More than 3,000 active pharmaceutical substances are being administered worldwide in prescription medicines, over-the-counter therapeutic drugs, and veterinary drugs. Their active ingredients comprise a variety of synthetic chemicals produced by pharmaceutical companies in both the industrialized and the developing world at a rate of 100,000 tons per year. Antimicrobial Resistance (AMR) accounts for an estimated 700,000 deaths per year worldwide. Over 23 million people have been tested with the aid of swab sticks for the novel Corona virus disease-19(Covid-19) in many parts of the world. This paints a gloomy picture on the quantum of pharmaceuticals that would have been used and eventually disposed. \u0000Purpose: This study explored and described pharmaceutical waste management practices by pharmacies and medicine stores operating in Bamenda Health District of the North West Region of Cameroon. \u0000Methodology: A mixed research design (Exploratory and Descriptive) was used. The study was cross-sectional. Exponential non-discriminative snowball sampling was used to identify 187 medicine stores while an official list from the Bamenda Health District of the NWR of Cameroon was sought to identify 13 pharmacies. Structured closed ended questionnaires and unstructured oral interviews were used to collect primary data. Descriptive statistics was used to describe the data while inferential statistics was used to test statistical significance. \u0000Results: In addition to sale of drugs, medicine stores offered services such as consultation, wound dressing, injection administration, laboratory tests, Rapid Diagnostic Tests (RDTs). Pharmacies carried out solely the sale of drugs. In Kenya malaria prevalence among adult females is 28% while in Mt. Elgon Sub County, which is categorised under highland epidemic prone malaria area, prevalence of malaria in pregnancy (MiP) is uncertain. Therefore, this study sought to determine intriguing predictors associated with malaria infection among pregnant women in Mt. Elgon Sub-County. \u0000Methods: Cross-sectional analytical design was employed and mixed methods used for data collection. For quantitative data collection, semi-structured questionnaire was used to collect primary data from pregnant women who attended ANC in selected health facilities in the study area. Further, qualitative approach adopted interview guides that targeted key informants in the health facilities. Systematic random sampling method was used to select 392 participants for the study. Data was analysed by SPSS version 25.0.  Bivariate logistic regression analysis was used to test the association between independent variable and dependent variable. \u0000Findings:  Study results revealed that, the prevalence of MiP was 16.2%. Prevalence was higher among women aged less than 25 (67.3%, n=262) compared to those aged > 25 years (33.7%, n=137). The following five factors were statistically significantly associated with malaria prevalence in pregnancy: place of residence (OR: 5.7; 95%CI: 2.6 – 12.4; p < 0.0001); those who tested positive in the last 2 years (OR: 1.7; 95%CI: 1.0 – 2.9; p = 0.05); preferred shape of ITN’s (OR: 3.8; 95%CI: 1.5 – 9.7; p = 0.008);earth floor (OR: 1.8; 95%CI: 1.0 – 3.1; p = 0.03);  mud wall (OR: 1.8; 95%CI: 1.0 – 3.1; p = 0.03). The World Health Organization (WHO) rolled out guidelines for treatment of HBV globally in 2015 and the Kenyan Ministry of Health started implementing these guidelines in 2018. About one thousand patients have been receiving the WHO-recommended treatment across the country. However, their treatment outcome has not been evaluated. Baringo County is one of the counties with high number of patients on treatment. The aim of this study, therefore, was to evaluate the treatment outcome of these patients. \u0000Methods: Forty-one HBV positive patients attending comprehensive care clinic in Marigat, Baringo County were randomly selected. Five millilitres of blood were drawn from each patient and processed into serum. Presence of hepatitis B surface antigen (HBsAg), surface antibodies (HBsAb), e-antigen (HBeAg), e-Antibodies (HBeAb) and core antibodies (HBcAb) were tested using lumiquick HBV-5 panel test kit (Diagnostic, lnc, USA). HBV DNA extraction was done using Qiagen extraction kit, amplified and visualized on 2% agarose gel. All positive extracts were sequenced. HBV viral load was done using TaqMan™.   HBV sequences obtained were aligned and phylogenetic trees developed. Genotypes were determined using geno2pheno software and mutations on the surface antigen and polymerase coding regions were determined from the aligned sequences. \u0000Finding: Out of 41 serum samples collected, 2.4% patient was HBsAg negative and the remaining (97.6%) positive. HBsAb positivity was 2.4%, all samples were negative for hepatitis B core antibodies (HBcAb), 14.6% tested positive and 35(85.6%) tested negative for HBeAg. The mean age (±SD) of patients was 33.59 ± 2.307 (8-80).  