Nwogu","doi":"10.5580/17d1","DOIUrl":"https://doi.org/10.5580/17d1","url":null,"abstract":"Thirty percent of patients undergoing neoadjuvant chemoradiation therapy (neo-CRT) for locally advanced esophageal cancer achieve a complete pathologic response (CPR) based on standard Hematoxylin and Eosin staining (CPR-HE). There is controversy regarding the prognostic value of CPR-HE. Immunohistochemical staining may be useful to detect subtle microscopic residual disease in such patients. We undertook this present study to test this hypothesis. Eighteen consecutive patients who achieved CPR-HE were identified. Seven patients had positive cytokeratin staining; three had microscopic residual disease that was not identified on original hematoxylin-eosin sections. Three of the seven patients (43%) with positive cytokeratin staining had systemic relapse (60 % of patients with systemic relapse).Cytokeratin staining could be a useful adjunct to routine hemaoxylin-eosin staining to identify residual tumor in such patients. It is a very rare tumor with an annual incidence of approximately 0.42 per 100,000. We describe two patients of Merkel cell carcinoma presented to us. Both patients were treated by surgery and chemotherapy. The response to treatment was very different in both the patients. Rashidi","doi":"10.5580/4e0","DOIUrl":"https://doi.org/10.5580/4e0","url":null,"abstract":"Vulvar and salivary gland involvement are rare in Langerhans cell histiocytosis (LCH), for which less than 60 and 5 cases have been reported in the literature, respectively. We report a 22-year old woman with LCH and multisystem disease including involvement of the brain (diabetes insipidus), lungs (pneumothorax), salivary glands and vulva (skin lesion). Vulvar lesion included a 3×2 cm erythematous and ulcerated plaque on the fourchette and a 1×1 cm similar lesion on the right labia major. The lesions were slightly infiltrated and without any discharge. The diagnosis was confirmed by skin biopsies positive by immunohistochemistry for S100 and CD68. The skin lesion and the neck mass regressed completely with systemic chemotherapy. B. Ngu","doi":"10.5580/17bf","DOIUrl":"https://doi.org/10.5580/17bf","url":null,"abstract":"Skin manifestations are frequently associated with the Human Immunodeficiency Virus (HIV) infection. Kaposi's Sarcoma (KS) is one of the most common cancers seen in people with HIV, and is an AIDS defining illness. The incidence of HIV associated Kaposi's Sarcoma has decreased since the advent of antiretroviral drugs in developed countries. This is not the case with Cameroon people present with generalized and aggressive KS. This study reviews the characteristics and prevalence of KS and the impact of antiretroviral drugs in a treatment center in YaoundE, Cameroon. The prevalence of KS among HIV infected patients was found to be 10.0%. Both sexes were equally affected unlike in endemic KS which is more common in males. Most patients had generalized disease, and death occurred within six months of diagnosis. These cases were associated with low CD4 cell count, anemia and low platelet counts. Patients with less extensive lesions, had tumor regression ranging from partial to total when Highly Active Antiretroviral Therapy was administered. The overall survival rate is 15-20%. Although incidence is declining, its prognosis remains poor. The etiological factors and pathogenesis of gastric cancer are not yet fully understood. The integrated research in molecular pathology clarified the details of genetic and epigenetic abnormalities of cancer-related genes in the course of development and progression of gastric cancer. Although epidemiological evidences indicate that environmental factors play a major role in the carcinogenesis, the role of immunological, genetic and immunogenetic factors are thought to contribute to etiopathogenesis of gastric carcinoma. In addition to better understanding of pathogenesis of gastric cancer, the incidence, diagnostic studies and the therapeutic options have also undergone important changes in the last decade. There is ongoing debate regarding the role of adjuvant treatment. In advanced disease, palliation of symptoms, rather than cure, is the primary goal of patient management. Several combination therapies have been developed and have been examined in phase III trials; however, in most cases, they have failed to demonstrate a survival advantage over the reference arm.
