all patients were divided into 2 main groups (25 people each) and a control group (24 people). The control group received basic treatment ― therapeutic gymnastics and massage of the lower limb on the affected side. In the I main group, 10 procedures of thin-layer mud applications were additionally prescribed. In the II main group, 10 procedures of fluctuophoresis of peat mud solution were prescribed for basic treatment. The rehabilitation course lasted 14–15 days. RESULTS: Repeated examination after the course of treatment showed a significant decrease in the VAS index in both main groups, more significant in group II, whereas in the control group the changes were not significant. In all the studied groups, there was a reduction in the execution time of the \"Get up and Go\" test, but it was most significant in both main groups. Positive shifts on the Oswestry scale were also observed in all groups, including the control group, but some advantage was determined in the main groups receiving peloid therapy. Analysis of repeated results of remote infrared thermography revealed a decrease in local hyperthermia in the area of surgical intervention in persons receiving non-thermal peloid therapy techniques, the most significant in the II main group. Indicators of anxiety on the HADS scale decreased by the end of rehabilitation in all the studied groups, and subclinical manifestations of depression ― only in the I and II main groups. CONCLUSION: The inclusion of non-thermal resource-saving peloidotherapy techniques in the rehabilitation complex of patients after microdiscectomy makes it possible to more effectively relieve pain syndrome, eliminate microhemodynamic disorders and increase the motor activity of patients, which contributes to the normalization of their psychoemotional state and the quality of their life. AIM: Evaluation of the effectiveness of the combined use of transcranial magnetotherapy and a pulsed low-frequency electrostatic field in patients with metabolic syndrome using the method of correlation adaptometry. MATERIALS AND METHODS: The study involved 100 patients with a diagnosis of metabolic syndrome established in accordance with clinical guidelines. All patients were divided into four groups of 25 by simple fixed randomization. Patients in the control group received a placebo effect (imitation of physiotherapeutic effects with the device turned off) for 10 days of observation, in comparison group 1 ― exposure to a low-frequency electrostatic field, in comparison group 2 ― transcranial magnetotherapy with a traveling magnetic field, in the main group ― exposure to a pulsed electrostatic field in combination with transcranial magnetotherapy. The objectification of the effects of physiotherapy (evaluation of functional, biochemical and hormonal parameters) was based on a two-stage examination (before and after a course of treatment). RESULTS: Correlation adaptometry was based on the results of correlation analysis using a correlation matrix of variables characterizing the condition of patients after course correction. For each of the four groups of patients, 28 correlation coefficients were determined. For each group, 378 correlation coefficients were calculated, presented as the weight of the correlation graph. Additionally, the indicator of the average adaptive correlation was calculated. It is shown that the use of therapeutic physical factors is accompanied by a decrease in the weight of the correlation graph, which indicates the severity of the clinical effect due to the sanogenetic action of physiofactors. The most pronounced decrease was noted in the group of combined physiotherapy. Correlation analysis between the parameters of the average adaptive correlation and the integral coefficient of therapy efficiency showed that there is a strong, negative significant relationship between the HOMA insulin resistance index and the Quetelet index, chosen as the most informative variables. CONCLUSION: It is concluded that the method of correlation adaptometry is highly informative in assessing the effectiveness of using a pulsed low-frequency electrostatic field, transcranial magnetotherapy and their combined effects in patients with metabolic syndrome. The relevance of this problem lies in the high damage to the mental health of the patient, leading to a deterioration in his quality of life. In addition, drug therapy with the use of type 5 phosphodiesterase inhibitors is accompanied by headaches, skin flushing, heartburn, nasal congestion and dizziness, visual impairment, and muscle pain. In this regard, it became necessary to use physiotherapeutic methods of treatment to restore erectile function with minimal side effects. Often, as a result of skin scarring, the psychological status of the patient changes due to the presence of persistent subjective sensations or the location of defects in open, functionally active areas of the skin. The steady annual growth in trauma, the number of surgical interventions and invasive procedures actualizes the question of finding an optimal, effective and widely available method for preventing severe skin scarring or correcting fibrous deformities. AIM: scientific and practical substantiation of the use of enzyme gel ultraphonophoresis with direct proteolytic activity in patients with hypertrophic skin scars. MATERIALS AND METHODS: A prospective interventional cohort comparative study of the efficacy and safety of enzyme gel ultraphonophoresis with direct proteolytic activity in patients with hypertrophic skin scars was performed. The study included 42 patients with a diagnosis of \"Hypertrophic scars\" (ICD-10, L91) of postoperative or post-traumatic genesis, divided into observation and comparison groups with emerging (n=20) and mature (n=22) scars. In the course of the study, clinical, instrumental and medical and sociological research methods were used. RESULTS: A pronounced positive dynamic of indicators of clinical and instrumental parameters of scar tissue was established, improvement of quality-of-life indicators was the most significant in patients of the observation group with developing scars. The expediency of using the enzyme gel ultraphonophoresis technique with direct proteolytic activity in patients with hypertrophic skin scars is confirmed by a combination of parameters: positive dynamics of the clinical status, functional properties of scar tissue and quality of life indicators, a high safety profile and patient satisfaction with treatment. The effectiveness of correction in patients with developing and mature hypertrophic scars was 83–87%.