Ten (24.3%) samples were detectable in qPCR and their viral load ranged from 4.6×104 to 1.04×101 IU/ml with majority of them (85.6%) having HBeAg negative and 80.4% having HBeAb positive. Two genotypes, genotype D (50%) and genotype A (50%) were identified among the HBV isolates. This review aimed at investigating these challenges to understand their nature, cause, and the impacts they have on health service provision. \u0000Methods: Database search was made in Google scholar, ACM Library, PubMed health, and BMC public health, and a total of 123 papers were generated. Only 14 fitted the inclusion criteria. Inclusion criteria included studies that were both quantitative and qualitative addressing challenges facing referral systems or health referral systems, studies describing the barriers to effective referral systems, and studies describing factors that affect referral systems. The correct use of antimalarial drugs is the key not only to therapeutic success but also to deterring the spread of drug resistance malaria. \u0000Aims: To assess the physicians’ compliance with the national anti’-malaria treatment guidelines among Under five (U-5) years children in secondary health facilities in Sokoto metropolis. \u0000Methods: This was a cross-sectional study conducted as an exit interview among 292 mothers/caregivers of febrile U-5 children with antimalarial prescription, that presented to the out-patient clinics of the selected secondary health facilities in Sokoto metropolis. Simple random sampling technique using balloting option was used to select 2 secondary Health facilities in the metropolis, Proportionate allocation was done to allocate study subjects to the selected Health facilities based on average weekly outpatient attendance. The data was collected using an interviewer-administered questionnaire. Descriptive statistics was used to analysed data using IBM SPSS version 22. \u0000Results: Of the 292 children, only 115(39.4%) had malaria parasite test requested by the physician, out of which 63(54.8) had microscopy, while 52(45.2) of the children had malaria Rapid Diagnostic Test (mRDT).  One hundred and eighty-five (65.1%) of the children were treated presumptively with anti-malarial drugs. Artemisinin based Combination Therapy (ACT) was prescribed in 221(77.8%) of the under-fives. The commonest prescribed ACT was Artemether-Lumefantrine in 113(77.4%) of the children. Majority 263(92.6%) of the prescriptions were in brand names. \u0000Recommendation: The physicians’ compliance with national malaria treatment guidelines using the proportion of U-5 children was suboptimal. There was over prescription of anti-malarial drugs, as majority of the patients treated for malaria had no laboratory evidence for the treatment. However, compliance with the use of ACT was good. This study shows also suboptimal compliance with WHO prescribing indicators with respect to prescription in generic names. Omoshalewa","doi":"10.47672/ejhs.755","DOIUrl":"https://doi.org/10.47672/ejhs.755","url":null,"abstract":"Introduction:  Children are usually very vulnerable to such abuses by the traditional healers since consent is only obtained from the parents who believes in the benefits derived from traditional uvulectomy. \u0000Purpose: To assess the perception of traditional uvulectomy and associated factors among mothers of under five-year children (0-59 months) in Sokoto state.                           \u0000Methods: A cross sectional-study was conducted to assess the perception of traditional uvulectomy and associated factors among 320 mothers of under-five year children selected by multi stage sampling technique from 1st January-31st March 2021 in Sokoto State, Nigeria. A set of pre-tested, semi-structured interviewer- administered questionnaire was used to collect data on the research variables. Data was analysed using IBM® SPSS version 22 statistical package. \u0000Results: Majority 136(42.5%) of the respondents were between 25-34 years with a mean age of 29.9± 8.3 years. Most 304 (95.0) were Hausa’s and Muslims 315 (98.4). Fulltime house wives constituted 234(73.1%) of the respondents, while those with children between1-5 were 244(76.2%). Almost two-third (65.0%) of the respondents had inappropriate perception on traditional uvulectomy (TU). Two hundred and fifteen (67.2%) of the respondents believed traditional uvulectomy should be done for under five years’ children. Most (57.2%) of them perceived it is not necessary for government to come up with a law to abolish the practice of TU in the society. Majority (59.9%) of the respondents believed TU should be done for under