The posture is the convergence of muscular activity (tonic and phasic) and is related to a cranio-cervico-mandibular balance. On an anthropologic plan, postural stability is a flexible concept due to many ways for a human being to maintain around his balanced position. The posture aims to allow the movement of a body segment whereas it stabilizes the others segments and insures the maintaining of the erected station and fights against gravity. Regarding the literature there is no real consensus on a link between dental occlusion and posture when using measuring instrument to evaluate it. The different studies are contradictory. During the Milan consensus conference in 1997, it has been reported that there was no scientific proof to support a link between occlusion and posture [1]. Perinetti in 2006, has shown on a 26 patient’s cohort, with the help of posturography, the lack of postural modifications when the occlusion was changing [2]. Mandredini, et al. [3] has demonstrated throughout a systematic review that different techniques and posturoraphic devices failed to detect an association between body posture and dental occlusion. In some cases a link was detected, but it couldn’t be statistically significant [4]. On the one hand, patients expect a painless and comfortable dental treatment, which becomes crucial especially in subjects suffering from dental phobia [1]. On the other hand, the skill of a dentist or oral surgeon himself is often measured by the ability of achieving profound and successful local anesthesia and pain relief. There are different, more or less known options of local anesthesia in the dental area, first of all infiltration anesthesia (IA) as well as the inferior alveolar nerve block (IAN). While IA interrupts the pain perception in the area of injection, the second mentioned technique is related to the innervation area of the mandibular nerve. Indeed, beside the interruption of pain perception, both local anesthetic methods block also the function of sensitive and motor nerves [2]. As a consequence patients experience loss of sensitivity as well as total or partial loss of function of facial muscles and/or the tongue for the duration of the anesthesia [3]. The result is a temporary restriction of patients’ daily life with reduced ability in speaking and eating, but also the risk of bite and burn injuries. Patients with intellectual disability are often a forgotten group when it comes to oral healthcare although they may require complex multidisciplinary care [1-3]. The evidence shows that persons with intellectual disability suffer worse oral health, periodontal status, and present a higher risk of caries [1-3]. In addition to compromised oral health status, these patients may also have greater difficulty in accessing oral healthcare services [1-6]. Nicola, Capurro Claudia, Merlino Emilia, Chiappe Marco Davide Giacomo","doi":"10.15761/OHC.1000191","DOIUrl":"https://doi.org/10.15761/OHC.1000191","url":null,"abstract":"The dentigerous cyst (DC) is one of the most common cystic lesions that can be observed in the jaw and is associated with the crown of an included or impacted tooth. Radiographically it appears as a unilocular radiolucency surrounding the crown of an included or impacted tooth with a well-defined sclerotic edge. The pathogenesis of DC is controversial, in most cases it is of disembriogenetic origin, but it can also have inflammatory pathogenesis starting from a compromised deciduous element. The DC has development and asymptomatic course, for this reason it can reach considerable dimensions, involving adjacent noble structures, can lead to dislocation of contiguous dental elements or to deformation of the marginal bone profile. The therapeutic options are: surgical excision of the cyst or marsupialization. The objective of this study is to demonstrate how marsupialization is a conservative treatment for the dental element involved in the cystic lesion, able to promote its spontaneous eruption. The authors report 4 cases of large-sized dentigerous cysts, localized apically to infected deciduous elements, with dislocation of the immature permanent teeth involved. The cases were treated by extraction of the deciduous elements and marsupialization of the cystic cavity. After surgery, radiographic checks were performed through Orthopantomography. According to the 2011 census in Hungary their number can be around 500.000. These people are considered as a vulnerable group even from a dental point of view, since the loss of function makes regular oral hygienic routine and dentistry control more difficult. Aim: In 2015 The Department of Community Dentistry and The National Institute of Medical Rehabilitation started to operate together a dental office. We gathered comprehensive information on patient’s oral health currently undergoing rehabilitation and living with disabilities. Method: This study was approved by the Medical Research Council, Hungary. The clinical examination were done by the recommendation of WHO. A total of 608 inpatients have gone through a full dental examination including a stomato-oncological screening. We analyzed the DMF-T values. Oral hygiene routine, dental care, demographic characteristics, self-reported smoking, drinking and eating habits related median DMF-T were surveyed. Results: The patients had poor oral hygiene. The median DMF-T was 24 in age group ’55 years or older’. Their complaints about dental and oral mucosal lesions are minimal compared to their general dental condition. 28.91% of the inpatients reported toothache in the past 12 months, whose median DMF-T was 19. The last dental checkup in the case of most patients exceeds 24 months. Conclusion: All of the values are worse than the average Hungarian population. For deeper color changes, peroxide bleaching techniques are used. These methods are based on the use of hydrogen peroxide applied directly to the tooth surface, or produced by a chemical reaction of carbamide peroxide or sodium perborate [1]. In spite of peroxide efficacy, adverse effects have become evident. Cervical root resorption and tooth sensitivity are possible consequence of internal bleaching and are more frequently observed in teeth treated with the thermo-catalytic procedure [2].
