The 6-minute walk test (6MWT), timed up and go (TUG), timed stand test and Romberg's test were used to evaluate physical fitness. The Borg scale was used to control the workload. The patients bicycled for 12-40 min with increasing workload according to their capabilities. Wilcoxon signed ranks and McNemar's tests were used for analysis.</p><p><strong>Results: </strong>Of the 21 participants, 12 completed 3 months of training. Their median (range) age was 69 (37-88) years, duration of dialysis was 2.5 (1-11) years and body mass index was 25.0 (20.0-31.9). Romberg's test was positive in four participants at the beginning but in three after 3 months. Nine participants completed 6 months of training; none had a positive Romberg's test at that time. Walking distance increased significantly after 3 and 6 months of training (p = 0.002 and p = 0.012, respectively), and time for the TUG decreased significantly (p = 0.041 and p = 0.044), as did time for the timed stand test (p = 0.015 and 0.018), compared with baseline values. This article reports the case of a previously healthy 37-year-old man presenting with acute-onset, severe, bilateral flank pain. Initially treated for ureteral colic and urinary tract infection, he was transferred to the nephrology clinic upon recognition of a rising serum creatinine. He was found to have FMD of bilateral renal arteries with a stenotic pattern on the right side and a dissecting aneurysm on the left side with resultant infarctions in both kidneys. On the basis of negative serological markers of vasculitis, a diagnosis of FMD complicated by bilateral renal infarctions was made. A stent was placed to the right stenotic renal artery, which resulted in sufficient lumen patency. No invasive procedure was performed on the other side owing to the complexity of the lesion. After 2.5 years of follow-up, the patient remained in good condition with normal renal function and adequate blood pressure control with dual antihypertensive therapy. Renal infarction complicating FMD of renal arteries is rare in the literature, with most of the cases having causative cardiovascular risk factors including coagulopathy, ischaemic heart disease, atrial fibrillation or structural cardiac abnormalities, none of which was present in this case. In conclusion, FMD may occur in atypical asymmetric presentations causing renal infarctions in both kidneys. The aim of this study was to assess and quantify changes in voiding parameters and prostate size in men with prostate cancer from before the start of endocrine treatment and during long-term follow-up. Material and methods. Seventy-seven patients were recruited from three clinics and followed prospectively until death, clinical deterioration making the patient unfit for participation, or the end of the study. Median age was 74 (range 54–85) years, and the median follow-up was 18 (3–90) months. Parameters and endpoints were total score on the Danish Prostatic Symptom Score (DAN-PSS-1) questionnaire, maximum flow rate, postvoid residual volume, frequency and voided volume, and prostate volume on transrectal ultrasonography. Results. All parameters improved significantly in the range of median 13–50% within the first 12 months. The greater part of the effect occurred during the first month, and thereafter the improvement rate slowed down. Intervention for local progression was estimated on Kaplan–Meier analysis to be about 20% after 4 years. 73% had a defined prostate-specific antigen nadir after a median of 6 (1–60) months with scheduled assessments up to 72 months after the nadir. All parameters were improved before the nadir and the improvement remained during biochemical progression except for the very latest visits where few patients contributed to the analyses. Conclusions. Androgen deprivation therapy improved lower urinary tract symptoms, objective voiding parameters and prostate volume in patients with prostate cancer who were not candidates for curative treatment. The improvement was significant within the first month and clinically relevant. The bladder was first examined in standard white light followed by PDD.</p><p><strong>Results: </strong>PDD was superior to white light diagnosis; PDD was positive in 16 patients (22%) where white light showed a normal bladder mucosa. Four of these patients had bladder tumour [4/73, 6%; two carcinoma in situ (CIS), two Ta]. The diagnosis was verified by transurethral resection of the bladder tumour in the operating room. In 20 patients (20/73, 27%) PDD identified additional tumour lesions that were not identified in white light (five CIS, 15 Ta). The false-positive detection rate of PDD was 0.41. False positivity was significantly reduced by simultaneous flex biopsies disproving malignancy. Biopsies were obtained from 57 patients and diagnosis of stage and grade were possible in 55 of these (97%). In two patients (4%) the tissue material was too small for diagnostic evaluation. Biopsies from 47 patients (83%) included muscularis mucosa and from 20 patients (35%) muscularis propria. After a flu-like prodrome, this viral disease often manifests with acute kidney injury. Use of non-steroidal anti-inflammatory drugs is discouraged during the disease, but no data from clinical investigations are available on the association between disease course and the use of analgesics.</p><p><strong>Material and methods: </strong>The charts of 59 patients admitted with NE to a hospital in 2007 were retrospectively analysed. Creatinine levels were compared between users of different analgesics.</p><p><strong>Results: </strong>Patients taking analgesics before admission had higher peak creatinine levels, but not after adjustment for confounders. Serum values of inhibin B reflect the state of germinative epithelium in cryptorchid testes. The aim of the study was to evaluate whether a simple blood sample of gonadotropins and inhibin B could diagnose bilateral vanished testes.</p><p><strong>Material and methods: </strong>Group I included five boys (4 months to 6 years and 3 months old) with bilateral vanished testes at laparoscopy. Group II included 82 boys with bilateral cryptorchidism younger than 7 years of age at surgery for bilateral cryptorchidism (median age 1 year and 9 months).