five-year children as a tradition. Statistically significant association was observed between age, tribe and educational status of the respondents with perception of TU; (p=0.041), (p=0.002,) and (p = 0.018) respectively. \u0000Conclusion and Recommendation: This study revealed a high proportion of the respondents with inappropriate perception of traditional uvulectomy and attributed most of the childhood illness to the uvula. By the applicable method, the study was able to access policies, regulations, and contents/findings in existing literatures. The researcher curated data from existing literatures, reports and thus primary source of data were not utilised as only secondary source of data were used accordingly. \u0000Findings: The study found that pandemic is a sudden/unaware outbreak of a disease that speedily spreads across a whole region and the entire globe with high prevalence as a result of a high rate of susceptible population; the study also found that where pandemic occurs, the probability is high for there to be a high mortality rate. In the study, it is also found that at different stages in the development of modern man and society, different diseases had been categorised as pandemic. PPIs are important drugs used as supportive therapy for various medical conditions like heart-burn and acid peptic disease. There is an on-going deliberation regarding the probable useful versus deleterious effects of PPI use in patients with COVID-19 infection. Noaman","doi":"10.47672/ejhs.515","DOIUrl":"https://doi.org/10.47672/ejhs.515","url":null,"abstract":"Purpose: This study conducts an in vitro comparison of the depth of penetration of a resin infiltrant material (Icon), a two-step self-etch resin-based material (OptiBondTM XTR), and a solvent-free self-etch resin-based material (Bond-1SF) into white spot lesions (WSLs).Methodology: Through an in vitro clinical trial, a total of 60 human posterior teeth, free from caries, were collected. The buccal and lingual surfaces of each tooth were marked, resulting in a total of 120 samples of surfaces, which represented the operational sample size. The samples were randomly divided into four equal main groups (n = 30) according to the materials used. Group 1 (M1) was the control group. Group 2 (M2) was restored using Icon, Group 3 (M3) using OptiBondTM XTR, and Group 4 (M4) using Bond-1SF. Each group was divided into two equal groups (n = 15) according to the surface treatment, and the first group (W1) was subjected to surface treatment but the second group (W2) was not. The resin penetration depth was observed using confocal laser scanning microscopy.Findings: The highest penetration depth was recorded for Group 2 with surface treatment (M2W1), and the lowest was recorded for Group 4 without surface treatment (M4W2). An analysis of variance test revealed a statistically significant difference in the penetration depth of tested materials (p < 0.05).Conclusion: Icon shows the best penetration depth value in relation to WSLs, whereas Bond-1SF shows the worst. Overweight and obesity are the fifth leading risks for global deaths (WHO, 2014).  In June 2013, the American Medical Association recognized obesity as a disease (WHO, 2014. In Cameroon, the obesity figures stood at 9.6% second in CEMAC after Gabon which was 15% in 2014 (WHO 2014). In addition, 44% of the diabetes burden, 23% of the hypertension burden and between 7% and 41% of certain cancer burdens and 60% of Cardiovascular Disease in Cameroon were attributable to overweight and obesity in that same year (WHO, 2014).Purpose: This study purports to calculate the obesity QALY and ACUR and to scrutinize the effect of obesity on mobility index and age at death in the North West region of Cameroon.Methodology: To calculate the QALY and ACUR, data was collected secondary source making up 149 individuals aged 50 and above who died in the Bamenda general hospital from obesity related illnesses between 2016 and 2018. To generate the mobility index, we issued and corrected the EQ-5D-5L questionnaire for testing quality of life to238 individuals in the Bamenda Municipality. To ensure the reliability of the instrument, Pre-tests were conducted. Descriptive and inferential statistics were used for the calculation of QALY.Findings: The Average cost utility ratios’ (ACURs) show that obesity treatment in the Bamenda Municipality will result in a 0.275 QALYs gained and monthly ACURs of $3,578.18/QALY. It also shows that obesity significantly and negatively influences mobility index and age at death. We thus conclude that obesity incurred major economic cost.Unique contribution to theory, practice and policy: Policy wise, the “2M (Mentality and Motivation) concept” of obesity treatment is recommended as major contributions to theory, practice and policy.
