Prognostically favourable indices were: absence of clinical symptoms (27% survival) and a peripheral site of tumour in the lung (28% survival). Duration of symptoms, when present, had no consistent influence on prognosis. Resection could be done in 419 cases. In this group the 5 years survival rate was strongly related to the extent of surgery and the completeness of resection. Cases with a radical lobectomy had a survival rate of 52% against 9% only in patients with a pneumonectomy that might not have been radical. Of 33 cases of resected small cell anaplastic cancer, 10 survived more than 5 years. This unexpectedly high survival rate persisted after revision of the histological typing and may justify an active surgical attitude even in this group of patients. Although hardly decisive in this context, confusion between the comprehensive term small cell carcinoma and its subtype, oat cell cancer, should be avoided.</p>","PeriodicalId":21508,"journal":{"name":"Scandinavian journal of respiratory diseases","volume":"60 5","pages":"297-302"},"PeriodicalIF":0.0,"publicationDate":"1979-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"11728816","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Estimated percentage distribution of Pi-phenotypes in the target population aged 15--70 years was M 87.30%, MS 4.65%, MZ 4.73%, FM 2.69%, SZ 0.13%, IM 0.20%, FZ 0.07%, S 0.06%, FS 0.07% and Z 0.06%. The distribution curve of AT had a normal (Gaussian) shape and the ranges of AT demonstrated great overlap of types MS and MZ with type M. In subjects with phenotype MZ neither respiratory symptoms nor physicians' diagnoses of chronic obstructive lung disease (COLD) were more frequent than in M subjects. Physicians' diagnoses of COLD were slightly more frequent (0.06 greater than P greater than 0.01) in subjects with phenotype MS than M, probably due to there being more smokers in the MS group. Spirometric variables given as per-cent of predicted values yielded large differences between smokers and non-smokers but no differences among phenotypes M, MS and MZ. Radiologic signs of hypertransradiancy and/or emphysema were evently distributed in M, MS and MZ subjects. The only subject observed with Pi-type Z and one out of three subjects with type SZ had COLD. Most of the ABPA cases (60%) diagnosed had an early onset of respiratory symptoms, i.e. below the age of 30 years, while most of ABPC cases (69%) had a late onset of respiratory symptoms, i.e. after the age of 30 years. The precipitin bands in ABPA and ABPC were R-type and H-type, respectively. Apparently, ABPA and ABPC are independent of one another in origin as suggested by specific precipitins and dual skin reaction. In ABPA, A. fumigatus appears to be the primary causal organism although the contributory role of other species of Aspergillus, which include A. flavus, A. nidulans, A. terreus and A. niger, is evident from the present study. A model of atelectasis was made either by simple collapse or by applying a positive pressure to the pleura and a negative to the airway. Five different ventilatory patterns were used for reinflation: simulated normal breathing with and without continuous positive airway pressure (CPAP), simulated deep breathing and mechanical ventilation with and without positive end-expiratory pressure (PEEP). All methods, except normal breathing without CPAP, reinflated the collapsed part with pressures well within the range used clinically. The most effective way of re-expanding collapsed lung was the application of CPAP during simulated normal breathing or PEEP during mechanical ventilation, which required smaller transpulmonary pressure swings than the other methods. A comparison between CPAP and PEEP showed CPAP to be preferable. Collateral reinflation occurred just as readily as normal reinflation and the results suggest that collateral reinflation is the primary choice. This route of reexpansion also has a potential secretion clearing effect in that pressure is built up distal to an obstruction.</p>","PeriodicalId":21508,"journal":{"name":"Scandinavian journal of respiratory diseases","volume":"60 5","pages":"260-6"},"PeriodicalIF":0.0,"publicationDate":"1979-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"11600334","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In all cases the diagnosis was made from clinical symptoms and confirmed by chest X-ray and biopsy. In five of the patients a fourfold rise or decrease was measured in the antibody titre against Yersinia enterocolitica, serotype 3, confirming an acute infection. In another two patients this titre remained significantly elevated greater than or equal to 80, indicating chronic infection. Five patients experienced joint symptoms and four erythema nodosum, symptoms common to both yersiniosis and sarcoidosis. A follow-up examination 14 to 41 months after the debut of sarcoidosis showed total remission (including chest X-ray and respiratory function) in five of the seven patients. A simple laboratory test of huC consumption by pigeon antigens is described, revealing high huC sensitivity in almost all sera of pigeon breeders with manifest or former disease. False-positive results were seldom observed. Discontinuation of antigen exposure and/or corticosteroid therapy had no effect on the huC consumption level in the test system, despite precipitin titres falling below the level of detection. Titration studies revealed an initial decrease of the huC-consuming serum factors, but titres settled at a definite constant level, even after antigen avoidance for many years.