</p><p><strong>Results: </strong>The serum levels of hormones for the patients with vanished testes were: inhibin B 5?18 pg/ml, FSH 41-191 IU/l and LH 3.9?56 IU/l. The patients all had karyotype 46,xy. The serum levels of hormones from group II were: inhibin B median 122 (range 20?404) pg/ml, FSH median 0.8 (range 0.2?3.5) IU/l and LH median 0.2 (range 0.1-3.2) IU/l. The serum levels of inhibin B, FSH and LH from the boys with vanished testes were significantly different from the serum levels of the boys with bilateral cryptorchidism (p = 0.0026, p < 0.0001 and p < 0.0001, respectively).</p><p><strong>Conclusions: </strong>The serum values of gonadotropins and inhibin B from boys with bilateral vanished testes were significantly different from those of bilateral cryptorchid boys, indicating no germinative epithelium, no Sertoli cells and compensatory high gonadotropins. It is a carbon dioxide-requiring Gram-positive rod which is overlooked if urine is cultured in ambient air or if there is growth of conventional species. This study aimed to find the frequency of A. schaalii in consecutive cohorts of patients with kidney stones, children with suspected UTI and patients with indwelling catheters.</p><p><strong>Material and methods: </strong>A quantitative real-time polymerase chain reaction (PCR) assay was used to screen consecutive urine samples from of 76 patients with kidney stones, 29 children and 37 with different indwelling catheters.</p><p><strong>Results: </strong>In patients with kidney stones, A. schaalii was found in seven (29%) of the 24 leucocyte esterase stix-positive urines, which was twice as often as in the stix-negative urines (p = 0.22), and in five (36%) of 14 children less than 3 years old but not in 15 children 3?15 years old (p = 0.02). The eight catheterized patients with A. schaalii (22%) were elderly and half had comorbidities. Questionnaire forms, Pittsburgh Sleep Quality Indexand Piper Fatigue Scalewere used for data collection. Mann-Whitney U test, Kruskal-Wallis variance analysis, ttest, one-way analysis of variance and Pearson correlation coefficient for independent groups were used for the evaluation of data.</p><p><strong>Results: </strong>Mean total fatigue and sleep quality scores (±1 SD) were 6.38 ± 2.18 and 8.17 ± 3.02, respectively. Patients in the study had severe fatigue and low sleep quality.</p><p><strong>Conclusions: </strong>Fatigue, sleep quality and related factors were determined in patients receiving peritoneal dialysis. {"title":"Incidence of occult lymph-node metastasis missed by standard pathological examination in patients with bladder cancer undergoing radical cystectomy.","authors":"Jørgen Bjerggaard Jensen,&nbsp;Søren Høyer,&nbsp;Klaus Møller-Ernst Jensen","doi":"10.3109/00365599.2011.599336","DOIUrl":"https://doi.org/10.3109/00365599.2011.599336","url":null,"abstract":"<p><strong>Objective: </strong>The aim was to conduct an exploratory study of the ability of standard pathological examination (SPE) of lymph nodes (LNs) to detect metastasis in LN specimens from patients with bladder cancer undergoing radical cystectomy (RC).</p><p><strong>Material and methods: </strong>From a cystectomy database, 10 LN-negative patients with invasive bladder cancer were randomly selected based on their Tstage (two T1, four T2 and four T3). LN specimens were step sectioned and stained by immunohistochemistry to identify occult metastases missed by the SPE.</p><p><strong>Results: </strong>In total, 173 LNs negative by SPE were investigated. Metastasis was identified in one LN in one patient with non-organ-confined disease. No other positive LNs were found. Thus, SPE had a negative predictive value of 99.4% per LN (95% confidence interval CI96.8?99.99%) in this small series.</p><p><strong>Conclusions: </strong>Occult nodal metastasis can be overlooked by SPE in a minority of patients undergoing RC because of bladder cancer. This is presumably more likely in patients with non-organ-confined tumours because of a higher risk of LN metastasis in this patient category. Therefore, the importance of a thorough LN dissection in presumably LN-negative patients is emphasized. {"title":"A prospective registration of catheter life and catheter interventions in patients with long-term indwelling urinary catheters.","authors":"Karin Jonsson,&nbsp;Anna-Lena E-Son Loft,&nbsp;Salmir Nasic,&nbsp;Hans Hedelin","doi":"10.3109/00365599.2011.590998","DOIUrl":"https://doi.org/10.3109/00365599.2011.590998","url":null,"abstract":"<p><strong>Objective: </strong>The aims of this study were to register the incidence of scheduled and acute urinary catheter changes and rinses (acute interventions) among nursing home patients, to relate the incidence of acute interventions to catheter material and time of catheterization, and to register the use of antibiotics for catheter-associated urinary tract infections.</p><p><strong>Material and methods: </strong>Catheter life and catheter-related interventions were followed prospectively for 1 year in all patients with long-term indwelling catheters in all 78 nursing homes in a county in western Sweden.</p><p><strong>Results: </strong>Altogether, 366 patients were followed: 117 (32%) women and 249 (68%) men. Acute changes (n = 718) were more common than scheduled ones (n = 519). The rate of acute interventions was not related to catheter material and was significantly lower in patients with a catheter for over 2 years. In 25% of the patients, acute interventions were virtually never necessary, in contrast to 10% where acute interventions were registered nearly every month. Antibiotic treatment for reasons assumed to be related to the urinary tract was instituted on 170 occasions among 85 men (34%) and 20 women (17%), a significant difference between the genders (p < 0.01). CONCLUSIONS; No catheter material appeared to be superior. The surprising finding that acute interventions were less common after 2 years' catheterization needs further study to be verified and explained.
